After spending 20 years in the healthcare industry, Dr. Jennifer Ty of Excell Home Care Inc. is familiar with the problems that plague in-home service providers — and what can be done to address them. “Not all in-home health care companies are created equally,” said Dr. Ty, CEO and founder of Excell Home Care, Inc.

No. 1: Insufficient training. California law requires a certified home health aide to complete a 120-hour training program to begin receiving certification. Title 22, Chapter 6, Article 3, Section 74707 of the California Code of Regulations Effective January 1, 1998, requires “registered nurses and licensed vocational nurses providing services in a patient’s temporary or permanent place of residence through a home health agency shall have one-year prior professional nursing experience.” Even with this regulation and program, registered nurses, licensed vocational nurses and aides can still lack the skills and experience required to give outstanding care.

“That’s why Excell Home Care requires our home care providers to go through an additional home health training,” noted Dr. Ty. “We also offer a mentoring program, which pairs the top at-home nurses with new employees.”

No. 2: Low morale. According to Home Care Pulse’s 2018 Home Care Benchmarking Study, turnover rates for caregivers are at an all-time high of 82 percent. “While job dissatisfaction can stem from a variety of sources, the main reason is low employee morale,” added Dr. Ty.

To improve morale, Dr. Ty has taken several measures to let her employees know that they are valued. Employees can nominate each other for the “You Made my Day” award, in which one person is picked daily. She also created a support group on Facebook called “Home Health Warriors.” Dr. Ty is committed to creating an engaging workplace culture on the very first day of a new employee. Its mission is to provide support to home health providers and home health nurses by giving the latest information available, provide resources that will serve as a tool in providing the highest standard of care, and offer support to licensed nurses (RN’s and LVN’s) and ancillary professionals (PT, OT, ST, CHHA and MSW) who will transition to home health.

Flip the Script Campaign Launches to Start the Conversation About Pain Among Older Adults

Flip the Script is an education initiative aimed at reducing opioid prescription misuse in older adults by building a greater understanding of pain, options for managing pain and safely using opioid pain medicine.

The Florida State University College of Medicine, Department of Geriatrics (FSU Department of Geriatrics) and the University of South Florida School of Aging Studies & Florida Policy Exchange Center on Aging (USF School of Aging Studies) announce the launch of a public information campaign to ‘flip the script’ on how Florida’s older adults think about and talk about pain.

Flip the Script is a multi-channel education initiative aimed at reducing opioid prescription misuse in older adults by building a greater understanding of pain, options for managing pain and safely using opioid pain medicine, when it is determined that opioids are the best treatment option for an individual patient.

“Chronic pain conditions are more prevalent for individuals 65 years old and older, with more than half of older adults indicating they’ve experienced some type of pain within the past 30 days,” said Dr. Lisa J. Granville, Professor of Geriatrics and Associate Chair of the Department of Geriatrics, Florida State University College of Medicine. “Pain isn’t just a part of growing older and shouldn’t hinder someone from doing the activities they enjoy the most. The heart of Flip the Script is helping people continue to do the things they love by understanding the individuality of their pain and the options for managing pain based on what matters most to them.”

Flip the Script was developed to help people:

  • Learn More about the types of pain and the impact of pain
  • Be Prepared to describe and track the pain they feel
  • Take Action to find relief and better manage pain by initiating conversations with a health care provider about their pain and ultimately share in the decision-making about how to treat pain

The campaign provides older adults and their care partners with resources, checklists and questions to help start conversations with a doctor, nurse, pharmacist or other health care professional about pain.

“Everyone reacts to pain differently,” said Kathryn Hyer, Ph.D. MPP, Professor and Director, Florida Policy Exchange Center on Aging at the University of South Florida. “Flip the Script is one of the first education campaigns that encourages people to take a whole-body approach to reduce the physical and emotional symptoms of pain. By understanding pain, the emotions that are associated with feeling pain and the variety of options available to treat pain, patients can feel empowered to start conversations with a health care provider and work toward an individualized approach to pain management.”

The opioid epidemic in the United States is a far-reaching and life-threatening problem that affects persons of all ages. For older adults, there are several factors that can contribute to opioid-related problems including conditions that may cause chronic pain, taking multiple medications, lack of understanding about medication interaction and serious side effects, including overdose, and the shame of substance misuse or addiction.

To educate Floridians about these topics, Flip the Script also includes information about pain medicine misuse among older adults, safe storage and disposal.

The FSU Department of Geriatrics and USF School of Aging Studies recognize that creating a broader awareness among older adults of the complexities of pain cannot be accomplished in a silo. The teams are grateful for the collaboration of state agencies, health care providers, community organizations and older adults themselves to ensure Flip the Script is an educational and useful resource for Florida’s older adults, their families, friends and health care providers.

Researchers at Worcester Polytechnic Institute (WPI) are developing a sensor the size of a Band-Aid that will measure a baby’s blood oxygen levels, a vital indication of the lungs’ effectiveness and whether the baby’s tissue is receiving adequate oxygen supply. Unlike current systems used in hospitals, this miniaturized wearable device will be flexible and stretchable, wireless, inexpensive, and mobile—possibly allowing the child to leave the hospital and be monitored remotely.

Ulkuhan Guler, assistant professor of electrical and computer engineering and director of WPI’s Integrated Circuits and Systems Lab, is leading the project focused on enabling sick, hospitalized babies to be untethered from wired sensors, so they can more easily and frequently be examined, held, and even allowed to go home. Guler and her team have developed a miniature oxygen monitor for babies, which measures blood gases diffusing through the skin and reports the data wirelessly.

“Extended stays in the hospital are costly and can be a strain on families,” said Guler. “And studies have shown that babies’ health improves when they are with their families. Our goal with this affordable, mobile device is to give doctors more flexibility in monitoring their patients both in the hospital and at home.”

Typically, measuring oxygen molecule levels transcutaneously involves using a system with an approximately 5-pound monitor plugged into an electrical outlet, and sensors that generally are wired to the monitor. Guler’s healthcare device will use wireless power transfer. It also will be connected to the Internet wirelessly so an alarm on a monitor in a doctor’s office or smartphone app would notify medical personnel and family members if the baby’s oxygen level begins to drop.

The device is designed to measure PO2, or the partial pressure of oxygen, which indicates the amount of oxygen dissolved in the blood—a more accurate indicator of respiratory health than a simple oxygen saturation measurement, which can be easily taken with a pulse oximetry device gently clamped on a finger. And measuring the PO2 level via a noninvasive device attached on the skin is as accurate as a blood test.

The wearable baby oxygen monitor also would be useful for adults, especially people with severe asthma and seniors with COPD, or Chronic Obstructive Pulmonary Disease, which is an incurable, progressive lung disease and the third leading cause of death in the United States, according to the Centers for Disease Control and Prevention. Guler will modify the wearable for adults, and create a related smartphone app, in another phase of her research.

Guler is collaborating with Pratap Rao, associate professor of mechanical engineering at WPI, and Lawrence Rhein, MD, chair of the department of pediatrics and an associate professor at the University of Massachusetts Medical School. Ian Costanzo and Devdip Sen, both graduate students in electrical and computer engineering at WPI, also are working with Guler to create a chip that will eventually act as the heart for the wearable device.

“The concept of the technology is that if we have more accessible data for a person of any age, we’ll be able to better take care of these patients,” said Rhein, who has been advising Guler on what’s needed in a hospital and home setting. “The idea of noninvasive, untethered, accessible data collection opens up a whole new world of care.”

The chip, designed to work inside the wearable oxygen monitor, activates the optical sensors, captures analogue signals from the sensor, handles power management, and contains required circuitry. Guler and the team have custom designed the individual circuits, such as signal capturing circuits and driver circuits for optical based read-out circuits. In the next phase of the research project, they plan to equip the chip with more circuitries to digitize the analogue signals, transmit the captured and digitized data, and create power from a wireless link. At that point, it will be a complete system on the chip.

In an interdepartmental collaboration, Guler and Rao are creating miniaturized thin and flexible sensors for the wearable healthcare devices so they will be comfortable and secure on the babies while they’re moving.

The team has written four papers (two more papers are in preparation about their research) focusing separately on miniaturizing devices, creating the prototype, oxygen measurement techniques, and building the chip. Three of the papers are under review. One, titled “A Prototype Towards a Transcutaneous Oxygen-Sensing Wearable,” was presented at the Biomedical Circuits and Systems Conference, or BioCAS 2019, in Nara, Japan in October.

Damo Consulting announces inaugural Board of Advisors


Damo Consulting, a leading digital business enablement platform for healthcare enterprises and technology firms, today announced the addition of John Glaser and Edward Marx as Advisory Board members.

John Glaser Ph.D was the founding chair of the College of Healthcare Information Management Executives (CHIME) and the Past-President of the Healthcare Information and Management Systems Society (HIMSS). Since 2015, John has been a senior executive with Cerner. He joined Cerner as a part of the Siemens Health Services acquisition, where he was Chief Executive Officer. John is focused on advancing Cerner’s strategies and thought leadership position in the industry. Prior to Siemens, John was Vice President and Chief Information Officer at Partners HealthCare. He also previously served as Vice President of Information Systems at Brigham and Women’s Hospital. He has been a Senior Advisor to the Office of the National Coordinator for Health Information Technology (ONC). He is the former Chair of the Global Agenda Council on Digital Health, World Economic Forum.

John is currently a member of the boards of InTouch Health, the American Telemedicine Association, the eHealth Initiative, PatientPing, and the National Committee for Quality Assurance (NCQA)

Edward Marx has served as Chief Information Officer at Cleveland Clinic, a $10 billion medical system where he was responsible for the development and execution of IT strategic planning and governance, driving optimal resource utilization, and team development and organizational support. He was also responsible for developing leaders and leveraging digital healthcare technologies to enable superior business and clinical outcomes. Prior to joining Cleveland Clinic, Edward served as Senior Vice President/ CIO of Texas Health Resources. In 2015, he spent over two years as Executive Vice President of the Advisory Board, providing IT leadership and strategy for New York City Health & Hospital.

Edward began his healthcare service as a janitor while in high school where his commitment to patient care began. He later served as an anesthesia technician before transitioning to the information technology field. Concurrent with his healthcare career, Edward served as an Army combat medic before becoming a combat engineer officer.
