5 Questions for Your Doctor

Artificial disc replacement doesn’t stop patients from bending or flexing normally, making it an increasingly popular option for chronic back or neck pain

Nearly all of us know someone who had surgery to replace their bum hip or knee with an artificial joint. But artificial discs? Fewer have heard of this increasingly prevalent orthopedic surgery, according to Praveen Kadimcherla, MD, of Atlantic Spine Center.

Approved by the U.S. Food and Drug Administration in 2004, artificial disc replacement implants a synthetic disc into the spine to replace a worn or damaged disc. The procedure’s goal, Dr. Kadimcherla says, is to relieve chronic back pain while enabling more normal range of motion that might be possible with other back procedures, such as spinal fusion.

While an estimated 8 in 10 American adults will experience back pain at some point, according to the National Institutes of Health, surgery is rarely necessary and only used when pain doesn’t improve with conservative treatments such as exercise, physical therapy or pain relievers. When spinal fusion surgery is performed to fuse painful vertebrae into a solid bone, fusion can prevent normal spine motion.

“But artificial disc replacement doesn’t stop patients from bending or flexing normally, making it an increasingly popular option for chronic back or neck pain,” Dr. Kadimcherla explains.

Best questions to ask in advance

Just because an innovative surgical option for back pain has emerged doesn’t make it right for everyone with back or neck pain, Dr. Kadimcherla notes. He suggests being ready to talk with your doctor about the possibility of artificial disc replacement with these 5 questions:

1.    Am I a good candidate for artificial disc replacement?

Disc replacement is a better option for certain patients, including those whose back pain originates in only 1 or 2 discs in the spine. People with slipped or herniated discs may be good candidates for the procedure, but not those who suffer from degenerative disc disease, which can affect the entire spinal column. It’s also important not to have significant joint disease or spinal nerve compression, Dr. Kadimcherla says. “Good candidates for this surgery aren’t excessively overweight and they haven’t undergone spinal surgery before,” he adds. “They also can’t have scoliosis or another deformity of the spine.”

2.    What happens during artificial disc replacement surgery?

In a 2- to 3-hour procedure, you’ll receive general anesthesia while lying on your back. While you’re asleep, surgeons will make an incision in your abdomen (if your disc replacement is occurring in the back), moving aside organs and blood vessels to allow access to your spine. If a disc is being replaced in your neck, the incision is made at the front of the neck. Your damaged spinal disc(s) will be removed and artificial disc(s) will be inserted in their place. After your incision is closed, you’ll go to a recovery room.

3.    Are there any risks to artificial disc replacement surgery?

Like any surgical procedure, artificial disc replacement poses certain risks, however minor. The operation requires greater access to the spine than other spinal surgeries, Dr. Kadimcherla notes. Potential surgical risks include infection; dislocation of the new disc; implant failure or breakage; implant loosening or wear; stiffness of the spine; narrowing the spine due to a breakdown in spinal bones; and blood clots in the legs.

4.    How can I prepare for artificial disc replacement?

Your doctor will take a detailed medical history and perform a thorough physical exam. You may also need imaging tests such as X-rays or an MRI or CT scan before surgery, along with blood tests. “Make sure to inform your doctor of all prescription and over-the-counter drugs you take as well as vitamins, herbs or other supplements, since medications and supplements can affect blood clotting and other aspects of healing,” Dr. Kadimcherla says. Just before surgery, you may be told not to eat or drink anything for several hours.

5.    What is recovery like from artificial disc replacement?

The days just after surgery should allow rest and healing, Dr. Kadimcherla says. You may stay in the hospital for a few days, and you won’t be allowed to drive right away when you get home. You may also need help at home with activities such as dressing, cleaning and shopping. Pain will be controlled with appropriate medications and you’ll be encouraged to walk about within the first day, he says. “While you’ll need to avoid any jarring movements for awhile, recovery should proceed steadily,” Dr. Kadimcherla explains. “Walking, stretching, and doing gentle exercises will all promote your return to normal life.”

Highmark, Boehringer Ingelheim Announce Value-Based Contract for COPD Medication Stiolto® Respimat®


Highmark and Boehringer Ingelheim today announced a new value-based contract for Boehringer Ingelheim’s Stiolto® Respimat® (tiotropium bromide & olodaterol) Inhalation Spray for the treatment of chronic obstructive pulmonary disease (COPD). This marks the second value-based contract between the two companies. It is also the first contract for Highmark to include Medicare plans in addition to commercial health plans.

This contract confirms the commitment of both companies to improving patient care and lowering overall health care costs by linking reimbursement to patient outcomes. Using the Global Initiative for Chronic Obstructive Lung Disease (GOLD) guideline-based care as a benchmark, Highmark will evaluate whether Stiolto Respimat reduces the total cost for both in-patient and out-patient COPD care for members compared to the costs for members who use other drugs to treat the health condition.

COPD is a progressive respiratory disease that affects more than one in 20 adults in the U.S. The condition is the fourth-leading cause of death in the U.S., and it is projected to cost the U.S. health care system about $49 billion in 2020, according to the Centers for Disease Control and Prevention (CDC). The CDC estimates that more than 16.4 million work days are lost each year due to the complications of COPD.

The new agreement, which expands Highmark’s efforts to address high prescription drug costs and enhance patient care, is the second agreement between Highmark and Boehringer Ingelheim. The first agreement focused on a drug from Boehringer Ingelheim that treats adults with type 2 diabetes who have cardiovascular disease.

IMPORTANT SAFETY INFORMATION AND INDICATION for STIOLTO RESPIMAT

Do not use STIOLTO if you have asthma. People with asthma who take long-acting beta2-agonist (LABA) medicines, such as olodaterol, (one of the medicines in STIOLTO), without also using a medicine called an inhaled corticosteroid, have an increased risk of serious problems from asthma, including being hospitalized, needing a tube placed in their airway to help them breathe, or death.

Do not use STIOLTO if you are allergic to tiotropium, ipratropium, atropine or similar drugs, olodaterol, or any ingredient in STIOLTO.

Call your healthcare provider or get emergency medical care if you experience symptoms of a serious allergic reaction including: rash, hives, itching, swelling of the face, lips, tongue, throat, and difficulties in breathing or swallowing.

Get emergency medical care if your breathing problems worsen quickly or if you use your rescue inhaler but it does not relieve your breathing problems. Call your healthcare provider if breathing problems worsen over time while using STIOLTO.

Do not use STIOLTO more often than prescribed by your doctor. Do not use STIOLTO with other LABAs or anticholinergics.

Do not use STIOLTO for treating sudden breathing problems. Always have a rescue inhaler with you to treat sudden symptoms.

Tell your doctor about all your medical conditions including heart problems, high blood pressure, seizures, thyroid problems, diabetes, kidney problems, glaucoma, enlarged prostate, and problems passing urine.

STIOLTO can cause serious side effects, including sudden shortness of breath that may be life threatening, fast or irregular heartbeat, increased blood pressure, chest pain, tremor, headache, nervousness, high blood sugar, or low blood potassium that may cause muscle weakness or abnormal heart rhythm. If any of these happens, stop taking STIOLTO and seek immediate medical help.

STIOLTO can cause new or worsening eye problems including narrow-angle glaucoma, and can increase the pressure in your eyes, which can cause the following symptoms: eye pain, blurred vision, seeing halos or colored images along with red eyes. If you have any of these symptoms, stop taking STIOLTO and call your doctor right away.

STIOLTO can cause new or worsened urinary retention. Symptoms of urinary retention may include difficulty passing urine, painful urination, urinating frequently, or urinating in a weak stream or drips. If you have any of these symptoms, stop taking STIOLTO and call your doctor right away.

The most common side effects of STIOLTO are runny nose, cough, and back pain.

Tell your doctor about all the medicines you take, including prescription and over-the-counter medicines, eye drops, vitamins, and herbal supplements. STIOLTO and certain other medicines may affect each other.

STIOLTO is for oral inhalation only.

The STIOLTO cartridge is only intended for use with the STIOLTO RESPIMAT inhaler.

SureFire CPR, an organization dedicated to educating people in first aid and cardiopulmonary resuscitation in Orange County California, additionally offers a scholarship for students pursing degrees in the medical field. The scholarship is now in its fourth year and has allowed students to subsidize the costs of obtaining degrees in these necessary academic areas. It is a prize of $1,000 and will allow students to pursue their field of choice.

The scholarship will be awarded to a student who is a superior academic, who showcases dynamic leadership qualities, and who is involved closely with their communities through service. Students enrolled in classes at schools offering training in medical fields like nursing, physical therapy or respiratory therapy are eligible to apply and students can be full-time, part-time or incoming. Previous scholarship rewards or monetary prizes have no impact on SureFire CPR’s award.

Applications are currently open. Those applying for the scholarship must provide a copy of an acceptance letter or a transcript verifying enrollment in a medical field. Also required is an essay of 200-300 words responding to the following questions: What was the defining moment when you knew that you wanted to pursue a career in the medical field? How do you plan to contribute to the medical community/humanity?

PMB wins big with three 2019 HREI Awards™ for medical developments in Arizona and California

One of PMB’s 2019 award winners was the 1100 Van Ness MOB in San Francisco.

We’re proud of the PMB team, which exhibited a great deal of creativity and determination to overcome a number of obstacles and deliver these much-needed medical facilities to their respective communities.

PMB was the big winner this year, becoming the first organization to have three winning healthcare real estate (HRE) development projects in a single year in the prestigious, national HREI Insights Awards™ program.

The PMB winners were: 1100 Van Ness, San Francisco, in the Best Medical Office Building (MOB) 100,000 or more square-foot category; Clinicas de Salud El Centro Health Center, El Centro, Calif., in the Best MOB 25,000-49,999 square-foot category; and Goodyear Medical Plaza, Goodyear, Ariz., in the Best Renovated or Repurposed Healthcare Facility category.

The annual awards, the first exclusively dedicated to recognizing excellence in the areas of HRE development and executive leadership, are presented by Healthcare Real Estate Insights™ (HREI) magazine, a national digital and print magazine that covers HRE development, financing and investment. The awards were presented in nine categories during the GlobeSt Healthcare Conference in Scottsdale, Ariz., Dec. 4.

“We are thrilled that the three healthcare development projects PMB entered in this year’s HREI awards were selected winners,” says Mark D. Toothacre, President of PMB. “We’re proud of the PMB team, which exhibited a great deal of creativity and determination to overcome a number of obstacles and deliver these much-needed medical facilities to their respective communities. These three awards are a great honor and a testament to our team’s hard work.”

The 250,000-square-foot, nine-story 1100 Van Ness facility is the first new multitenant MOB constructed since the 1980s in the highly competitive and regulated real estate market in San Francisco. The building is in a prime central location across the street from the new 274-bed California Pacific Medical Center and affords easy access to major San Francisco neighborhoods. The facility has enabled Sutter Health to better serve the growing population and comply with new California seismic building codes. The MOB and Sutter’s new master plan are allowing the health system and other private practices in the facility to consolidate multiple offices and services, which is decreasing their overall footprint and increasing practice and delivery efficiencies.

Murray Wolf, Publisher of HREI magazine, said the HREI Awards judges had a number of positive comments about PMB’s three winning entries, especially the 1100 Van Ness project. “One of the judges said that 1100 Van Ness was the most complicated project of this year’s competition,” says Mr. Wolf. “Another judge said, ‘A once-in-a-career project in terms of scope, complexity, size and quality. An epic development process and schedule in a great American city. This one has got to be the hands-down winner!’”

The new 45,000-square-foot Clinicas de Salud del Pueblo El Centro Health Center (Clinicas) was needed to expand healthcare services and access for the medically underserved, low-income community of El Centro, Calif., and the greater Imperial Valley. The new clinic also provides a much needed location for local students to complete their medical training/internship programs. PMB arranged for Federal New Markets Tax Credits (NMTC) to help finance the new MOB with 100 percent of the NMTC benefit going to Clinicas. The new MOB is enabling Clinicas to launch new services, including pharmacy, dental and ophthalmology, create 152 new permanent jobs and increase patient capacity by 85 percent.

One HREI Awards judge said of the Clinicas project, “Kudos for delivering such a nice project to an underserved rural area.” Another judge echoed that sentiment: “Love the concept of using New Market Tax Credits and building in underserved areas.”

The Goodyear Medical Plaza in Goodyear, Ariz., was a challenging adaptive reuse project in which PMB converted the outdated, 1980s former Harkins Palm Valley movie theater into a modern, state-of-the-art medical facility. The new 50,000-square-foot MOB is allowing Abrazo Medical Group, an employed physician group for Phoenix-based Abrazo Community Health Network, to expand in the marketplace and serve the rapidly growing population. It also is enabling the physician group to provide additional office space for its newly recruited physicians. PMB brought the repurposed facility to market approximately 50 percent faster than a traditional ground-up construction project, allowing Abrazo to expeditiously reduce its physician shortage in the market.

The HREI judges were equally complementary about the Goodyear Medical Plaza. One noted, “Great reuse of a unique building.”

PMB-developed projects have been named finalists a total of 11 times and winners seven times in the seven years of the HREI awards.

Dr. Goel is board certified in physical medicine and rehabilitation with subspecialty certifications in pain medicine and neuromuscular medicine. He is also board certified by the American Board of Electrodiagnostic Medicine and is a Fellow of the Royal College of Surgeons of Edinburgh in Scotland. He has received numerous accolades from medical publications over the years, including “America’s Best Physicians,” “America’s Top Physicians” and “Top Doctor.”

The highest-ranking nominated physicians are asked to serve on a selection panel, which helps to determine the final list of Super Doctors. The final list represents 5% of Texas physicians. Walmart-Amazonian Culture Creates More Players in the Healthcare Retail Space

What matters to consumers of every generation is the ability to understand their options and make what for them is the right choice.

Earlier this fall, Walmart opened a new 10,000-square-foot multi-service health center in the Atlanta suburb of Dallas, Ga. A second center, also more than six times the size of Walmart’s existing in-store care clinics, is planned for early next year [1]. More recently, Amazon has announced plans to extend its in-store pickup option, already available at 100 Rite Aid pharmacies, to a total of 1,500 GNC nutrition stores, Health Mart pharmacies and Stage Stores retail outlets [2]. “Increasingly,” says Dave Rich, CEO of Ensurem LLC, “healthcare offerings—including health insurance—are being made more easily available to people as they go about their everyday lives.”

Rich, whose company is an online, multi-carrier insurance brokerage, describes these developments as further signs that healthcare is an industry in transition—including his company who is at the forefront of the Medicare-insurance retail market. To point: The number of walk-in urgent care clinics in the U.S. increased almost 20 percent between 2015 and 2018 and now handle more than 29 percent of all primary care visits and nearly 15 percent of all outpatient physician visits [3]. These facilities are rapidly expanding not just in terms of importance, but also of capability; Walmart’s new health center in Georgia, in addition to primary and urgent care, offers lab testing, X-rays, EKG tests, psychological counseling, dental care, optical care and hearing services [4].

The emergence of walk-in clinics and expanded health services in Walmart locations points to the growing desire for consumers to access healthcare when they need it and where they want it.

Along with an expanding range of medical services, notes Rich, in-store clinics and kiosks serve as a frontline source of essential healthcare-related information. The Annual Enrollment Period (AEP) for Medicare, for example, affects millions of people, as 10,000 Baby Boomers turn 65 every day [5]. While some beneficiaries may qualify for special enrollment periods, changes to existing Medicare coverage for all other beneficiaries must be made during the annual time window from October 15 to December 7. Outside of AEP, Medicare Advantage enrollees are only allowed to make one additional change during the Medicare Advantage Open Enrollment Period (OEP) from January 1 to March 31. Making the wrong decision can have serious long-term consequences, yet the options are often confusing and unclear [6].

To help seniors sort through these options and obtain the coverage they need, Ensurem, which has relationships with most of the major health insurance issuers in the U.S., maintains a presence in kiosks within several retail store chains. To date, with the help of companies specializing in health kiosk placements, Ensurem has deployed its first-in-industry software solutions to 4,500 of these kiosks in pharmacies and retail locations throughout the country to simplify the Medicare buying process.

Ensurem’s kiosk representatives provide information about, and help with, a wide range of other insurance issues as well. While Medicare coverage is a primary concern of Boomers, all age groups, says Rich, seem to enjoy face-to-face interaction. Warby Parker, he notes, which began life as a Millennial-centric, online-only optician, now has 96 bricks-and mortar stores in the U.S. and three in Canada [8]. The quintessentially Millennial haberdasher Untuckit, also originally an online-only operation, now has a chain 75 stores, coast to coast [9].

“What matters to consumers of every generation,” Rich says, “is the ability to understand their options and make what for them is the right choice. Whether a consumer accesses our platform purely online or through a visit to an instore kiosk, our goal is to simplify and clarify the process of evaluating and purchasing insurance—whatever kind of insurance that consumer needs.”

Ensurem has entered the retail Medicare solutions market with a large and swift capacity to assist over 300,000 seniors per month trying to navigate Medicare coverage options, including:

1.    Online marketing to create awareness and drive seniors to their easy-to-navigate Medicare sales platform;

2.    Call centers that log over 2,000 calls per day answering questions from confused seniors as to their benefits;
