Hand and Upper Limb Orthopedic Surgeon Dr. Alejandro Badia Discusses What It Means To ‘Jam’ a Finger; Offers Tips for Finger Injuries in General

Should a finger get jammed, “do not pull on it, thinking it is just dislocated. You might only make the injury worse.”

When it comes to ball control in sports, avoid giving it the fingers. That’s because a hard-thrown or hit ball in sports can “jam” a finger, causing a tendon injury known as mallet finger – sometimes called baseball or hammer finger. It occurs when an extended fingertip receives a sudden blow or is forcibly flexed, says noted hand and upper limb orthopedic surgeon Dr. Alejandro Badia.

The trauma can rupture the soft-tissue extensor tendon, which runs along the backside of the finger (opposite the palm), or literally pull the tendon off the distal phalanx, the finger’s farthest – and smallest – bone, leaving the fingertip in a noticeably drooped position, unable to be extended without help, says Dr. Badia, an expert in musculoskeletal disorders, especially of the hand and upper limbs. Tenderness and swelling are also likely to develop in and around the fingertip joint.

A mallet injury in sports frequently affects the ring or small finger on the dominant hand. But Dr. Badia emphasizes mallet finger is not confined to athletics.

“The injury may occur if a finger is accidentally crushed in a closed drawer or door, for example, or the back of the fingertip is deeply cut while using tools or cooking knives,” he states. “Older individuals, especially women, can incur mallet finger when performing common household tasks like tucking a bedsheet under a mattress.”

More worrisome than the finger injury itself is the tendency of patients to dismiss the problem – and the pain – as simply a minor finger sprain and delay or not even seek medical treatment. “The problem is particularly prevalent in sports when players want to remain active and play through the pain,” says Dr. Badia, who is founder and chief medical officer of the Florida-based Badia Hand to Shoulder Center and OrthoNOW®.

“The term ‘jammed finger’ may not sound serious, but the injury can actually lead to long-term problems,” Dr. Badia says. “General urgent-care centers oftentimes use this term without distinguishing between occult fractures, ligament injuries, or tendon injuries, including mallet finger and boutonniere injuries.” Boutonniere refers to a deformity caused by a tendon injury to the middle finger joint.

“A significant digit injury should be seen by an orthopedist, preferably a hand specialist, within days,” Dr. Badia urges. An appointment is usually not required if a neighborhood orthopedic urgent care center, like OrthoNOW®, is available.

“Most importantly, avoid a hospital emergency department to treat hand or finger injuries. Besides being expensive and time-consuming, emergency departments may miss the problem or recommend incorrect treatment,” he advises.

Fingers consist of relatively complex systems, involving an interplay of bones; bone joints that act as hinges; ligaments – strong bands of tissue connecting the bones; and tendons, which are cords of collagen anchoring bones to muscle. The anatomy ensures motion, stability and hand strength, allowing the flexing and extension of fingers for maximum gripping and grasping.

“Untreated damage to any of these elements can lead to stiffness and permanent deformity of the fingertip, including development of a swan neck deformity, which causes the finger to bend in unusual ways,” Dr. Badia notes.

Mallet finger is most concerning in children and adolescents whose fingers are still in the process of growth and development. Injuries to their fingers can result in deformities and loss of function, Dr. Badia warns.

Most cases of mallet finger – those involving only soft-tissue injuries or small bone fractures – are treated conservatively by splinting the injured finger. Splints normally must be worn for approximately eight weeks, followed by night-time-only splinting for another four-to-six weeks. Patients engaged in sports or high-risk work activities may have to wear splints beyond the initial healing period, Dr. Badia says.

In limited instances, surgery is needed to repair a tendon and stabilize the fingertip’s bone structure and joint, especially if the mallet injury caused the tendon to pull away a large fragment (more than 30 percent) of bone when it was avulsed from the distal phalanx or if the injury has become chronic because it went untreated for too long.

“For large fractures, surgical approaches include stabilization of the extensor tendon and wire fixation of the distal interphalangeal joint (fingertip joint),” explains Dr. Badia. “In cases of chronic mallet finger, the surgeon may initially try splinting; otherwise, surgical repair or reconstruction of the extensor tendon is performed to improve joint extension and correct flexion deformity of the fingertip.”

Should mallet symptoms persist, the surgeon many employ a salvage procedure that involves use of wire, screws or pins to fix the interphalangeal joint in a slight flexion to enhance finger function and alleviate pain, Dr. Badia adds.

Meanwhile, Dr. Badia offers this advice for mallet finger and finger injuries in general:

Immediately apply ice to the injured, raise the hand so that fingers are above the heart and take over-the-counter non-steroidal anti-inflammatory medications for control of pain and swelling.

If finger pain persists more than a few days following injury, contact an orthopedic specialist for evaluation. Don’t delay.

If splinting of the finger is recommended, be compliant and wear the splint. When removing the splint to wash or dry it, keep the injured finger on a flat surface to prevent bending the finger and re-injuring the tendon.

Perform suggested exercises to prevent the splinted finger’s middle joint from stiffening.

Coming off a very successful year, Nike Tennis Camps looks to continue the momentum of 2019 by providing quality, fun tennis camps for campers to return to year after year and provide new locations for junior and adult tennis players to improve their game.

As a staple division of the company, Nike Tennis Camps looked to build on its long-standing overnight tennis camps that have been running for more than 10 years including: Amherst College, Lawrenceville School, USD, Duke, Stanford and Lake Tahoe at Granlibakken Resort. Other strong locations such as MIT, Minnesota and Michigan State all saw significant spikes in camper popularity.

While the growth of top tier, long standing locations was a large focus, Nike Tennis Camps also looked to grow their day tennis camp market. The additions of Fayetteville High School, Middlesex School, Quail Ridge Country Club, Jack Kramer Club and Agawam Hunt Club were solid additions that look to have strong growth in 2020. Nike Tennis Camps also introduced three new programs to their summer junior tennis programs this year: U10 (10 and Under Tennis), Universal Tennis Rating (UTR) and USTA Tournament Challenge (offered at Amherst College).

U10 locations were able to create groupings for under aged campers to use lower compression balls (red, orange and green dot) so they are easier to hit, allowing kids to get to the ball and develop optimal swing patterns. Nike Tennis Camps and UTR have developed a special partnership where campers that are enrolled for camps at a specific camp location get to create or develop their UTR score. UTR has become increasingly popular for tennis players to improve their skills and evaluate their matches.

The USTA Tournament Challenge at Amherst College was a one-of-a-kind program where campers were able to enroll for a 2-week camp. The first week was dedicated to mental and skill development to prepare for the tournament weekend. Players then got to experience competing in an official USTA tournament over the weekend. The second week is devoted to further fine-tuning skills based on what was charted during the USTA matches. When asked about her daughter’s experience, a parent nothing but positive comments, “My daughter had another wonderful week at camp. She loves the on-court time with coaches who care about her and want to help her improve.”

Coming off a very successful year, Nike Tennis Camps looks to continue the momentum of 2019 by providing quality, fun tennis camps for campers to return to year after year and provide new locations for junior and adult tennis players to improve their game. Liberty IT Solutions Awarded $434M Health Integration and Modernization (HI&M) Task Order


Liberty IT Solutions LLC, which specializes in Health Information Technology has been awarded their largest award to date with the 60-month (1-year base period, plus 4 option years) Health Integration and Modernization (HI&M) Task Order from the Department of Veterans Affairs (VA). This $434M Task Order will consolidate the Enterprise Portfolio Management Division (EPMD) Health Portfolio’s application under one modernization effort. This will allow VA to institute leading development practices to benefit all applications. Liberty will partner with Geek Sources Inc. and Ad Hoc LLC on the HI&M Task Order, both bringing unique experience and expertise to the program.

Through the modernization effort, Liberty will consolidate Office of Information and Technology (OIT) EPMD Health Portfolio projects and products into the Cerner Electronic Health Record (EHR) system. This will require Liberty to create a System of Systems (SoS) that supports Cerner and the Health Portfolio.

PMB Positively Impacts Healthcare in the Underserved Heart of California’s Imperial Valley

PMB, one of the nation’s leading real estate developers and owners across the continuum of care, is pleased to announce the completion of their newest project, Clinicas de Salud del Pueblo’s El Centro Health Center (ECHC), in the heart of California’s Imperial Valley.

Clinicas de Salud del Pueblo, a Federally Qualified Health Center (FQHC), needed more than a typical medical office building – it has a special purpose and story for how it came to be. It’s important to understand the dynamics of the environment surrounding this new healthcare facility located in El Centro, Calif., halfway between San Diego and Arizona, just 12 miles north of the Mexico border. The fabric of the community is based on the long-standing agricultural landscape throughout the area and the workers that support it, half of the community to be exact. El Centro is situated within Imperial County, a geographically isolated region in California’s vast southeastern desert. It has a constant flow of bi-national agricultural workers, tourists and residents crossing the nearby U.S./Mexico border daily. Imperial County ranks 57th out of 57 California counties for health factors overall, with a severe shortage of medical, dental, and mental health providers. Its social and economic demographics contribute to many barriers when trying to access health care services. Given its population of migrant and seasonal labor, Imperial County continually has the highest unemployment rate in California. As of June 2019, Imperial County’s unemployment rate was 18.6 percent vs. 3.7 percent in the United States overall. (Source: U.S. Bureau of Labor Statistics Labor Force http://www.bls.gov/lau/data.htm accessed 8/2019) There is also a deficit in facilities for local students to complete their medical training and internship programs, further adding to the medical provider shortage.

Through the partnership with Clinicas, PMB secured an off-market site located within a larger retail setting with tremendous access and visibility. PMB led the development of the new two-story, 45,000 square foot outpatient care center which is the new face of health and wellness in the Imperial Valley. The Project advanced Clinicas’ mission to expand healthcare access, enhance preventative and clinical services and bolster training and education to the population of the Imperial Valley.

Yvonne Bell, CEO of Clinicas de Salud de Pueblo noted that, “Clinicas partnered with PMB to expand services, improve access and redefine the patient experience in the Imperial Valley. PMB’s ambulatory care strategy expertise, partnership culture and agility in navigating through a highly-complex New Markets Tax Credit financing structure allows Clinicas to continue our mission to improve the health and well-being of the communities we serve through providing access to excellent care, available to all.”

The ultimate financing structure was as unique as the project itself, as traditional equity and debt financing was limited due to the non-traditional market, demographics and Clinicas unique FQHC reimbursement structure and financial profile. As a result, PMB analyzed many different financing structures and identified the Federal New Markets Tax Credit (NMTC) program as the most beneficial financing construct for Clinicas. PMB received a $24,500,000 Qualified Equity Investment allocation from three Community Development Entities through the Federal New Markets Tax Credit program. One hundred percent of the New Markets Tax Credit benefit, which netted $4.3 million, was allocated to Clinicas which was used to fund fixtures, furniture and equipment in the facility.

“The larger, new healthcare center has enabled Clinicas to increase patient capacity by 85 percent, create 152 new permanent jobs, and expand its healthcare services and specialties,” says Ben Ryan, PMB Partner and CFO. “Most importantly, the new facility is helping Clinicas deliver on their mission and improving local residents’ access to the highest quality healthcare.”

With the increased capacity the new clinic is accepting patients for adult medicine, behavioral health, dentistry, pediatrics, podiatry, neurology, radiology, women’s health, 3D-4D OBGYN ultrasounds, and has expanded the medical department as well as adding a new pharmacy, and optometry services. These new services have increased patient access to specialty services and enhances the integrated care system that addresses multiple, diverse needs at a single facility and will lead to a much more efficient, effective, comprehensive, patient‐centered model of care. The new facility will also house the El Centro Clinic in addition to Clinicas’ administrative staff.

The new clinic is designed to complement and embrace the local geography and environment. The concrete, tilt-wall building reacts to the natural geography and climate of the area and includes specially designed solar shading at the east, west and southern exposures. The site also utilizes a local, drought tolerant plant palette that is harmonious with the contemporary building design. Nadel Architects, ranked amongst the top architectural firms in the United States, provided building architectural services. Javier Diaz Architects, who provides architectural services to the Imperial Valley and San Diego County, provided tenant architectural services and San Diego-based Pacific Building Group, one of the region’s leading general contracting firms, served as the General Contractor.
