Clinician Choices Often Differ From Those of Experts When Switching HIV Treatment in Patients With Virologic Suppression

Healthcare providers show marked differences in regimen choice compared with experts when selecting a switch regimen for patients requiring a change in ART for reasons other than virologic failure. The finding was derived from an analysis of data captured between March 2018 and April 2019 from an online treatment decision support tool developed by Clinical Care Options (CCO). The tool allows users to define a specific patient case scenario based on variables such as current regimen, treatment history, and patient characteristics and comorbidities and then to instantly see which switch regimens are recommended for that exact scenario by a panel of 5 leading HIV experts.

In nearly two thirds of the 1098 cases entered into the online tool, healthcare providers were planning to switch to regimens that differed from those selected by experts based on cardiovascular disease, the presence of mutations associated with HIV drug resistance, or the patient’s HLA-B*5701 status.

“Our data show not only that there is a major discrepancy between switch decisions taken by community clinicians compared with experts, but also that an online decision support tool can help guide clinicians to better choices. In many cases where clinicians initially selected an option that differed from the experts, they indicated that they would change their treatment plan after using the tool,” said Edward King, MA, Executive Vice President of CCO.

The Arc Westchester Foundation to Host 16th Annual “A Matter of Taste” on October 22 in New Rochelle


For more than 15 years, the funds raised at this event have helped support the innovative and comprehensive programs that The Arc Westchester provides for individuals with intellectual and developmental disabilities throughout our community.

The Arc Westchester Foundation, which raises public awareness and financial resources for The Arc Westchester’s programs and services for people with intellectual and developmental disabilities and their families, will host its 16th annual “A Matter of Taste” fundraiser on Tuesday, October 22, at Glen Island Harbour Club in New Rochelle. The event, which raised nearly $400,000 in 2018, will feature signature cuisine prepared by the finest chefs in the area, accompanied by a wide assortment of fine wine.

“A Matter of Taste” will include two awards of recognition: the Family Partner Award, which will be given to the Kurzer Family, and the Corporate Partner Award, which will be presented to Dan Molino, President of Grassy Sprain Pharmacy.

Rx Drug Abuse & Heroin Summit Announces Move to Nashville in 2020

HMP, a leading healthcare event and education company, today announced that the 9th annual Rx Drug Abuse & Heroin Summit—the nation’s largest meeting addressing the opioid epidemic—will take place for the first time in Nashville, Tennessee, April 13-16, 2020.

Founded in 2012, the Rx Drug Abuse & Heroin Summit is the largest annual gathering of professionals from “federal to family”— local, state, and federal agencies, public health officials, business executives, academia, treatment providers, advocates, law enforcement, and family members working to find solutions that address the prescription and illicit drug abuse and misuse epidemic. The Summit attracted a record 3,500 attendees in 2019.

Highlighting the 2019 conference were remarks by President Donald J. Trump and First Lady Melania Trump. President Trump was the Summit’s second sitting president to deliver an address, following President Barack Obama in 2016, underscoring the gravity of the opioid crisis and the government’s commitment to fighting it.

The misuse of and addiction to opioids—including prescription pain relievers, heroin, and synthetic opioids such as fentanyl—is a serious national crisis that affects public health as well as social and economic welfare. There were 70,237 drug overdose deaths reported in the United Sates in 2017; a 9.6% increase from 2016. Promising new research suggests, however, that overdose trends may be starting to reverse. The Centers for Disease Control and Prevention recently released preliminary 2018 data showing a decrease in opioid-related overdose deaths for the first time in nearly three decades.

“The opioid epidemic does not discriminate. It touches every community across our nation, directly or indirectly, and combatting it requires a holistic approach,” said Nancy Hale, president and CEO, Operation UNITE, the educational advisor for Rx Summit. “When we started Operation UNITE 17 years ago, there were limited resources. The growth and development we’ve seen in programs across the nation is strongly supported by the annual Rx Summit. New Study Discovers the Three-Dimensional Structure of the Genome Replication Machine


Mount Sinai researchers have discovered how the enzyme DNA polymerase delta works to duplicate the genome that cells hand down from one generation to the next. In a study published in Nature Structural & Molecular Biology, the team also reported how certain mutations can modulate the activity of this enzyme, leading to cancers and other diseases.

“DNA polymerase delta serves as the duplicating machine for the millions to billions of base pairs in human and other genomes. We were able to present for the first time a near-atomic-resolution structure of the complete enzyme in the act of DNA synthesis,” says lead investigator Aneel Aggarwal, PhD, Professor of Pharmacological Sciences at the Icahn School of Medicine at Mount Sinai. “This knowledge furthers our basic understanding of this complex enzyme which is essential for survival in higher organisms from humans to yeast. At the same time, our work provides insights into how cancers can arise when DNA polymerase delta is not functioning properly, and offers a novel basis for designing inhibitors of the polymerase that could potentially serve as effective treatment in certain cancers.”

While DNA polymerase delta has been studied by scientists for decades, many questions remain about its overall architecture and dynamics. “We showed how the various pieces of this complicated machine work synchronously with one another to copy the genome with amazing accuracy,” explains Dr. Aggarwal. His team, which included co-author Rinku Jain, PhD, Assistant Professor of Pharmacological Sciences at the Icahn School of Medicine, also mapped a number of inherited mutations (which are passed down from parent to child) and somatic mutations (which occur by chance during someone’s lifetime) in DNA polymerase delta that are associated with “hypermutated” tumors. In addition to cancers, these mutations may be associated with multi-symptom mandibular hypoplasia, deafness, and lipodystrophy syndrome.

Essential to the Mount Sinai researchers’ work were recent advances in cryo-electron microscopy. This technology, which allows for the imaging of rapidly frozen molecules in solution, is revolutionizing the field of structural biology through its high-resolution pictures. This technique allowed Dr. Aggarwal and his team to examine not only individual atoms of the DNA polymerase delta but also how they move to achieve accurate replication of the genome. Integral to this phase of the research was Mount Sinai’s partnership with the Simons Electron Microscopy Center in New York City.

Building on its latest groundbreaking work around DNA polymerase delta, Mount Sinai will continue to explore the unique structure and mechanism of the polymerase, particularly its relationship to cancer and disease pathogenesis. “We know that certain cancers become dependent on this enzyme for their survival,” says Dr. Aggarwal, “and inhibiting its activity could provide a valuable therapeutic window for future medical research.”

United Church of God gathers in worldwide assembly to celebrate coming time of Christ-centered global peace

These ancient holy days reflect personal and global transformation.

Advancing a dynamic life journey as dedicated spiritual pilgrims, about 14,000 members and families of the United Church of God, an International Association, will gather to observe the Christ-centered biblical Festival of Tabernacles October 14-21, an eight-day season that portends a future time of unprecedented peace. Church members follow the historic example of Jesus Christ and the first century Church in observing this time of celebration, which biblically reflects the coming Kingdom of God as prophesied by Jesus Himself.

According to Victor Kubik, church president, United Church of God members will assemble at more than 60 sites in the United States, Canada, Asia, Africa, the Caribbean, Europe and Latin America to take part in a contemporary Christian application of the biblical ancient Hebrew Holy Days.

The festival season reflects the goal of a life-long spiritual journey of Christians, who are taught by Jesus from the Bible to “seek first the Kingdom of God” in the book of Matthew, chapter 6, verse 33. “Followers of Jesus are generally described in the Bible as disciples—committed people who confidently spend their life apprenticed to learning the ways of Jesus Christ—and pilgrims—people who regard their current life on earth as temporary and one that represents a deliberate journey toward receiving eternal life,” explained Mr. Kubik.

The church president also pointed out the 14th chapter of the Old Testament book of Zechariah, which predicts a future global restoration and celebration of the Festival. Those alive on earth after the coming return of Jesus Christ as King of Kings (Revelation 19:16) “will go up year after year to worship the King, the Lord Almighty, and to celebrate the Festival of Tabernacles” (verse 16, New International Version).

“We read in the New Testament book of Hebrews that many disciples over the years died in the faith, but died confident of their eternal reward, recognizing that they were but ‘strangers and pilgrims’ on the earth,” he continued, citing Hebrews chapter 11 in verse 13. “The annual observance of the Festival of Tabernacles is rich with spiritual parallels as members journey to distant festival sites to reside in hotel rooms or camps as temporary dwellings.”

“Many scholars confirm that the early Christian church, whose members originally were mostly of a Jewish background, continued to observe the ancient Holy Days in a new spirit and with a new Christ-centered understanding of their meaning,” said Mr. Kubik. “Today we have the marvelous understanding of how these ancient Holy Days reflect personal and global transformation – and how they represent the important message brought by Jesus Christ which outlines God’s incredible plan for all humanity—including a future time where humanity is freed from concerns and troubles like nuclear war and global conflict.”

“We understand from many biblical references that the fall Holy Day seasons—basically outlined in the 23rd chapter of the book of Leviticus—actually prophetically picture forthcoming global events that will completely transform the world as we know it today,” Mr. Kubik added.

Daily worship services (typically in the morning) present powerful sermons and Bible studies that magnify and explain these spiritual principles. The eight-day period also features many family activities and opportunities for fellowship, Mr. Kubik noted.

In the United States, members will gather at 18 sites: Bend-Redmond, Oregon; Big Sandy, Texas; Branson, Missouri; Cincinnati, Ohio; Glacier Country, Montana; Jekyll Island, Georgia; Lahaina, Maui, Hawaii; Lake Junaluska, North Carolina; Lancaster, Pennsylvania; Oceanside, California; Panama City Beach, Florida; Phoenix, Arizona; St. Petersburg, Florida; Steamboat Springs, Colorado; Tampa, Florida; Tucson, Arizona; Wisconsin Dells, Wisconsin. The Church will also conduct the festival at five Canadian sites (Blainville, Quebec; Cochrane, Alberta; Midland, Ontario; Osoyoos Lake, British Columbia; Spencerville, Ontario; and St. John’s, Newfoundland) as well as sites in the Caribbean, China, Latin America (six sites), Europe and the British Isles (six sites), Africa (10 sites), Australia (four sites), Malaysia, New Zealand, the Philippines, and Sri Lanka.

Mr. Kubik will address all church members through a special live webcast originating from the Panama City Beach festival site on Saturday, October 19.

How do New Testament disciples observe these days? Church members typically save about a 10th of their income for use at the festival (also called the Feast of Booths in various English translations). These personal funds allow members to fully celebrate the eight-day biblical event, as the Bible specifically directs members to “rejoice before the Lord your God” during this season (see Leviticus 23:40 for details). This helps spiritually foreshadow the coming abundance inherent in the future Kingdom of God. Church members attend the festival with their families, and the Church provides many opportunities for family time together. The worldwide nature of the festival also gives rise to global fellowship and travel, and many friendships are made or renewed each season.
