Patients should not administer more than one dose within 24 hours or 8 doses per month.

The FDA approval of this medication was based on two identical pivotal trials of approximately 1,200 women demonstrating that Vyleesi improved desire and reduced distress related to HSDD as measured by validated patient-reported outcome instruments. At the end of the core six-month studies, women were given the option to continue in an open-label extension study for an additional year. Approximately 80% of the women who completed the six-month studies chose to volunteer for the open label extension in which all the women received Vyleesi. In the pivotal trials, the most common adverse events were nausea, flushing, injection site reactions, headache, and vomiting. The majority of these events were reported to be transient and mild-to-moderate in intensity. Vyleesi also caused small, transient increases in blood pressure and decreases in heart rate, as well as darkening of the skin in some women. Women who are pregnant, or have uncontrolled hypertension, known heart disease or are taking oral naltrexone-containing products intended to treat opioid or alcohol addiction should not use this medication. Women who can become pregnant should use effective birth control during treatment with Vyleesi.

A September 2 article on CBS News details a new study finding that patients who had undergone weight loss surgery were significantly less likely to suffer heart attacks, strokes, kidney failure, and heart disease versus a control group of obese patients who did not undergo bariatric surgery. Los Angeles-based weight loss surgery specialists Dr. Feiz and Associates says the study adds further evidence to an overwhelming consensus in the medical community that bariatric surgery offers all of the many health benefits associated with defeating obesity simply by making weight loss more achievable.

Dr. Feiz and Associates says that severely obese individuals who attempt serious weight loss regimes typically fail over time, in most cases through no real fault of their own. Instead, the clinic notes that when patients attempt to lose weight they may find that initial success – even dramatic success – is often followed by a regaining of all of the weight that was lost, sometimes with a small bonus. The catalyst of what is typically referred to as “yo-yo dieting,” the clinic says, is a hormone known as ghrelin, often thought of as the most active hormone when it comes to hungry feelings. Obese individuals tend to produce more of it compared to thinner people. Worse, when a dieter starts to lose weight, production of the hormone increases and the body essentially does everything it can to encourage the individual to eat more. It’s a mechanism humanity has developed to promote survival when starvation is a risk which, ironically, backfires when high-calorie foods are all too easily obtainable.

The clinic says that shrinking the capacity of the stomach, a procedure such as a sleeve gastrectomy also appears to have a major impact on hormone production. By removing roughly 75 to 85 percent of the stomach, the centers for the production for ghrelin may also be removed, doctors theorize. In removing these production centers, a patient’s body may be less able to sound the hunger alarm, making it significantly easier for patients to change their relationship with food. Without nagging pangs of hunger, the clinic continues, bariatric patients are able to achieve significant and permanent weight losses, reversing the usually dismal statistics on weight loss for obese patients.

The clinic notes that it is common for obesity-related ailments such as type 2 diabetes to dramatically improve or even go into remission after weight loss surgery. Obesity has a long list of negative conditions associated with it, the clinic explains, and by finally losing significant amounts of weight, threats to the length and quality of a patient’s life dramatically decline.

Trucking Industry Battle: Proposed Drive-Time Regulations

“Fatigue is a real factor in driving, and while there is no magic way to eliminate it, it is something that simulation training can help drivers recognize and deal with.”

In mid-August, the Transportation Department’s Federal Motor Carrier Safety Administration (FMCSA) proposed a set of changes to the hours-of-service rules that govern the hours truckers can spend behind the wheel and the total number of breaks they are required to take[1]. “This proposal addresses some real issues and frustrations confronting truckers and the industry,” says John Kearney, CEO of Advanced Training Systems. Kearney, whose company is a leading designer and manufacturer of virtual simulators for driver training. “However, we must always make safety, both for drivers and other motorists, our number-one concern.”

Current hours-of-service rules include the following provisions:

1.    Truckers can drive for a cumulative daily maximum of 11 hours following 10 consecutive hours off duty.

2.    Drivers are required to take an off-duty period of at least 30 minutes after a maximum of eight consecutive hours of driving.

3.    They cannot drive beyond the 14th consecutive hour after coming on duty[2].

The proposed new rules include these suggested revisions:

1.    Drivers would be able to take a break while on duty but not driving–i.e. while the truck is stationary and waiting to be loaded or unloaded.

a.    Under current rules, stationary on-duty hours count against the driver’s allowable daily driving time.

2.    Drivers would be permitted to “pause” the 14-hour driving window for an off-duty break of up to three hours.

a.    This would create a possible 17-hour workday, with the provision that the driver still takes 10 consecutive hours off duty at the end of the shift[3].

Many drivers are paid by the mile, which means that excessive wait time at loading docks—known in the trade as “detention time”—costs drivers money. According to a recent FMCSA report, detention time costs the average driver $1,281 to $1,534 per year, or 3-3.6 percent of the driver’s total annual income. The report also estimates that excess detention time raises the likelihood a driver will be involved in a crash by 6.2 percent, due to a consequent urge to make up for lost time[4].

“We can argue about what specific hours-of-service limits should be,” Kearney said. “No one questions that reasonable limits should exist and should be enforced. Fatigue is a real factor in driving, and while there is no magic way to eliminate it, it is something that simulation training can help drivers recognize and deal with. Given that, and given the seriousness of this whole issue, I would suggest that one urgently needed change in FMCSA regulations is to make simulator experience a mandatory part of commercial driver training.”

Whether or not the proposal goes through, simulator training can help prepare drivers for long hours on the road–before they even hit the road.
