Don’t Get It Too Late or Too Early, Study Finds for Silicon Valley Orthopaedics

News Image

We agree with Dr. Pagnano’s comments on this study. While patients make the ultimate decision on whether to proceed with total knee replacement, the surgeon should advise patients on their rehab potential and overall health, says Dr. Gay

In a recent article in USA TODAY, the results of this study, published in The Journal of Bone and Joint Surgery, were detailed. The research suggests that for far too many patients, having a knee replacement procedure is done either too late or too soon. The team at Northwestern University’s Feinberg School of Medicine in Chicago found that 90% of patients who could benefit from the procedure waited too long to have it. Alternatively, about 25% of patients who don’t need the surgery are having it prematurely when the benefit is minimal, and they run the risk of complications. Patients with osteoarthritis in the knee should time the surgery to ensure maximum benefits if they are eligible for knee replacement.

In the article, Dr. Mark Pagnano, who is unaffiliated with the study and a professor and chairman of the Department of Orthopedic Surgery at Mayo Clinic, told USA TODAY that despite advancements in the procedure, some people that should have knee replacement surgery hold on to antiquated notions of its risks and outcomes and put off having it done. However, he also emphasized that there may be serious issues in getting the surgery too early. “There’s a sense among some people that a knee replacement will magically cure any problem that’s related to the knee. Particularly for younger, active patients, they will continue to push for a permanent fix to a problem when perhaps a surgical option is still not in their best interest,” Dr. Pagnan explained.

“We agree with Dr. Pagnano’s comments on this study. While patients make the ultimate decision on whether to proceed with total knee replacement, the surgeon should advise patients on their rehab potential and overall health,” says Dr. Gay.

“At our practice, we spend a great deal of time counseling patients so they can make the most informed decision possible,” explains Dr. Reynolds

At Silicon Valley Orthopaedics, Dr. Nic Gay and Dr. Kerisimasi Reynolds fully understand the implications of having joint replacement surgery and typically only recommend joint replacement when conservative or joint preservation treatments are not viable options for eliminating pain and restoring function. They use indicators such as pain level, joint function, age and x-ray assessment to determine if or when knee replacement is necessary. Dr. Gay and Dr. Reynolds have intimate knowledge of knee anatomy and associated illnesses, providing patients with access to the highest standard of care.

Allegheny Health Network Expands ‘Healthy Food Center’ Program to Allegheny General Hospital

News Image

With the great success of our first Healthy Food Center at West Penn, we look forward to bringing this unique resource to patients at AGH, linking food and medicine so that they can better manage their chronic conditions and lead healthier lives.

At the region’s first Healthy Food Center, which opened in 2018 at West Penn Hospital, nearly 40,000 meals have been provided to patients and their families who lack access to nutritious food items. Starting next week, Allegheny Health Network (AHN) will reach even more patients in need with the opening of a second Healthy Food Center at Allegheny General Hospital.

The Healthy Food Center acts as a “food pharmacy” where patients who lack access to food can receive nutritious food items, education on disease-specific diets and additional services for other social challenges they might face.

Food insecurity refers to the lack of consistent access to nutritious foods for an active, healthy life. According to Feeding America, more than 14.3 million American households last year were food insecure with limited or uncertain access to enough food.

“We empathize with patients who are faced with having to prioritize other necessities over buying nutritious foods, and understand that this is a reality for many families. But, particularly for patients living with chronic health conditions such as heart disease and Type 2 diabetes, we also know that they simply can’t afford not to maintain healthy diets,” said Stuart Fisk, CRNP, Director, AHN Center for Inclusion Health. “With the great success of our first Healthy Food Center at West Penn, we look forward to bringing this unique resource to patients at AGH, linking food and medicine so that they can better manage their chronic conditions and lead healthier lives.”

At the AGH Healthy Food Center, located in the Allegheny Professional Building (Suite 103), patients who receive a referral from a medical provider can visit the center to meet with an onsite dietitian to discuss their dietary needs based on their condition.

Residents in Pittsburgh’s Northside may also receive a referral to the center via community-based initiatives such as ‘Thrive 18,’ in which community health workers canvas Northside neighborhoods to uncover barriers to good health and connect residents with necessary resources.

After shopping at the center for the recommended food items, patients go home with two to three days’ worth of food for all members of their household. Patients may visit the Healthy Food Center once every 30 days for six months as part of their referral. During that time, patients have access to a range of educational resources, nutrition counseling and cooking classes, as well as social support services via the Center for Inclusion Health. After six visits, the patient may be issued a new referral if needed.

More than 800 patients have been referred to the West Penn Healthy Food Center since its opening in April 2018. The program was originally piloted among patients with diabetes who were screened by their doctor as being food insecure.

“Maintaining a well-balanced, healthy diet can have a significant impact on blood glucose and blood pressure control. This has been the result for many of my patients with diabetes who previously faced a barrier in accessing healthy foods and now utilize the Healthy Food Center,” said Patricia Bononi, MD, Medical Director, AHN Center for Diabetes and Endocrine Health.

The Healthy Food Center is supported by its many partners including 412 Food Rescue, Gateway Health, Greater Pittsburgh Community Food Bank, Highmark, The Food Trust and Travelers Aid of Pittsburgh.

SVHI PRP Therapy offers innovative 'platelet rich plasma' therapy focused on hair loss at its Foster City, California

PRP for Hair Loss Bay Area

PRP therapy for hair loss is an innovative option for many Bay Area patients.

SHVI PRP Therapy, the leader in innovative hair loss treatments based on PRP (Platelet Rich Plasma) at https://svhi-prp.com/, is proud to announce a new website. The new website is a “microsite,” focused specifically at the use of PRP to mitigate hair loss. Many Bay Area consumers are open to cutting edge technologies, and PRP is one option in their hair loss journey.

“PRP therapy for hair loss is an innovative option for many Bay Area patients,” explained Miguel Canales, surgeon and founder of SVHI PRP therapy. “Even more important, a patient who seeks out a consultation with me on hair loss will be able to review the entire range of options from PRP therapy to hair loss to hair transplantation. I will work with each patient to create the best treatment program for their hair loss needs.”

Persons who would like to learn more about the company can visit the new website. In particular, they can focus on the page specific to PRP therapy can visit https://svhi-prp.com/prp-therapy/. Another option is to read the FAQ (Frequently Asked Questions) document at https://svhi-prp.com/faq/. That document explains as follows. Hair thinning in women, which affects nearly 40 million American women, is mainly due to heredity and can start any time after puberty. However, we also know that female pattern hair loss can be strongly influenced by age, nutrition, stress, surgery, medications, illness, social/lifestyle factors, hair care and more. As hair follicles weaken and stop producing hair, hair loss occurs.

Remarkably, PRP for hair loss can impact the hair itself, often resulting in hair rejuvenation.

PRP or Platelet Rich Plasma for hair loss is an in-office, non-surgical, treatment derived from the patient’s own blood that can be performed in about an hour. After the area of concern is identified, evaluated and measured, standardized medical photos are taken in the photo suite. Special gentle and sterile techniques and equipment are used to separate and concentrate the platelets and plasma from a small sample of peripheral blood. PRP contains growth factors and cytokines that have been shown to be responsible for stimulating and enhancing hair follicle function. The scalp is prepared with antiseptic solution and anesthetic (a “ring block,” not simply local anesthetic gel) is used to completely numb the scalp. Small injections of the PRP deliver the powerful platelet-derived growth factors into the skin at the level of the weak follicles. Electronic, mechanical microneedling is performed.
