Penn Wharton Budget Model Analyzes Senator Sanders’ Medicare for All


Health care ranks as one of the most important issues to voters in the Democratic primary. The Medicare for All Act of 2019 backed by Senator Bernie Sanders would create a single federal insurance program to cover all U.S. residents.

Penn Wharton Budget Model (PWBM) has produced the first integrated analysis of the Medicare for All Act of 2019, capturing health and demographic effects, interactions with other government programs, and macroeconomic changes. PWBM projects:


  • Under current law, the percent of the population without medical insurance will grow from around 10 percent today to over 27 percent by 2060. Under Sanders’ Medicare for All, the uninsured rate would essentially fall to zero by design.
  • Sanders’ Medicare for All would improve population health by 2060, reduce the share of the population that is seriously ill from 15 percent to 13 percent, increase life expectancy by two years, grow the population three percent, and increase worker productivity.
  • Taken literally, Sanders’ Medicare for All Act lacks a financing mechanism, which by long-standing Congressional Budget Office and PWBM convention implies deficit financing. Under deficit financing, the plan would reduce GDP by 24 percent by 2060, despite large efficiency gains from lower overhead and reimbursement costs.
  • As a presidential candidate, however, Senator Sanders has stated his intent to raise taxes, although without details. We analyze two alternative financing mechanisms. With premium financing, where most workers pay the same insurance premium (subsidized for lower-income workers), GDP increases by 0.2 percent in 2060. With payroll tax financing, where workers with higher wages pay more, GDP falls by 15 percent.
  • We also present alternative scenarios. Without expanding benefits to include long-term care or dental—but still eliminating most deductibles while covering all workers—GDP increases by 12 percent with premium financing. These results indicate that Medicare for All can be designed in a way that boosts economic growth.

PhotoDynamic Secures Investment from the American Association of Orthodontists (AAO)


PhotoDynamic, Inc. has secured investment funding from the American Association of Orthodontists (AAO), completing a series of successful financing rounds totaling CAD$2.3M. The Halifax-based company has developed a patented light-activated antimicrobial oral hygiene technology designed to kill bacteria which causes plaque and overcome challenges associated with traditional oral hygiene regimens. Discovered by Dr. Sherri McFarland, a world-renowned expert on photo activated chemistry, the technology completed successful clinical trials in 2018.

“While applicable to anyone’s hygiene regimen, our technology solves a particular problem for orthodontists and their patients who can face real difficulties maintaining good oral hygiene during treatment,” said Martin Greenwood, CEO of PhotoDynamic. Scheduled to launch this fall, the company’s daily oral hygiene technology combines a natural botanical foam and a light-emitting medical device. Greenwood explains that “Receiving the AAO investment provides a great opportunity to further connect with the orthodontic community and further promote the potential patient impact of our technology.”

In May 2019, the AAO established a fund to support innovation that advances the specialty of orthodontics. The fund can be used to develop new products and services for orthodontists and patients by building, partnering or investing. “The AAO thoroughly vetted PhotoDynamic. A growing number of people can benefit from interventions to manage this disruptive skin condition.

Good sleep, fulfilling activities, close relationships, positive self-esteem – these can be casualties of atopic dermatitis, the chronic skin condition often called “eczema.” “We’ve seen rates of atopic dermatitis – or ‘AD’ – climb,” notes Angie Seelal, R-PAC specializing in dermatology with Advanced Dermatology PC. “A growing number of people can benefit from interventions to manage this disruptive skin condition.”

Atopic dermatitis usually starts at a young age, most often under five. Dry, scaly, red patches of skin develop, often on the face, arms or legs. “These affected areas are extremely itchy,” explains Seelal, “interfering with sleep and triggering scratching that can bring additional skin problems: worsened inflammation and the risk of infection.”

Following the first occurrence, symptoms may subside and flare up. About half will see symptoms cease completely by adulthood. For others, the red, itchy patches can continue to distressingly interfere with work and play.

Since the 1970s, AD cases have increased, with some industrialized countries’ numbers tripling. “Research has yet to fully identify why,” says Seelal, “but in the U.S., about 9.6 million of those under eighteen have AD, and more than 16 million adults are dealing with the condition.”

Atopic dermatitis is not contagious, despite its appearance. Research points to a confluence of genes and environment that trigger an immune system misfire. Genetically, there is a connection between AD, asthma and hay fever; parents with any one are more likely to have children who develop AD. “Research has also focused on the skin protein ‘filaggrin,’ which helps maintain a protective skin barrier,” explains Seelal. “A mutation in the gene that codes for filaggrin may be implicated in AD.”

At this point, there is no cure for AD. “But,” emphasizes Seelal, “we have effective treatments that can help patients control symptoms. And early treatment can ward off long-term damage, like lichenification – the skin thickening that can result from chronic scratching.”

To help patients manage AD, Seelal suggests the following:

Six Tips to Help Control Atopic Dermatitis

1. Moisturize, moisturize, moisturize: “Dry skin is the enemy,” observes Seelal, “and we also want to build up the skin’s protective barrier. With AD, a super-effective moisturizer is needed. Ointments’ higher oil content provides the most protection. People should avoid any additives that may be irritants. Ask your dermatologist. And the National Eczema Association’s website has a list of evaluated products. Do a trial run on a small spot to check for sensitivity. Your moisturizer should be handy at every household sink for post-washing and packaged to go for re-application when out and about.”

2. Bathe smart: “An effective bath or shower routine helps us maximize moisturizing,” says Seelal. “‘Effective’ means lukewarm water instead of hot, using a gentle cleanser for sensitive skin, and staying away from scrubbing and soap. Then, we’re set up for moisturizing.”

3. Follow the three-minute rule: “After washing,” explains Seelal, “we’re primed to moisturize, but we need to do this in a timely way: an oil-rich moisturizer should be applied within three minutes to seal in moisture. If we have topical medications, those should be applied first, followed by the moisturizer.”

4. Topical treatments? Understand the options: “Moisturizing is required,” states Seelal. “But often it’s not enough. Corticosteroids, topical calcineurin inhibitors (TCIs), and PDE4 inhibitors are topical treatments with different mechanisms for tamping down the body’s immune response to decrease inflammation. Patients should talk with their doctor to get a clear understanding of how each medicine works and its side effects. Steroidal treatment, for example, is often limited, while TCIs may be long term.”    

5. Light therapy requires a commitment: “Phototherapy – most often in the form of UVB rays – can help up to 70 percent of patients who haven’t improved with topical treatment,” notes Seelal. “But it involves frequent ongoing treatments.”

6. Learn about targeted ‘biologic’ treatment: “In 2017,” states Seelal, “the FDA approved Dupixent® as the first ‘biologic’ drug for AD: It works on a genetic level to block proteins that trigger skin inflammation. Patients receive every-other-week injections.”

“Your dermatologist,” concludes Seelal, “can help customize a plan so that AD doesn’t devastate your life.”
