Clinerion technology used by the Swiss Personalized Health Network to enable feasibility queries on clinical data across all five of Switzerland’s university hospitals


Clinerion was selected by the Swiss Personalized Health Network (SPHN) to be the backbone of its Federated Query System (FQS) due to the unique nature of its patented federated cloud infrastructure, which allows patient data to remain inside the IT infrastructure and under the control of each hospital, while still allowing queries of anonymized and obfuscated patient data from other hospitals in the network.

The five university hospitals of Switzerland are:

  • Centre hospitalier universitaire Vaudois – CHUV (Lausanne University Hospital)
  • Hôpitaux Universitaires de Genève – HUG (Geneva University Hospitals)
  • Inselspital, Universitätsspital Bern – INSEL (University Hospital of Bern)
  • Universitätsspital Basel – USB (University Hospital of Basel)
  • Universitätsspital Zürich – USZ (University Hospital of Zürich)

The FQS allows researchers to verify the feasibility of their project by assessing whether suitable patient data for specific research questions exist at Swiss university hospitals. The available information in the system, currently more than 70 million data elements from over 450’000 patients, who have consented to the further use of their data for research purposes, is a subset of the clinical data of all five university hospitals.

The platform includes demographic data (age and gender), diagnosis (coded in ICD-10), procedures (coded in CHOP), medication (coded in ATC), laboratory tests (coded in LOINC) and laboratory test results with units (according to UCUM). Under the project lead of the SPHN Data Coordination Center (DCC) managed by the SIB Swiss Institute of Bioinformatics, the clinical partners harmonized and standardized the anonymized clinical information, making it available for federated queries via a common front-end.

The FQS uses a customized version of Patient Network Explorer, enabling researchers to search for fully anonymized clinical information across all five university hospitals, while allowing the hospitals to retain full control over their data. It fully complies with privacy requirements, since only aggregated search results are shared with users and small patient numbers are obfuscated to prevent potential re-identification.

“USA COVID MALE PATIENT NO.1” will reveal a first-person account of David Yakobian, the original patient believed to have contracted the Coronavirus in 2019, at least a month before doctors and the United States were aware of the deadly coronavirus. Yakobian talks about what happened during his travels and encounters with others leading to falling ill with Covid. He describes how his life changed dramatically despite taking over 20 medicines daily, following 2 heart attacks, a stent, a pacemaker, and other complications. The documentary shines a light on human resilience and how patient number one is still dealing with long-term covid complications while also finding what matters in life to find hope, encouragement, and wellness.

David Yakobian says, “The focus on my story isn’t only on my status as the first Covid-19 person in the United States of America … it’s really more about how I have dealt with the long-term or long haul Covid symptoms. This era of Covid as a pandemic is not going away, yet I feel that now with more people vaccinated and more understanding on the long-haul problems there’s greater hope now than there ever was before.”

The documentary follows David Yokobian and his ability to overcome adversity through inner strength as an extremely spiritual and caring person.

Murrieta Genomics Spinout simplSEQ Announces Data Showing Value of New DNA and RNA Isolation Technology Across Liquid Biopsy Applications

The simplSEQ workflow offers many workflow advantages over other methods of cell-free DNA extraction especially speed, simplicity, reproducibility, which are critical as liquid biopsy becomes more common.

simplSEQ, Inc., a spinout of Murietta Genomics focused on technology for isolating and analyzing circulating nucleic acid to better understand health and disease, today presented a poster at the Association for Molecular Pathology (AMP) annual meeting showing the value of the simplSEQ technology in isolating DNA and RNA from whole blood. The data presented showed that the patented simplSEQ method was able to isolate more DNA, eliminate size biases, and isolate both DNA and RNA from a human whole blood sample.

Cell-free DNA (cfDNA) is becoming an increasingly common input in liquid biopsy-based molecular diagnostics for wide-ranging applications including cancer diagnosis, cancer recurrence and treatment monitoring, transplant monitoring, and non-invasive prenatal screening, among others.

Despite the growing prevalence of these applications, clinical laboratories are often challenged to mitigate effects of hemolysis in order to maximize the opportunity to find cfDNA signal amid other DNA within a sample and are typically unable to conserve sample in order to perform other assays, such as RNA sequencing. Despite advances in preanalytical technology, there has been limited innovation in nucleic acid isolation, and failure to isolate enough DNA from which to prepare a sequencing library remains a challenge across liquid biopsy applications.

The simplSEQ novel extraction protocol adds modified nucleotides to the 3’ ends of the nucleic acids which are bound to a solid matrix. It then uses template independent polymerase Terminal Transferase to extend the 3’ ends of DNA and RNA and magnetic beads enable attachment of the affinity labeled cfDNA to the matrix. When the beads are washed and magnetically separated, the isolated, bead-attached cfDNA can be used either directly in PCR based assays, or cDNA (complimentary DNA) copies can be created for downstream whole-genome or targeted next-generation sequencing assays.

“Next-generation sequencing assays performed using the simplSEQ protocol with DNA attached to beads produced better quality data with an improved percentage of aligned reads, on-target reads, and coverage uniformity,” said Brandon Young, cofounder and Chief Scientific Officer at simplSEQ. “Since these quality metrics correlate to enhancing sequencing depth, laboratories following this protocol can expect to reduce overall sequencing costs, and improve their rates of successfully completing diagnostic tests.”

This method also enables improvements to turnaround time resulting from beginning the sample extraction process while the sample is still in transit, reducing labor and consumables costs due to a simpler and more reproducible workflow, improving data quality and nucleic acid yield, and enabling laboratories to perform multiple DNA and RNA extractions from a single sample.

Mary M. Newman, MS, Co-Founder, President & CEO of the Sudden Cardiac Arrest Foundation, has been named recipient of the 2021 American Heart Association Resuscitation Science Champion Award. The award “recognizes an individual who has demonstrated commitment to the field of resuscitation science, through championing research and/or clinical improvements, supporting resuscitation scholars, and serving as a passionate advocate for our field.”

“Congratulations on behalf of the American Heart Association and the Resuscitation Science Symposium Program Planning Committee,” said Benjamin S. Abella, MD, MPhil, FAHA, and Michael W. Donnino, MD. “We thank you for your hard work in establishing the Sudden Cardiac Arrest Foundation and your dedication to resuscitation science and education to improve survival from cardiac arrest.”

Sarah Perman, MD, who introduced Newman, added, “Ms. Newman identifies connecting with patients and families affected by sudden cardiac arrest and helping them navigate healthy paths forward as her most meaningful experiences. This is echoed by the many families and survivors—those whose impressions count the most—who sing her praises for her tireless advocacy and unwavering dedication to their recoveries.”

Newman is a long-time advocate who has worked relentlessly to help save lives threatened by sudden cardiac arrest. In her remarks at the symposium, she described her career path and acknowledged those who helped guide her along the way. She saluted her mentors, James O. Page, founding publisher of the Journal of Emergency Medical Services; Hans H. Dahll, founding president of Laerdal Medical U.S.; and Peter Safar, MD, one of the inventors of cardiopulmonary resuscitation (CPR).

She also shared her perspective about progress in the field in recent years, highlighting:

  • Heart screening initiatives
  • More widespread CPR training in schools
  • More widespread deployment of automated external defibrillators (AEDs)
  • Technological advances such as notification apps like PulsePoint
  • The growing community of survivors
  • Deeper appreciation for the need to support both survivors and caregivers in their journeys.

Yet so many critical challenges remain, she said.

  • If sudden cardiac arrest is a leading cause of death in the U.S., why do most people draw a blank when they hear the phrase?
  • Why do people still confuse cardiac arrest with heart attack?
  • Why don’t all students learn CPR before high school graduation?
  • Why do we continue to see so many tragic reports of young athletes who die from cardiac arrest?
  • Why aren’t AEDs as ubiquitous as fire extinguishers?

“The Sudden Cardiac Arrest Foundation is working to address these issues,” said Newman. “As we celebrate our 15th anniversary, we are focused on:

  • Further development of our Survivor Network
  • Research in collaboration with academic universities to develop resources for survivors and caregivers, and
  • Promotion of the national Call-Push-Shock educational movement, designed to empower laypersons to help save lives.”

“As we move forward in our shared quest to improve survival,” said Newman, “Let this be our guiding principle: Every life saved is a win. Every life lost is one too many.”

Sudden Cardiac Arrest is a life-threatening emergency that occurs when the heart suddenly stops beating. It strikes people of all ages who may seem to be healthy, even children and teens. When SCA happens, the person collapses and doesn’t respond or breathe normally. They may gasp or shake as if having a seizure. SCA leads to death in minutes if the person does not get help right away. Survival depends on people nearby calling 911, starting CPR, and using an automated external defibrillator, or AED, (if available) as soon as possible.
