Comagine Health participates in study to reduce the risk of post-incarceration opioid overdose in women


“We know from experiences in other states, that women are at a significantly higher risk of opioid overdose compared to their male counterparts,” says Elizabeth Needham Waddell, Ph.D., an assistant professor in the OHSU-PSU School of Public Health.

More than 75 percent of incarcerated women in Oregon’s state prison system require treatment for a substance use disorder. Further, according to Elizabeth Needham Waddell, Ph.D., an assistant professor in the OHSU-PSU School of Public Health, opioid overdose is a leading cause of death following prison release.

“We know from experiences in other states, that women are at a significantly higher risk of opioid overdose compared to their male counterparts,” she says. “Especially in the first few weeks following their release.”

While sex-related differences specific to overdose risk in incarcerated adults are not yet fully understood, there are multiple individual and social factors to consider, Waddell explains. For example, compared with incarcerated men, women have higher rates of substance use disorders and co-occurring medical and mental health conditions. Limited access to treatment, supportive social networks and affordable housing also are related to overdose risk.

“When these factors aren’t considered, and effective links to treatment and recovery support services are not provided prior to release from prison, women are not set up for success in the community and are more likely to return to substance use,” Waddell says. “We need to offer an appropriate continuum of treatment and recovery support that starts before their release.”

Introducing ROAR

Waddell is collaborating with researchers in the OHSU-PSU School of Public Health, OHSU School of Medicine, the Oregon State University/OHSU College of Pharmacy and Comagine Health in conducting a three-year pilot study to evaluate a new opioid overdose prevention program called Reducing Overdose After Release from Incarceration, or ROAR.

Comagine Health is providing support to certified recovery mentors from CODA and Bridgeway Recovery Services and leading data collection efforts, which include baseline surveys collected prior to release from Coffee Creek Correctional Facility, follow-up surveys collected three and six months after release, and qualitative interviews collected three months after release.

Launched in June 2019 in collaboration with the Oregon Department of Corrections, ROAR is specifically designed to meet the unique needs of incarcerated women diagnosed with moderate to severe opioid use disorder. The program, funded by the Centers for Disease Control and Prevention, has already recruited more than 20 participants.

Certified recovery mentor Morgan Nelson works with a number of them. Nelson, formerly incarcerated herself, works for ROAR program partner Bridgeway Recovery Services. She credits a five-year stint in the state prison system with saving her life.

“Many incarcerated women have the odds stacked up against them from the beginning of their time in prison: mental health, addiction and minimal support,” she says. “The experience of being released from prison can be incredibly overwhelming and emotional. It is a realization that the world continued on without you. That children have grown older, that simple things like banking or shopping can be hard in so many ways.”

Nelson believes ROAR can set women up for success and help ease the stressful transition back to their community.

“Now, as a mentor, I get to build relationships with women with similar experiences to mine,” she says. “The combination of substance use disorder treatment and a mentor’s ability to relate to the challenges of incarceration create a support system that can help to normalize the emotions they are experiencing around release.”

The ROAR experience

A cross-disciplinary approach, ROAR combines initiation of medication and peer support with an ongoing link to community substance use disorder treatment, prior to release.

In the month before leaving Coffee Creek Correctional Facility, Oregon’s only women’s prison, program participants connect with certified recovery mentors via “reach-in” visits, which help to prepare participants to enter community-based treatment. The visits also help to educate adults in custody about the importance of carrying a nasal naloxone overdose rescue kit, commonly known as Narcan, upon release. All participants are released with Narcan rescue kits provided by the project.

Women who elect to participate in the ROAR pilot study receive an injection of extended release naltrexone from a correctional health services provider in the week prior to release. Extended-release naltrexone is an opioid antagonist drug that can block the effects of opioids for up to four weeks, as well as provide overdose protection.

Following incarceration, a certified recovery mentor will help the study participant connect to substance use disorder treatment and provide support for treatment retention and ongoing recovery efforts. Participants can work with their health care providers at partnering treatment recovery facilities, CODA and Bridgeway Recovery Services, to continue extended-release naltrexone or explore alternative treatments.

Tracking ROAR impacts

Working with Oregon’s Department of Corrections, Department of Human Services and the Oregon Health Authority, Waddell and colleagues will use vital statistics, hospital and Medicaid data interviews to assess the pilot project’s impact on overdose rates among participants in four Oregon counties — Clackamas, Marion, Multnomah and Washington — compared with women released in the rest of the state. Insights obtained from the participants will help the research team assess feasibility and acceptability of scaling up the project in future years.

“We’ll follow these women to learn about their experiences in the program, conducting interviews with them as well as their mentors and clinicians, for six months after their release,” says Waddell, who hopes to involve 100 participants in the program over the next 18 months.

She and the research team hope to see a reduction in overdose rates, and, together with the Department of Corrections, identify concrete, feasible strategies for scaling up the program to reach both men and women incarcerated in Oregon and other states. Results are anticipated 2022.

This work is funded by a $2 million grant from the Centers for Disease Control and Prevention (grant: 1 R01CE003008-01-00). Study partners include OHSU, PSU, Oregon State University, Oregon Department of Corrections, Comagine Health, CODA, Inc. and Bridgeway Recovery Services.

Netflix Reality Series Highlights the Benefits of a More Personalized Approach to Medicine, says Dr. Michael Farzam of House Call Doctor Los Angeles

Modern mobile medicine offers the most commonly performed medical tests.

Medical quandaries can usually be avoided when a doctor is allowed to take a little extra time and is able to see patients in their regular environment.

An August 20 article on USA Today reports on Netflix’s new series “Diagnosis,” which follows seven real-life medical cases in which patients were misdiagnosed and better diagnoses are being sought. Dr. Farzam notes that a house call from a 24-hour doctor offers personalized and intimate care that most busy medical clinics simply do not have the time or resources to provide. He adds that, when doctors take the time to get to know their patients’ overall situation, they and their patients are much more likely to find a successful solution together. He adds that meeting patients at home can often provide additional clues about a patients’ lifestyle that can lead to a more comprehensive and accurate diagnosis.

With today’s advances in medical technology, many of the capabilities that have made modern medicine so successful are now portable, Dr. Farzam adds. Patients can expect nearly all of the most commonly used tools, such as blood and urine tests, X-rays, and ultrasounds. He also notes that in most cases, a house call doctor can arrive within an hour of being called—comparable, if not much faster than a typical emergency room visit and, depending on the situation, very possibly at a lower cost to the patient.

Patients who receive a house call doctor can do so from the comfort of their home, office, or virtually anywhere that is conducive to a medical examination, says Dr. Farzam. He notes that this also reduces the risk of being exposed to illnesses in a crowded waiting room and presents an option for those who may be facing multiple health issues that make getting to and from doctors’ offices difficult.

The board-certified internist adds that a house call doctor is capable of treating virtually all of the ailments that patients may suffer that are not immediately life-threatening. This can include conditions ranging from simple upper respiratory infections and flu to chronic issues, minor injuries, various infections, allergies, and more. In short, the doctor adds, most patients can expect the same level of treatment as a fully equipped medical office but with more personal attention and without the hassle or stress of staying in a crowded waiting room.

New BullGuard Commissioned Study Reveals Substantial Gaps Between Privacy Concerns And Actual Behaviors Of American, British and German Consumers

Amid ongoing identity theft and privacy-related breaches, new research published today by cybersecurity company, BullGuard, revealed substantial gaps between consumers’ privacy concerns and their actual behaviors. The study shows 53% of Americans, 48% of Brits and 62% of Germans are concerned about privacy, but 74% of Americans, 80% of Brits, and 86% of Germans don’t use a VPN (Virtual Private Network) when accessing public Wi-Fi, with convenience trumping privacy and security risks in their rush to get online. A VPN empowers consumers with a highly effective, easy way to safeguard themselves when using public Wi-Fi from their smartphone, tablet or laptop, and adds an extra layer of security and privacy when used on the home network.

Unbeknownst to most consumers, hackers can easily spoof and set up malicious open Wi-Fi networks that appear to be legitimate (e.g. ‘Free Airport Wi-Fi’), but in fact intercept and record all network traffic, including people’s intimate personal data, and allow fraudsters to steal usernames, passwords, credit card details, bank account information and more. Alarmingly, approximately one in five Americans (19%) used a credit card while connected to public Wi-Fi, 17% of Brits performed online banking, and 14% of Germans accessed an insecure website — even though 53% of Americans, 48% of Brits and 62% of Germans believe their data is not secure while on a public Wi-Fi network and two-thirds of all survey respondents stated they are most worried about their banking information being hacked and stolen.

“Consumers are playing Russian Roulette with their personal data and privacy, opting for convenience over safety when using public Wi-Fi,” said Paul Lipman, CEO of BullGuard. “The research findings clearly indicate that Americans, Brits, and Germans do not feel safe online, yet they are ignoring their fears and risking financial theft, identity theft, account fraud and more in their quest to use public Wi-Fi. Accessing public Wi-Fi without the use of a VPN is akin to driving your car without insurance – sooner or later you’re going to end up paying the price for being negligent.”

The research also revealed 48% of Americans, 62% of Brits and 69% of Germans are only using antivirus (AV) software to protect their online information on their home Wi-Fi. AV software prevents, detects and removes malware and helps protect users from other threats such as ransomware, malicious URLs, phishing scams, botnet DDoS attacks and more, but it does not hide a consumer’s origin IP address or prevent others – including ISPs (Internet Service Providers) and government organizations – from monitoring a consumer’s online browsing activity, including what websites they visit, what they download or what services and applications they use.

“A disconnect currently exists in the minds of consumers between the benefits and differences of AV software and VPN. The research findings indicate a need to educate consumers that VPN is not simply a useful tool for streaming geo-locked content, but an integral component to safeguard their overall online security and privacy,” stated Lipman. “While antivirus software is essential for detecting and removing malware from your PC, smartphone and tablet devices, it offers no protection from having your personal data intercepted by a malicious hotspot or blocking your ISP from monitoring your online browsing activity.”

Additionally, a significant number of individuals (19% of Americans, 12% of Brits and 7% of Germans, respectively) use no additional methods and rely solely on their ISP and browser to keep their online information private and secure. This despite results that show consumers believe the following entities are tracking their online activities:

2.    Internet Browser (51% Americans, 57% Brits and 57% Germans)

3.    Internet Service Provider (53% Americans, 58% Brits and 44% Germans)

Chairman of San Diego’s Largest Non-Profit Steps Down After 39 Years

TERI Inc., San Diego’s largest nonprofit serving adults and children with intellectual and developmental disabilities and their families announced today that 39-year Chairman of the Board, Cheryl Kilmer has stepped down. Melinda Newsome will assume chair responsibilities immediately.

New leadership is a ‘natural evolution,’ says founder and CEO.

TERI Inc., San Diego’s largest nonprofit serving adults and children with intellectual and developmental disabilities and their families announced today that 39-year Chairman of the Board, Cheryl Kilmer has stepped down. Kilmer founded TERI 39 years ago and has served as CEO and Chairman of the Board since that time.

“It’s really the natural evolution of any organization,” explains Kilmer, who will stay on as CEO. “And we’re all extremely excited about our new chair.”

Melinda Newsome will assume chair responsibilities immediately. She comes to TERI with an entire career of working with non-profit organizations, specializing in fundraising, grant writing and sustainability. Plus, she’s passionate about TERI and its mission.

“You know, I never expected to be working with TERI,” Newsome said. “But when I was called back in 2014 to help them with some development issues, I fell in love with them – their clients and their organization. I’m honored to use my skills to help ensure that TERI has a dynamic and sustainable future.”

Kilmer couldn’t be more pleased with her successor. “Melinda understands our culture, our love for our clients and our commitment to quality programs,” Kilmer said. “Plus her background is excellent. We are extremely grateful to have her.”

Laura White, who spent years as TERI’s development director, and who now sits on the board, is optimistic about the extra time the new leadership will afford Kilmer to work on completing TERI’s San Marcos-based Campus of Life. “Cheryl is really a unique woman,” White said. “She’s dedicated her entire life to serving the special needs community. I’m glad she will have more time to really focus in on completing the Campus with a new chairman in place.”

Work is currently under way for phase II of the campus which will include an 8,460-square-foot vocational center that will offer TERI’s special needs clients career training in floral arts, woodworking, food processing, culinary arts and sales. A 6-acre organic farm will offer client training in growing, harvesting, packaging, selling and delivering organic produce to local restaurants and farmers markets. The second phase will also provide 1,750 additional training and volunteer opportunities for TERI clients and the community.

Completing the campus, designed to serve as an international model, while maintaining TERI’s high standard of quality for it’s over 850 clients and their families is at the top of Kilmer’s list. “There’s never a day that goes by that I’m not extremely grateful for the opportunity I have to work with these special people,” said Kilmer, who was originally inspired to found TERI by a young girl she worked with named Yvette, who was living in deplorable conditions in an institution. “We owe our clients this new campus,” Kilmer added. “We have so many people depending on it. Recent studies show that 1 in five people now have a disability. We really do owe them this.”
