Senior living communities must meet or exceed all state and federal standards in order to accomplish a deficiency-free survey. This accomplishment is particularly difficult to achieve in California, due to its rigorous inspection process. A deficiency-free survey is uncommon to achieve and indicates a high level of service at a senior living community. In 2016-17, there were over 17,000 deficiencies among senior living facilities documented in CDPH’s data report.

In California, senior living communities are inspected periodically to ensure services and systems in place are compliant with federal and state laws and regulations. Over a course of multiple days, inspectors arrive unannounced to assess the facility. Surveyors determine compliance with regulations governing areas such as policies and procedures, resident care, quality of care and quality of life, medication administration, medical records, kitchen sanitation, staff competencies, dietary needs, equipment, safety, and overall wellness of the community. The senior living community is awarded a “deficiency-free” survey when all of these regulations are met.

This report, which surveyed health care benefits brokers across the United States, uncovers current challenges brokers are facing and how the role has evolved due to changing employer needs.

Reported findings show that navigating the rising costs of health care benefits, while considering employee experience and the need for integrated technology, are top challenges for benefits brokers. When asked to rank the importance of certain topics to their position, 68 percent of benefits brokers reported that ways to maximize client cost savings was considered of utmost importance.

Cost wasn’t everything, however. When brokers were later asked the most likely reason they would consider changing their clients’ third-party administrator, “service and support for participants” won out at 56 percent, while “better pricing in the current market” came in far behind at 15 percent.

Technology is also top of mind for brokers, as 44 percent reported that giving clients access to a self-service platform to manage their employees would make it easier to manage and recommend tax-advantaged accounts.

Based on the study results, the report also offers insights for refining the health care broker benefits strategy to better service employer clients—and their employees—in the new decade, including tips for recommending cost-effective technology and ways to remain a valuable advisor in a tumultuous health care benefits landscape.

Dylan Walker, founder of New Existence Recovery Centers, offers tips to those in recovery whose New Year’s resolutions include reconnecting with family

Let’s face it; the holidays were a stressful time, a time when the weaknesses in family dynamics came out strongly. Old family issues, rivalries, and secrets were revisited. It’s easy to understand why many folks prefer to duck out of the holidays altogether. For many of those coping with addiction and their families, the holidays posed additional stress for which there is no “cookie-cutter” solution.

Dylan Walker, founder of New Existence recovery centers in Huntington Beach, California, and now four years sober, knows this very well, “I spent three years of Christmas holidays in recovery centers, missing family gatherings, leaving my parents the terrible task of explaining why I was not there. Sometimes I think it was harder on them than on me.” Dylan also takes it upon himself to work and support those in his recovery centers who will be missing holidays with their families. Last month he was supporting a woman and mother in particular who was struggling with being away from her family, “I reminded her that she was here for a reason, that the whole purpose here was to be reunited with her family, the right way, for the rest of their lives. I know what it’s like. There is a light at the end of the tunnel if you are willing to work especially during the toughest times.”

Below are some options to consider when reconnecting with family and friends, including birthdays and holidays:

1. Don’t take special occasions personally. In the end, they are “days” like any other. A reunion can be created at any time.

2. Don’t give a person coping with addiction a material item they could easily pawn or trade to feed their addiction.

3. If a person in recovery is coming to a family gathering, ask them how they feel about having people consume alcohol. If they are fine with it, share this with other family members, as having family members going into hiding to have a drink can make things awkward.

4. Make use of a 10 minute Skype, GoToMeeting or zoom call. This can be an ideal way to connect without incurring issues when being together in person. It’s a great way for family and friends to connect – a small amount of pressure to prevent a large number of secrets, lies and hurt.

5. Again, don’t take things personally. Often a person coping with addiction is not with you because they want to get better, and want to protect you. By seeing this a loving act it can be easier on everyone. It’s hard enough to be away from loved ones without feeling the additional responsibility of the pain you are causing them. This is ultimately about ending the pain altogether.

Largest Autism Sequencing Study to Date Identifies 102 Genes Associated With the Condition

In the largest genetic sequencing study of autism spectrum disorder (ASD) to date, researchers have identified 102 genes associated with risk for autism. The study also shows significant progress towards teasing apart the genes associated with ASD from those associated with intellectual disability and developmental delay, conditions which often overlap. The study results are published online January 23 in the journal Cell.

For this study, an international team of researchers from more than 50 sites collected and analyzed more than 35,000 participant samples, including nearly 12,000 with ASD, the largest autism sequencing cohort to date. Using an enhanced analytic framework to integrate both rare, inherited genetic mutations and those occurring spontaneously when the egg or sperm are formed (de novo mutations), researchers identified the 102 genes associated with ASD risk. Of those genes, 49 were also associated with other developmental delays. The larger samples size of this study enabled the research team to increase the number of genes associated with ASD from 65 in 2015 to 102 today.

“This is a landmark study, both for its size and for the large international collaborative effort it required. With these identified genes we can begin to understand what brain changes underlie ASD and begin to consider novel treatment approaches,” said Joseph D. Buxbaum, PhD, Director of the Seaver Autism Center for Research and Treatment at Mount Sinai, and Professor of Psychiatry, Neuroscience, and Genetics and Genomic Sciences at the Icahn School of Medicine at Mount Sinai.

Obtaining such a large sample was made possible by the Autism Sequencing Consortium (ASC), an international group of scientists who share ASD samples and data. Co-founded by Dr. Buxbaum in 2010 and originally funded by the Beatrice and Samuel A. Seaver Foundation and the Seaver Autism Center for Research and Treatment at Mount Sinai, the ASC is now a multiple-Principal Investigator grant funded by the National Institute of Mental Health.    

In addition to identifying subsets of the 102 ASD-associated genes that have disruptive de novo variants more often in people with developmental delays or those with ASD, the researchers showed that ASD genes impact brain development or function and that both types of disruptions can result in autism. They also found that both major classes of nerve cells—excitatory neurons, which trigger a positive and activating change in the downstream neuronal membrane upon firing, and inhibitory neurons, which trigger a negative change upon firing—can be affected in autism.

“Through our genetic analyses, we discovered that it’s not just one major class of cells implicated in autism, but rather that many disruptions in brain development and in neuronal function can lead to autism. It’s critically important that families of children with and without autism participate in genetic studies because genetic discoveries are the primary means to understanding the molecular, cellular, and systems-level underpinnings of autism,” said Dr. Buxbaum. “We now have specific, powerful tools that help us understand those underpinnings, and new drugs will be developed based on our newfound understanding of the molecular bases of autism.”

SOMOS Community Care, a New York healthcare system with over 600,000 patients, has also launched Diet ID to support its focus on preventive, innovative approaches towards community-based wellness. SOMOS JumpStart participants have begun to use Diet ID to track improvements in diet and wellness, leading to a reduction in avoidable hospitalizations.
