Deep Learning Model Classifies Brain Tumors with Single MRI Scan

This is the first study to address the most common intracranial tumors and to directly determine the tumor class or the absence of tumor from a 3D MRI volume.

A team of researchers at Washington University School of Medicine have developed a deep learning model that is capable of classifying a brain tumor as one of six common types using a single 3D MRI scan, according to a study published in Radiology: Artificial Intelligence.

“This is the first study to address the most common intracranial tumors and to directly determine the tumor class or the absence of tumor from a 3D MRI volume,” said Satrajit Chakrabarty, M.S., a doctoral student under the direction of Aristeidis Sotiras, Ph.D., and Daniel Marcus, Ph.D., in Mallinckrodt Institute of Radiology’s Computational Imaging Lab at Washington University School of Medicine in St. Louis, Missouri.

The six most common intracranial tumor types are high-grade glioma, low-grade glioma, brain metastases, meningioma, pituitary adenoma and acoustic neuroma. Each was documented through histopathology, which requires surgically removing tissue from the site of a suspected cancer and examining it under a microscope.

According to Chakrabarty, machine and deep learning approaches using MRI data could potentially automate the detection and classification of brain tumors.

“Non-invasive MRI may be used as a complement, or in some cases, as an alternative to histopathologic examination,” he said.

To build their machine learning model, called a convolutional neural network, Chakrabarty and researchers from Mallinckrodt Institute of Radiology developed a large, multi-institutional dataset of intracranial 3D MRI scans from four publicly available sources. In addition to the institution’s own internal data, the team obtained pre-operative, post-contrast T1-weighted MRI scans from the Brain Tumor Image Segmentation, The Cancer Genome Atlas Glioblastoma Multiforme, and The Cancer Genome Atlas Low Grade Glioma.

The researchers divided a total of 2,105 scans into three subsets of data: 1,396 for training, 361 for internal testing and 348 for external testing. The first set of MRI scans was used to train the convolutional neural network to discriminate between healthy scans and scans with tumors, and to classify tumors by type. The researchers evaluated the performance of the model using data from both the internal and external MRI scans.

Using the internal testing data, the model achieved an accuracy of 93.35% (337 of 361) across seven imaging classes (a healthy class and six tumor classes). Sensitivities ranged from 91% to 100%, and positive predictive value—or the probability that patients with a positive screening test truly have the disease—ranged from 85% to 100%. Negative predictive values—or the probability that patients with a negative screening test truly don’t have the disease—ranged from 98% to 100% across all classes. Network attention overlapped with the tumor areas for all tumor types.

For the external test dataset, which included only two tumor types (high-grade glioma and low-grade glioma), the model had an accuracy of 91.95%.

“These results suggest that deep learning is a promising approach for automated classification and evaluation of brain tumors,” Chakrabarty said. “The model achieved high accuracy on a heterogeneous dataset and showed excellent generalization capabilities on unseen testing data.”

Chakrabarty said the 3D deep learning model comes closer to the goal of an end-to-end, automated workflow by improving upon existing 2D approaches, which require radiologists to manually delineate, or characterize, the tumor area on an MRI scan before machine processing. The convolutional neural network eliminates the tedious and labor-intensive step of tumor segmentation prior to classification.

Dr. Sotiras, a co-developer of the model, said it can be extended to other brain tumor types or neurological disorders, potentially providing a pathway to augment much of the neuroradiology workflow.

“This network is the first step toward developing an artificial intelligence-augmented radiology workflow that can support image interpretation by providing quantitative information and statistics,” Chakrabarty added.

CISCRP & OxfordPharmaGenesis Collaborate with Partners For Article in Future Oncology

CISCRP (Center for Information and Study on Clinical Research Participation) and Oxford PharmaGenesis worked together with Daiichi Sankyo, AstraZeneca and Dr. Shanu Modi of Memorial Sloan Kettering Cancer Center in New York to write a plain language summary publication (PLSP) of the results of the DESTINY-Breast01 clinical study. The participants in the study received a treatment called trastuzumab deruxtecan, also known as T-DXd. T-DXd consists of a chemotherapy drug linked to a manmade antibody. The antibody in T-DXd is a protein that specifically targets and attaches to the HER2 protein on tumor cells. Public, patient, and health care community demand for plain language information on results of clinical trials is extremely high. For more than ten years, CISCRP — a non-profit organization — has been translating scientific clinical trial results information into plain, non-technical language for patients and the public around the world to be communicated in print and digital formats.

The teams involved—Daiichi Sankyo, AstraZeneca, CISCRP, Oxford PharmaGenesis, and Dr. Modi —worked to ensure the PLSP was easy-to-read by adding creative visuals, tables and key questions answered about the DESTINY-Breast01 trial. The PLSP was reviewed by an editorial panel inclusive of patients, patient advocates, the public and healthcare professionals to evaluate and confirm that a ‘patient-first’ approach was taken in the writing, design, and layout to help patients and caregivers understand the trial results.

The PLSP highlights the DESTINY-Breast01 clinical study. Both parts were open label, meaning that the participants, researchers, study doctors, and other staff knew what each participant was receiving. Part 1 was conducted to find out how T-DXd acted in the body, and to choose a dose to give to all participants in Part 2. To do this, the doctors gave different doses to the participants in Part 1 and took blood samples at different times during the study. In Part 1, the researchers used a computer program to randomly choose which dose of T-DXd each participant received. This helped make sure the doses were chosen fairly and comparing the results of the doses was as accurate as possible.

Study participants included 253 women aged 18 and older who had HER2 positive breast cancer that was metastatic or unresectable, had previous treatment with T-DM1, and were able to move around easily or were fully active. Study participants were from Belgium, France, Italy, Japan, South Korea, Spain, United Kingdom and the United States.

The results from Part 1 identified the recommended T-DXd dose of 5.4mg/kg for Part 2. In Part 2, T-DXd 5.4mg/kg was tested for its effect on tumor growth. T-DXd at 5.4mg/kg reduced tumor growth by more than 30% or caused the tumor to disappear in up to 60.9% (112 out of 184) of participants who had already received treatment with T-DM1 for their metastatic or unresectable HER2 positive breast cancer. Treatment response lasted nearly 15 months on average.

Serious adverse events occurred in 22.8% (42 out of 184) of patients during treatment with T-DXd in Part 2 of the study, with 15.2% (28 out of 184) of patients discontinuing treatment due to the adverse event. Adverse events that led to death included general deterioration in physical health (0.5%), pneumonia (0.5%), organ failure (0.5%) and shut down due to loss of blood (0.5%). There were two cases of interstitial lung disease (1.1%) that were considered to be related to the study treatment.

National Policy Required to Relieve Healthcare Inequities, but No Success So Far

According to the US Census Bureau minorities make up almost 40% of the US population. Healthcare inequities translate into real health problems–including fatal outcomes.

It’s time to move [the healthcare bias] conversation forward. The pandemic really highlighted just how difficult it can be for minority individuals to receive the same level of care as other patients; we need to work on setting up national policies.

The latest data from the U.S. Census Bureau shows people of color—including African-Americans, Asians, American Indians, Pacific Islanders, and Hispanic or Latino people—make up almost 40% of the United States population.(1) But several determinants of health, such as access to healthcare and systemic racism put these people at higher risk for poor healthcare outcomes, even fatal results.   • Hispanic and African-American youth are more likely to die from diabetes.
  • Some Asian populations have poorer survival compared to non-Hispanic whites for certain types of cancers at certain stages.

Healthy People 2020 defines healthcare inequities as “a particular type of health difference that is closely linked with social, economic, and/or environmental disadvantage.” In contrast, health equity is defined as the “attainment of the highest level of health for all people.”(3) But equity is closely tied to equality, which means that each individual gets the same resources or opportunities as every other individual.(4)

Part of improving healthcare for minority populations is their inclusion in the healthcare industry itself. 2017 data from the U.S. Department of Health and Human Services shows that white workers make up over 50% of studied health occupations such as health diagnosing and treating practitioners, health technologists and technicians, and healthcare support. Along with people of color, women are also underrepresented in these professional fields.(5)

There’s no one-size-fits-all solution to fixing the problem and addressing healthcare inequities on a national level will take a concentrated effort by lawmakers. As part of his campaign platform, President Joe Biden promised to:(6)

  • Defend health care protections for all.
  • Double investment in community health centers.
  • Expand healthcare insurance coverage to low-income Americans.
  • Provide a public health insurance option similar to Medicare.

On his first day in office, Biden also signed an executive order focused on advancing racial equity and supporting underserved communities.(7) But so far, efforts have fallen short. National leaders aren’t particularly focused on improving health equity. The pandemic has provided more clarity on the scope of the problem. It’s clear institutional racism still plays a part in how healthcare services are supplied, and specific patient populations, such as low-income patients, aren’t receiving the extra support they need to stay healthy. But lawmakers will need to set clear standards that have not consistently emerged from major healthcare associations.

Some healthcare entities are already aware of this and are working toward this goal. But this will take continuous effort to evaluate inclusion strategies, regroup and revise if those strategies aren’t working, and hold the right people accountable for change.

Social movements such as Black Lives Matter and #MeToo have already helped advance the national conversation on inclusion in society. Impact4Health’s Inclusion Scorecard for Population Health is helping healthcare organizations effect real change within their culture by assessing current levels of equity and identifying key activities to improve their health equity strategy. But without the support from national and state leaders, civic organizations, and prominent healthcare groups like the American Medical Association, the fight for health equity continues its uphill battle.

“We cannot achieve better population health without focusing on reducing healthcare inequities among people of color,” says Hernandez. “Until this problem is recognized and dealt with on a national level, inequities will continue to negatively impact health outcomes for these individuals.”
