The green wall is a vital part in fulfilling our commitment to the community that the design of the new substation would revitalize the area’s landscape, establish an inviting gateway to REO-Town, and reconnect the area to downtown.

The Lansing Board of Water & Light’s new Central Substation in the city’s historic REO Town neighborhood features a two-sided, 425-square-foot living wall installed with the LiveWall® Outdoor Living Wall System. Completed in July 2019, the living wall greens up the exterior of the new substation and establishes a visual link to the Scott Sunken Garden.

“The Central Substation provides more reliable, affordable power to downtown Lansing,” said Dick Peffley, general manager, Lansing Board of Water & Light (BWL). “The green wall is a vital part in fulfilling our commitment to the community that the design of the new substation would revitalize the area’s landscape, establish an inviting gateway to REO Town, and reconnect the area to downtown.”

“We envisioned the living wall as a green focal point that softens the view of the Central Substation’s brick façade, which people see as they drive into or exit downtown,” said Bob Ford, ASLA, principal, Landscape Architects & Planners, Inc., the landscape architecture firm for the project. “It also serves as a prominent landscape feature that visually connects the substation to the historic Scott Sunken Garden, which was saved and relocated as part of the project.”

“As seen from the Sunken Garden, the living wall is an extension of the garden landscape and unifies the overall landscape,” said Paul Toaz, landscape manager, Lawn Sprinklers Sales, Service & Design, Inc., the project’s landscape contractor and living wall installer. “For example, when you exit from the Sunken Garden parking lot, you face the west section of the living wall and see its natural beauty.”

The LiveWall Living Wall was installed in two sections on the north and west walls of the Central Substation. Each section measures 21’-4” x 10’-3 3/8” and fits into specially recessed frames built into the substation’s brick walls. In total, it includes 330 LiveWall modular planter boxes, which are high-impact, UV-resistant, architectural quality moldings. Manufactured in a cedar color to complement the color of the bricks of the substation’s exterior walls, the planter modules contain re-usable inserts that hold the growing medium for the plants and contain a mix of six different annuals.

Delaware Valley Accountable Care Wins 2019 Doyle Award for Innovative Post-Acute Care Strategy

Having MCG’s care guidelines as evidence-based tools to help guide our care coordination efforts with our post-acute care partners are some of the important components that help drive our success.

MCG Health, part of the Hearst Health network and a leading provider of informed care strategies, has named Delaware Valley Accountable Care Organization (DVACO) a recipient of the 2019 Richard L. Doyle Award for Innovation and Leadership in Healthcare.

The Philadelphia-area ACO was recognized for implementing a unique post-acute strategy which significantly reduced patient length of stay in skilled nursing facilities (SNFs) and improved care coordination across the post-acute continuum of care. By leveraging MCG’s Chronic Care and Recovery Facility Care guidelines and a web-based tool to track their patients’ stays in post-acute care, Delaware Valley ACO reduced the average length of stay (LOS) by one full day and achieved a $1.3 million savings in Medicare expenditures. The ACO also decreased home health spend by 8% over the same timeframe. Since the ACO’s inception in 2014, they have been able to reduce overall SNF spend in their network by an impressive 30%.

Delaware Valley ACO’s strategic initiative used claims data to define the top 20 diagnoses for patients utilizing a SNF. The ACO then partnered with MCG to publish length of stay guidelines for these diagnoses and share them across their SNF network. This critical step encouraged proactive multi-disciplinary care planning at the time of admission. In turn, this helped to identify barriers to discharge earlier in the patient’s stay and initiate the proper actions to resolve them. Delaware Valley ACO’s SNF partners now proactively communicate with a patient’s primary care physician (and/or ambulatory care coordinator) to discuss clinical, social, medication, or palliative care goals.

“We learned pretty quickly that establishing preferred post-acute care networks was the easy part of our strategy; leveraging them to improve the care of our patients and improve value was more challenging,” said Beth Souder, PT, MSPT, the Director of Post-Acute Networks at Delaware Valley ACO. She added, “Having MCG’s care guidelines as evidence-based tools to help guide our care coordination efforts with our post-acute care partners are some of the important components that help drive our success.”

“We’re seeing more risk-bearing organizations implement evidence-based, informed care strategies across the full spectrum of patient care,” said MCG President and CEO, Jon Shreve. “Delaware Valley ACO’s results show that applying the evidence properly can positively impact coordination efforts between providers working across different settings. About the Richard L. Doyle Award for Innovation and Leadership in Healthcare

The Doyle Award was developed to recognize organizations that make innovative use of the care guidelines to help deliver effective healthcare. Dr. Doyle, the care guidelines founding editor, was a hospital chief-of-staff when he began creating clinical guidelines in the 1980s to help improve healthcare efficiency and quality at Mercy Hospital in San Diego. He later joined Milliman & Robertson and in 1988 published the first set of what was to become the care guidelines.

Judges for this award are independent healthcare quality experts, not currently associated with MCG. Applicants were judged on how well their projects supported the MCG mission to help drive effective care, and the names and locations of the nominees were redacted prior to the judges’ review. Judges looked for evidence of improvements in healthcare quality and patient safety; patient/member satisfaction; staff efficiency, productivity and satisfaction; internal/external communication; and effective use of resources.

PROJECT OUR TOWN is a Denver-based 501(c)3 non-profit organization that empowers people to help others. Through both on-going volunteering and an annual 3-day Weekend of Action, PROJECT OUR TOWN creates opportunities for volunteers to participate in meaningful, impactful projects that serve the community. The Weekend of Action, November 8-10, 2019, will include more than 1,500 volunteers blanketing the Denver metro area performing over 100 community service projects for over 50 non-profit agencies. The projects are designed to provide for those in need, support meaningful causes, and create positive volunteer experiences.

With the support of RefiJet’s sponsorship, volunteers will make and serve meals at residential treatment facilities and homeless shelters, perform a variety of service projects for veterans groups, food banks, and provide support for organizations addressing causes such as homebound seniors, at-risk children, veterans, active military, seniors, animals, poverty, mental illness, abuse, physical and intellectual disabilities, illness, and more.
