Periodontal Care Importance Highlighted by Possible Increased Risk of Esophageal and Gastric Cancer, says Beverly Hills Periodontics & Dental Implant Center

Dr. Peiman Soleymani’s oral health practice treats patients from the Westside and all of Greater Los Angeles.

Gum disease is nothing to shrug off and proper periodontal care is a must.

An October 20 article on Express reports on a pair of Harvard studies that found a strong link between a patient’s history of gum disease and a higher chance of developing esophageal or gastric cancer. The two studies together looked at a sample size of 150,000 participants, noting that patients in the study who had gum disease had a 43% higher chance of esophageal cancer and a 52% higher chance of stomach cancer than those without that history. Oral health clinic Beverly Hills Periodontics & Dental Implant Center says that the study results highlight that, whatever else may be true, gum disease is nothing to shrug off and proper periodontal care is a must.

The West Los Angeles area clinic says periodontal disease has already been tied to many serious health conditions such as heart disease, diabetes, and Alzheimer’s. Nevertheless, the addition of certain types of cancers only strengthens the point that seeking treatment for any stage of gum disease should be a priority for patients. Without periodontal disease treatment, the early stages of gum disease can rapidly develop into periodontitis and, if that is left untreated, tooth loss.

The oral health center notes that the most obvious symptoms of gum disease include bleeding gums, especially when eating, brushing, or flossing; recessed gums, swelling, pus, loose teeth, and chronic oral pain or discomfort. However, the center also says that gum disease can progress without any notable symptoms until it reaches an advanced stage. As such, the center says it’s exceedingly important to have a specialist check a patient’s teeth and gums periodically to make sure no hidden ailments are developing.

Beverly Hills Periodontics & Dental Implant Center says that treatment for periodontal diseases tends to vary by patient, but generally starts with scaling and root planning – sometimes referred to as deep cleaning. This procedure, the center continues, involves thoroughly disinfecting pockets and surfaces of the oral cavity to significantly reduce the amount of harmful bacteria. The surfaces of teeth are also smoothed out to make it harder for bacteria and other contaminants to take hold, limiting the chances of a resurgence of the disease. In milder cases, this treatment combined with good oral hygiene is enough to arrest the problem. However, when the problem is more advance, numerous more advanced methods are available and can lead to positive outcomes in most cases.

The beauty and cosmetics industries have done little to address these very real issues that affect average men and women every day.

The Essential Secrets Masterclass addresses the underlying causes of skin aging and presents real, natural solutions that individuals can incorporate into their daily routines immediately. The Masterclass includes the latest breakthroughs from leading doctors, scientists, and aestheticians from around the world.

Human-Centric eClinical Platforms Can Eliminate Inefficiencies Rooted in Traditional Clinical Trials

A recent study shows that about 85 percent of clinical trials of new drugs and medical devices will experience delays, and over 90 percent of them for a month or longer.

The solution to [clinical trial delays] … lies in the adoption of a cloud-based decentralized platform approach. Rather than tether patients to a central, usually urban medical facility, a decentralized clinical trial enables bi-directional communication between study sites and participants.

A recent study shows that about 85 percent of clinical trials of new drugs and medical devices will experience delays, and over 90 percent of them for a month or longer. The cost of these delays to the developer run between approximately $600,000 and $8 million per day.(1) “While this is bad news for the pharmaceutical industry, not to mention patients hoping for new treatments, it is also a wakeup call,” says Dr. Harsha Rajasimha, founder and CEO of Jeeva(TM) Informatics. “Today’s technology and near-universal digital accessibility, offer ways to significantly lessen delays—and possibly costs—in clinical trials.”

In addition to delays, says Dr. Rajasimha, the traditional approach to clinical trial structure and management tends to discourage patient diversity. Chief among the many reasons for this is the fact that trials are held in a limited number of large metropolitan cities, study protocol logistics are suboptimal for the participants concerned with missing work and finding childcare, and because of a lack of information. Patients who participate in these studies skew heavily white—in some cases, 80 to 90 percent.(2) Noting that November is Pancreatic Cancer and Lung Cancer Awareness Month(3), he points out that though people of color have a statistically greater disposition toward pancreatic cancer, they are under-represented in clinical trials for treatments of this life-threatening disease.(4)

This sort of under-representation, says Dr. Sharlene Brown, Jeeva Informatics Director of Business Development, can be traced to the narrow pool from which clinical trial participants tend to be drawn. In one case, she recalls encountering a patient foundation leader charged with explaining to veteran trial operators why a proposed site in Vermont—a state in which white people make up 94.2% of the population(5)—would be less successful at recruiting local participants living with sickle cell disease—a disease that in the U.S. most commonly affects people of color.(6) In a broader sense, adds Dr. Rajasimha, recruitment problems are a leading cause of delays in clinical trials. Earlier this year, approximately 1,000 organizations involved in medical research announced delays or disruption in planned clinical trials, the bulk of them caused either by slow enrollment(7) or complex data handling issues(8).

The solution to these problems, he suggests, and with it the potential for reduced delays and greater study effectiveness, lies in the adoption of a cloud-based decentralized clinical trial platform approach. Rather than tether prospective patients to a central, usually urban medical facility, a decentralized clinical trial enables bi-directional communication between study sites and participants, primarily through the Internet. If empowered by the right bring-your-own-device (BYOD) eClinical platform, anyone with access to a cell phone, or a tablet, or a PC, can use it to interact with the study investigator teams by video, telemedicine, SMS, or email. Among other benefits, says Dr. Rajasimha, this approach enables sponsors to increase both diversity and enrollment by recruiting through social media platforms.

Some of these needed changes in clinical trials, notes Dr. Rajasimha, have been accelerated by the COVID-19 pandemic, which has made many prospective enrollees unable or unwilling to travel to a hospital or other major medical facility, to participate in a new drug study. At the same time, the pandemic has strained the ability of hospitals to deal with swelling crowds of severely ill patients, let alone simultaneously administering a non-COVID19 clinical trial. In response, many trial sponsors have begun adopting a decentralized structure out of necessity.

Dr. Rajasimha hopes that this trend will continue. “Whatever we do,” he says, “we must not go back but forward.” Across the board, decentralizing clinical studies is an important part of the solution for delayed and under-enrolled clinical trials. Medical research is, he notes, in the end about patients—about people. “For the clinical trial process to live up to its own expectations, we must go where the people are, and communicate with them—and they with us—in the most effective way possible.”

About Jeeva Informatics

The personal experience of losing a child born with a rare disease and a brother with a chronic disease became the springboard for Dr. Harsha Rajasimha to apply his years of postdoctoral training at NIH and FDA to accelerate therapies for rare and common conditions. He knew that technology in itself is not the limiting factor and that patient-centered design guided by stakeholder needs and regulatory requirements would guide their continuous learning digital platform. By digitizing and automating manual repetitive tasks and reducing the logistical burdens on patients and study teams by over 70%, Jeeva accelerates the process of bringing new medicines or vaccines to patients who need them by over 3x faster. The Virginia-based company’s modular software-as-a-service platform is fully scalable and facilitates patient enrollment, engagement, and evidence generation in clinical trials on any browser-enabled mobile device. Hushabye Nursery Receives $250,000 Grant from The Bob & Renee Parsons Foundation

Hushabye Nursery logo

The Bob & Renee Parsons Foundation granted $250,000 to Hushabye Nursery in support of its inpatient detox nursery and recovery center for infants experiencing Neonatal Abstinence Syndrome (NAS). Hushabye Nursery provides a therapeutic environment, short-term medical care and social services for infants suffering from NAS, along with outpatient services and resources for families experiencing opioid use disorder (OUD). NAS affects newborn babies who were exposed to opioids in the womb and requires a complex and painful detoxification process in the days following birth. According to the American Academy of Pediatrics (AAP), in the United States, a baby is born every 15 minutes with NAS resulting from prenatal opioid exposure.

“The team at Hushabye Nursery is providing a new level of compassionate care to substance-exposed infants,” said President and Executive Creative Director of PXG Apparel Renee Parsons. “The organization is giving these babies and their families hope, and the tools they need to move forward.”

Hushabye Nursery’s approach is distinctly different than the typical hospital neonatal intensive care unit. Hushabye Nursery provides a comfortable, safe environment – with quiet, private rooms and dim lighting – for babies to be cared for as they go through detox. While hospitals treat NAS babies primarily with medication, Hushabye resorts to drugs only when necessary. Hushabye’s approach has lowered treatment time during this painful period to an average of only six days compared to an average of 16 to 21 days in traditional hospital neonatal ICUs.

“Created by nurse practitioners, Hushabye Nursery is one of only three nationwide medical care models dedicated to treating infants experiencing Neonatal Abstinence Syndrome,” said Hushabye Nursery executive director and founder Tara Sundem. “Thanks to generous donors like Bob and Renee Parsons, we are able to support and comfort babies through the withdrawal process and offer moms and dads who are in recovery a nonjudgmental place to develop a long term plan for treatment and keeping their family safe.”

Hushabye Nursery takes a holistic approach to treating babies born with NAS and supporting their families by offering behavioral health and educational resources. In addition to the nursery, Hushabye offers wraparound services, including both prenatal and postnatal programs. Even after a baby is discharged, the organization helps coordinate long-term care such as pediatric support, counseling and OUD treatment, as well as recovery and prevention programs.

“The harsh reality is the infants being cared for by Hushabye Nursery are born addicted to opioids and suffering the excruciatingly painful process of withdrawal,” said PXG Founder and CEO Bob Parsons. “The loving care Hushabye extends, along with social services and family support, sets a foundation for these children to have some chance for a better future.”

The Covid-19 pandemic exacerbated opioid use disorder and it is estimated that one in eight children ages 17 or younger resided in households with at least one parent who had a substance use disorder (SUD) in the past year. The prevalence of OUD within families underscores the need for Hushabye Nursery’s comprehensive approach to OUD prevention, one that treats the entire family, is community-based and culturally informed and addresses the traumatic experiences that may accompany substance use. This integration of healthcare and social services helps keep children safely with their families instead of being separated during recovery, mitigating the impact of adverse childhood experiences on families. To learn more about the organization or how to get involved, please visit the Hushabye Nursery website at http://www.hushabyenursery.org.

Abdominal Core Health Quality Collaborative Provides Data-Driven Resource for Abdominal wall Reconstruction Coding

The Abdominal Core Health Quality Collaborative (ACHQC) is proud to announce its peer-reviewed coding update for the performance of abdominal wall reconstruction (AWR), “A pragmatic, evidence-based approach to coding for abdominal wall reconstruction” was recently published in the journal Hernia. This publication represents an innovation in coding – utilizing real world data to justify coding updates.

“Applying an evidence-based analysis from the real world data which takes into account patient complexities, operative time and outcomes is a novel approach to substantiate the need for coding updates,” shared Benjamin Poulose, MD, MPH, ACHQC Director of Quality and Outcomes. “Specifically, this update defines abdominal wall reconstruction and clarifies the coding for performance of repairs using advanced myofascial release techniques.”

Recognizing ambiguities within the current coding environment specific to abdominal wall reconstructions, the ACHQC analyzed data from 15,246 elective ventral hernia repairs with mesh collected in the ACHQC registry. Three distinct clinical vignettes and associated outcomes are presented which support the recommendation that coding for AWR cases should be applied based on myofascial releases performed, regardless of surgical approach (open, laparoscopic, or robotic-assisted).

“We believe this type of clinically driven evaluation is crucial when considering the most appropriate coding for abdominal wall reconstruction,” shared Michael Rosen, MD, ACHQC Medical Director. “We hope this work clarifies current AWR coding methodology and can be used as a resource by coding professionals, surgeons, hospitals, and professional societies,”

The complete peer-reviewed article is available free of charge at https://link.springer.com/article/10.1007/s10029-021-02458-w.

Founded in 2013, the Abdominal Core Health Quality Collaborative (ACHQC) is a non-profit collaborative focused on maximizing quality and value of care delivered to patients with hernias and other abdominal core diseases.
