Understanding REMS During a Public Health Emergency

An FDAnews/CenterWatch Webinar

How does one handle risk evaluation and mitigation strategy (REMS) programs in the age of COVID-19?

The FDA just issued guidance related to pharmaceutical sponsors who fail to meet certain REMS requirements during the COVID-19 outbreak. They won’t face FDA enforcement action. This guidance will remain in effect until the public health emergency is over.

During the pandemic one may find it difficult to complete some program activities such as finish lab tests or imaging studies required by REMS as patients self-isolate or are quarantined. How should one proceed?

Catherine Baldridge, MS, owner/founder of Essential Pharmacovigilance, LLC, will cover REMS critical components, explain the importance of advanced planning and proper resource allocation to prepare one for the ultimate success of one’s REMS programs during this public health emergency:

  • Specific elements to assure safe use (ETASU) in one’s REMS
  • Recommended lead time for preparing for a REMS program in order to provide adequate scheduling for companies and contract research organizations
  • Recommended quality and compliance processes one needs to consider during an implementation
  • Importance and value of cross-functional engagement when developing and implementing a program
  • Impact of the program on the global pharmacovigilance (PV) system, including financial impact, relevance to the qualified person responsible for pharmacovigilance (QPPV) and risk management plan (RMP) programs
  • State-of-the-art operational PV tactics (tools, SOPs, training) one must know for implementing and managing a program

This is the first time this topic is being addressed from an industry perspective. If one is planning for a REMS this opportunity is critical.

Link Between Diabetes and Periodontal Disease Highlights Need for Excellent Dental Hygiene, says Santa Clarita Valley Dental Care

Serving the entire Santa Clarita Valley area and beyond.

Maintaining one’s oral health appears to have ramifications in virtually all other areas of one’s health.

A March 9 article on Medical Daily reports on a study that found strong links between lifelong oral hygiene habits, a severe form of gum disease known as periodontitis, and diabetes. One of the primary findings of the study, which occurred in South Korea, saw that individuals who were 51 or younger were 14 percent less likely to have diabetes than those who only brushed once a day or not at all. The study also found that people who suffered from diabetes were likely to have periodontitis as well. Santa Clarita Valley Dental Care says that this study echoes studies with similar findings, says that maintaining one’s oral health appears to have ramifications in virtually all other areas of one’s health.

Santa Clarita Valley Dental Care notes that good dental hygiene appears to play a key role in protecting one’s total wellbeing. The dental clinic says this starts with brushing after every meal for about two minutes, paying careful attention to reach all surfaces of the teeth. The clinic adds that many patients sometimes give a little less attention to the backs of their teeth, which can cause a variety of problems down the line, including tooth decay, even if the front surfaces of the teeth appear normal.

Flossing is also important, the clinic notes, as it breaks up the buildup of plaque and other contaminants like bits of food in between the teeth. The clinic says that it is important to remember not to brush and floss too aggressively though, as this can be counter-productive. Over time, aggressive or harsh brushing can wear down tooth enamel, exposing the yellow dentin layer, and damage the gums. The clinic adds that a receding gum line is sometimes attributed to harsh brushing because the gums lose their ability to heal properly after a long period of abuse. Naturally, the clinic says that combating this is quite simple: use soft bristle toothbrushes, gentle brushing motions, and floss tooth gaps lightly, taking extra care to be gentle around the gums.

Santa Clarita Valley Dental Care adds that sticking to good dental habits even when they are not overly convenient can save patients a lot of time, money, and pain in the future. Of course, the clinic also adds that even the most diligent tooth brushers are still likely to suffer from some kind of oral malady even if it’s just the need for one or two dental fillings at some point in their lives, which is when routine dental checkups come into play. The clinic notes that visiting a dentist for regular dental exams can ensure that there are no oral ailments in the works. On the other hand, if there are issues and they are caught early enough, the treatment plan may be far less severe, the clinic adds.

Ultimately, Santa Clarita Valley Dental Care concludes that there is practically zero incentive for not taking great care of one’s teeth. For more information on the clinic, readers can visit https://www.scvdentalcare.com/ or call (661) 493-8866.

MicroGenDX Receives FDA Emergency Use Authorization for COVID-19 Test to Expand Testing Capacity

The hurdles to COVID-19 testing have been daunting. We speak daily with governors, business owners and hospital administrators looking for better options. The additional support of the FDA through its EUA opens the door for us to meet the demand and utilize our full testing capacity.

MicroGenDX Laboratories, the world leader in providing PCR and Next Generation DNA Sequencing in microbial identification, has received FDA Emergency Use Authorization for its validated real-time Reverse Transcriptase PCR (rRT-PCR) assay for detecting the SARS- CoV-2 virus.

Increasing evidence supports the use of sputum and saliva for rapid and accurate detection of SARS-CoV-2. Since late March 2020, MicroGenDX has collected and tested the largest sampling of saliva and sputum (more than 14,000 tests) and ongoing analysis continues to find these samples to be high-quality and more sensitive than nasal samples. A recent study out of Yale also found saliva to be more sensitive for SARS-CoV-2 detection in patients than nasopharyngeal swabs.

The test allows patients to self-collect sputum, a mucus-like mixture of saliva and other elements that come from the respiratory tract. Additionally, the EUA is granted for the lab’s testing with NP swabs, of which the lab has performed more than 16,000 tests. However, due to supply shortages for swabs and PPE (Personal Protective Equipment) and in light of mounting scientific validation, the lab sees greater potential for increased sputum and saliva testing. MicroGenDX is currently performing the requested paired sample analysis for its saliva-only test and anticipates FDA support on it in the near future.

MicroGenDX is listed on the FDA Website as a resource laboratory for COVID-19 Testing as “Southwest Regional PCR dba MicroGenDX Laboratory.”

MicroGenDX was one of the first national laboratories to validate and compare the testing of a variety of sample sources: sputum, saliva and nasopharyngeal (nasal). As a high-complexity lab developing and processing its own validated Laboratory Developed Test (LDT), and not a test kit, all of the lab’s COVID-19 tests are validated under its Clinical Laboratory Improvement Amendments (CLIA) license. However, MicroGenDX sought EUA from the FDA to further clarify the validity of the test for hospitals, clinicians and patients.

“We continue to work tirelessly to make a significant impact on the testing crisis. The hurdles to COVID-19 testing have been daunting. We speak daily with governors, business owners and hospital administrators looking for better options. The additional support of the FDA through its EUA opens the door for us to meet the demand and utilize our full testing capacity. We have bolstered our existing diagnostic teams to scale testing in accordance with the growing influx of tests we’ve started performing since last month,” said Rick Martin, CEO of MicroGenDX.

For more information on how to receive tests, please visit https://microgendx.com/covid19/.

Additional Test Information

The benefits of increased saliva and sputum testing are immense. These tests do not require swabs and other supplies that are experiencing a shortage. A provider does not have to perform the sample collection, thus decreasing patient-to-provider transmission of the virus. The patient does not suffer an uncomfortable collection experience.

In addition to a simplified collection process, MicroGenDX’s testing turnaround is 24-hours, eliminating the chances that the RNA will break down waiting for processing, which has led to false negatives for tests at other laboratories.

MicroGenDX runs its COVID-19 Rapid PCR Test at its diagnostic laboratory in Lubbock Texas, and accepts samples from every state.

About MicroGenDX

MicroGenDX is the global leader in molecular testing services through PCR and Next Generation DNA Sequencing to identify microbes in human samples having run more than 400,000 DNA tests over the last 12 years. We are a state-of-the-art, CAP- accredited, CLIA-licensed Molecular Diagnostic facility backed by a team of molecular biologists, biochemists, bioinformaticians, computer scientists, and physicians. Our advanced instrumentation provides the industry’s most informative microbial diagnostic tests for ENT, orthopedic, urology, wound care, OB/GYN and other areas of medicine.

Charles E. Crutchfield III, M.D. Requests that Governor Walz and Legislators Repeal the MinnesotaCare Tax

Charles E. Crutchfield III, M.D.

“Having physicians, hospitals, and other caregivers step up to the plate, risk the health of themselves and their families, and then to place a tax on them for so heroically answering the call in this pandemic flies in the face of all that is fair and reasonable,” states Dr.Crutchfield.

Dr. Crutchfield sent a letter to the Governor and several key legislators, The Honorable Melissa Hortman, The Honorable Kurt Daudt, The Honorable Paul Gazelka, The Honorable Susan Kent, The Honorable Michelle Benson, The Honorable John Marty, The Honorable Rena Moran, The Honorable Deb Kiel, asking the Minnesota Care tax to be discontinued. “Having physicians, hospitals, and other caregivers step up to the plate, risk the health of themselves and their families, and then to place a tax on them for so heroically answering the call in this pandemic flies in the face of all that is reasonable, just and fair. It is time to repeal this tax,” states Dr.Crutchfield.

Subject: Request to Eliminate MinnesotaCare Tax during COVID-19 Pandemic

Dear Governor Walz:

As the former President of the Minnesota Association of Black Physicians that backed you for Governor – our first-ever endorsement – thank you for your leadership during this time. As we navigate this crisis, I ask you to please review the equity and fairness of the Minnesota Medical Provider Tax.

Physicians are putting their lives on the line to care for critically ill patients. It seems terribly unfair to tax them while they are doing this or caring for any other patients.

While the recent MinnesotaCare Tax grace period may help clinics like ours already forced to furlough staff, the pandemic reveals the negative impacts of this aged tax once created to fund healthcare for those without coverage. New federal and state initiatives have reduced that need, and while the tax provides coverage for some, its surpluses are used on programs not intended.

The pandemic shows the tax to be a financial and administrative burden on those putting their lives on the line to care for patients afflicted by COVID-19 as well as a cost to patients themselves. It goes against Minnesotan common sense to hit our exhausted medical community with a tax of attainder to pay for a benefit intended for all. No other critical industry, such as farming or construction, faces a targeted tax on those who produce public value.

The MinnesotaCare Tax exclusively hits patients and doctors at a time when they can least endure it. Like other small businesses, independent clinics have suffered enormous losses, and patient avoidance has led to massive layoffs. People needing care of all types are now hit with a tax when scrambling to make ends meet while unemployed or losing their small businesses.

Our clinic conducts skin cancer evaluations, where delay in diagnosis can cost a patient their chance of survival. We’ve already furloughed 40% of our staff and may lay off more if the crisis continues. The MinnesotaCare Tax has worsened this painful circumstance.

Governor Walz, thank you for all you do and for taking the time to read my letter. Please consider my request to dispense with a tax that burdens those giving their best at a time when they can least afford it. I’d be pleased to speak further with you or any member of your staff.

Sincerely,

Charles E. Crutchfield III, M.D.

The mouth is the gateway to the body. “Your body can affect your mouth and likewise, your mouth can affect your body,” says Pamela McClain, DDS, president of the American Academy of Periodontology. “Taking good care of your teeth and gums can really help you live well longer.”

According to The Mayo Clinic, studies suggest that oral bacteria and the inflammation associated with severe gum disease, or periodontitis, may play a role in some diseases. In order to prevent this, The Mayo Clinic suggests practicing good oral hygiene on a daily basis, including the use of mouthwash to remove food particles left after brushing and flossing.

SmartMouth’s suite of Activated Oral Rinses deliver oral health benefits through their patented two solution system with Zinc Ion Technology.

SmartMouth is the only mouthwash that contains Activated Zinc Ions. When the two SmartMouth solutions are combined, billions of activated Zinc Ions are released which bind to the bacteria in the mouth, inhibiting the bacteria’s ability produce stinky sulfur gas, the real cause of bad breath, for 12 hours with every rinse.

SmartMouth’s Resident Dentist and ADA Member Dr. Elizabeth Clary affirms, “Nothing works better than SmartMouth’s Activated Zinc Ion Technology.”

Zinc not only disrupts bacteria in the mouth, according to The National Institutes of Health, when Zinc is administered as a lozenge or Zinc-containing syrup that temporarily “sticks” in the mouth and throat, allowing Zinc to make contact with rhinovirus in those areas, several studies have found Zinc to significantly reduce the duration of cold symptoms, including cough, nasal discharge and muscle aches. Additionally, a Cochrane review concluded that “Zinc (lozenges or syrup) is beneficial in reducing the duration and severity of the common cold in healthy people, when taken within 24 hours of onset of symptoms.”

For a healthier state of the mouth, THINK ZINC. Think SmartMouth.
