5 Questions for Your Doctor

Artificial disc replacement doesn’t stop patients from bending or flexing normally, making it an increasingly popular option for chronic back or neck pain

Nearly all of us know someone who had surgery to replace their bum hip or knee with an artificial joint. But artificial discs? Fewer have heard of this increasingly prevalent orthopedic surgery, according to Praveen Kadimcherla, MD, of Atlantic Spine Center.

Approved by the U.S. Food and Drug Administration in 2004, artificial disc replacement implants a synthetic disc into the spine to replace a worn or damaged disc. The procedure’s goal, Dr. Kadimcherla says, is to relieve chronic back pain while enabling more normal range of motion that might be possible with other back procedures, such as spinal fusion.

While an estimated 8 in 10 American adults will experience back pain at some point, according to the National Institutes of Health, surgery is rarely necessary and only used when pain doesn’t improve with conservative treatments such as exercise, physical therapy or pain relievers. When spinal fusion surgery is performed to fuse painful vertebrae into a solid bone, fusion can prevent normal spine motion.

“But artificial disc replacement doesn’t stop patients from bending or flexing normally, making it an increasingly popular option for chronic back or neck pain,” Dr. Kadimcherla explains.

Best questions to ask in advance

Just because an innovative surgical option for back pain has emerged doesn’t make it right for everyone with back or neck pain, Dr. Kadimcherla notes. He suggests being ready to talk with your doctor about the possibility of artificial disc replacement with these 5 questions:

1.    Am I a good candidate for artificial disc replacement?

Disc replacement is a better option for certain patients, including those whose back pain originates in only 1 or 2 discs in the spine. People with slipped or herniated discs may be good candidates for the procedure, but not those who suffer from degenerative disc disease, which can affect the entire spinal column. It’s also important not to have significant joint disease or spinal nerve compression, Dr. Kadimcherla says. “Good candidates for this surgery aren’t excessively overweight and they haven’t undergone spinal surgery before,” he adds. “They also can’t have scoliosis or another deformity of the spine.”

2.    What happens during artificial disc replacement surgery?

In a 2- to 3-hour procedure, you’ll receive general anesthesia while lying on your back. While you’re asleep, surgeons will make an incision in your abdomen (if your disc replacement is occurring in the back), moving aside organs and blood vessels to allow access to your spine. If a disc is being replaced in your neck, the incision is made at the front of the neck. Your damaged spinal disc(s) will be removed and artificial disc(s) will be inserted in their place. After your incision is closed, you’ll go to a recovery room.

3.    Are there any risks to artificial disc replacement surgery?

Like any surgical procedure, artificial disc replacement poses certain risks, however minor. The operation requires greater access to the spine than other spinal surgeries, Dr. Kadimcherla notes. Potential surgical risks include infection; dislocation of the new disc; implant failure or breakage; implant loosening or wear; stiffness of the spine; narrowing the spine due to a breakdown in spinal bones; and blood clots in the legs.

4.    How can I prepare for artificial disc replacement?

Your doctor will take a detailed medical history and perform a thorough physical exam. You may also need imaging tests such as X-rays or an MRI or CT scan before surgery, along with blood tests. “Make sure to inform your doctor of all prescription and over-the-counter drugs you take as well as vitamins, herbs or other supplements, since medications and supplements can affect blood clotting and other aspects of healing,” Dr. Kadimcherla says. Just before surgery, you may be told not to eat or drink anything for several hours.

5.    What is recovery like from artificial disc replacement?

The days just after surgery should allow rest and healing, Dr. Kadimcherla says. You may stay in the hospital for a few days, and you won’t be allowed to drive right away when you get home. You may also need help at home with activities such as dressing, cleaning and shopping. Pain will be controlled with appropriate medications and you’ll be encouraged to walk about within the first day, he says. “While you’ll need to avoid any jarring movements for awhile, recovery should proceed steadily,” Dr. Kadimcherla explains. “Walking, stretching, and doing gentle exercises will all promote your return to normal life.”

Since the development of the NRMP in 1952, the recruitment and match process has remained largely static. A recent study of the residency application process published in The Journal of Graduate Medical Education noted that scheduling interviews and engaging with potential matches in a time-sensitive manner creates “a cumbersome and labor-intensive step for the residency staff, which is associated with long delays in interview date confirmation after preferences are sent by applicants.”

Highmark, Boehringer Ingelheim Announce Value-Based Contract for COPD Medication Stiolto® Respimat®


Highmark and Boehringer Ingelheim today announced a new value-based contract for Boehringer Ingelheim’s Stiolto® Respimat® (tiotropium bromide & olodaterol) Inhalation Spray for the treatment of chronic obstructive pulmonary disease (COPD). This marks the second value-based contract between the two companies. It is also the first contract for Highmark to include Medicare plans in addition to commercial health plans.

This contract confirms the commitment of both companies to improving patient care and lowering overall health care costs by linking reimbursement to patient outcomes. Using the Global Initiative for Chronic Obstructive Lung Disease (GOLD) guideline-based care as a benchmark, Highmark will evaluate whether Stiolto Respimat reduces the total cost for both in-patient and out-patient COPD care for members compared to the costs for members who use other drugs to treat the health condition.

COPD is a progressive respiratory disease that affects more than one in 20 adults in the U.S. The condition is the fourth-leading cause of death in the U.S., and it is projected to cost the U.S. health care system about $49 billion in 2020, according to the Centers for Disease Control and Prevention (CDC). The CDC estimates that more than 16.4 million work days are lost each year due to the complications of COPD.

The new agreement, which expands Highmark’s efforts to address high prescription drug costs and enhance patient care, is the second agreement between Highmark and Boehringer Ingelheim. The first agreement focused on a drug from Boehringer Ingelheim that treats adults with type 2 diabetes who have cardiovascular disease.

IMPORTANT SAFETY INFORMATION AND INDICATION for STIOLTO RESPIMAT

Do not use STIOLTO if you have asthma. People with asthma who take long-acting beta2-agonist (LABA) medicines, such as olodaterol, (one of the medicines in STIOLTO), without also using a medicine called an inhaled corticosteroid, have an increased risk of serious problems from asthma, including being hospitalized, needing a tube placed in their airway to help them breathe, or death.

Do not use STIOLTO if you are allergic to tiotropium, ipratropium, atropine or similar drugs, olodaterol, or any ingredient in STIOLTO.

Call your healthcare provider or get emergency medical care if you experience symptoms of a serious allergic reaction including: rash, hives, itching, swelling of the face, lips, tongue, throat, and difficulties in breathing or swallowing.

Get emergency medical care if your breathing problems worsen quickly or if you use your rescue inhaler but it does not relieve your breathing problems. Call your healthcare provider if breathing problems worsen over time while using STIOLTO.

Do not use STIOLTO more often than prescribed by your doctor. Do not use STIOLTO with other LABAs or anticholinergics.

Do not use STIOLTO for treating sudden breathing problems. Always have a rescue inhaler with you to treat sudden symptoms.

Tell your doctor about all your medical conditions including heart problems, high blood pressure, seizures, thyroid problems, diabetes, kidney problems, glaucoma, enlarged prostate, and problems passing urine.

STIOLTO can cause serious side effects, including sudden shortness of breath that may be life threatening, fast or irregular heartbeat, increased blood pressure, chest pain, tremor, headache, nervousness, high blood sugar, or low blood potassium that may cause muscle weakness or abnormal heart rhythm. If any of these happens, stop taking STIOLTO and seek immediate medical help.

STIOLTO can cause new or worsening eye problems including narrow-angle glaucoma, and can increase the pressure in your eyes, which can cause the following symptoms: eye pain, blurred vision, seeing halos or colored images along with red eyes. If you have any of these symptoms, stop taking STIOLTO and call your doctor right away.

STIOLTO can cause new or worsened urinary retention. Symptoms of urinary retention may include difficulty passing urine, painful urination, urinating frequently, or urinating in a weak stream or drips. If you have any of these symptoms, stop taking STIOLTO and call your doctor right away.

The most common side effects of STIOLTO are runny nose, cough, and back pain.

Tell your doctor about all the medicines you take, including prescription and over-the-counter medicines, eye drops, vitamins, and herbal supplements. STIOLTO and certain other medicines may affect each other.

STIOLTO is for oral inhalation only.

The STIOLTO cartridge is only intended for use with the STIOLTO RESPIMAT inhaler.

Do not spray STIOLTO into your eyes. Your vision may become blurred and your pupils may become larger (dilated).

STIOLTO® RESPIMAT® (tiotropium bromide and olodaterol) Inhalation Spray is a prescription medicine used long term, 2 puffs 1 time each day, in controlling symptoms in adults with chronic obstructive pulmonary disease (COPD). COPD is a chronic lung disease that includes chronic bronchitis, emphysema, or both.

Annual Eduventures® Transfer Student Research Report Examines Preferences, Motivations, and Barriers to Success for College Students in the Transfer Process

Students often enter a community college or other two-year school with the strong intention of eventually transferring to a four-year school…if these schools sit back and let students find them, they are missing key opportunities to engage them early.

ACT® | NRCCUA®, an educational data science and research organization, announced today the publication of the 2019 Eduventures Transfer Student Research Report. The annual report explores the transfer process from the perspective of students who are currently enrolled at post-secondary institutions and are considering transferring to a different institution. In particular, it explores two distinct segments of the transfer market, those transferring from two-year institutions and those currently enrolled at four-year schools.

Increasing Competition for Transfer Students

Prospective transfer students are a little-understood population that is often treated as a homogeneous group by institutions—many of whom have come to rely on them to make enrollment targets. As community college enrollment has dropped by 20% and the transfer-in enrollment at private schools has risen 23% over the past decade, an increasing number of colleges are faced with more competition amid dwindling numbers of transfer applicants. This report outlines the primary motivations, priorities, and concerns among key transfer segments that could impact how they are best identified and cultivated, with implications for a successful transition to their new institutions and, ultimately, academic success.

Key Research Findings Include:

1. 62% of students at two-year schools solidify their transfer plans before enrolling at their original institution. About half of these students are highly sensitive to cost and are greatly concerned about receiving less financial aid (51%) and having to take on more debt as a result of their transfer (53%). These students also appear to have a greater need for support by others in the transfer process.

2. 74% of students at four-year schools start thinking about potential transfer schools while enrolled at their original enrollment institution, indicating less certainty about their transfer plans. Transfer motivations are driven by dissatisfaction with their current institution. While financial considerations are a top priority in the search process, four-year transfers worry much more about the credit transfer process (55%) and a delayed graduation timeline (35%). These students tend to be more self-sufficient in their search process.

3. Students at four-year institutions approach the transfer school search with a do-it-yourself mindset. 45% start their search on online search engines and 40% start on institutional websites, while those at two-year institutions seek help from trusted advisors (33%) and family and friends (39%).

This is the time to continue fostering relationships that institutions have already started to build when these students were still in high school.”

SureFire CPR, an organization dedicated to educating people in first aid and cardiopulmonary resuscitation in Orange County California, additionally offers a scholarship for students pursing degrees in the medical field. The scholarship is now in its fourth year and has allowed students to subsidize the costs of obtaining degrees in these necessary academic areas. It is a prize of $1,000 and will allow students to pursue their field of choice.

The scholarship will be awarded to a student who is a superior academic, who showcases dynamic leadership qualities, and who is involved closely with their communities through service. Students enrolled in classes at schools offering training in medical fields like nursing, physical therapy or respiratory therapy are eligible to apply and students can be full-time, part-time or incoming. Previous scholarship rewards or monetary prizes have no impact on SureFire CPR’s award.

Applications are currently open. Those applying for the scholarship must provide a copy of an acceptance letter or a transcript verifying enrollment in a medical field.
