Vytalize Health, a primary care management services organization, recently selected Trella Health, a leading healthcare analytics company, for their care network management solution. Trella’s recently released software solution, Mosaic, enables networks such as Accountable Care Organizations (ACOs) and health systems to develop and manage partnerships to improve patient outcomes and reduce the cost of care. With this partnership, Trella’s Mosaic solution will assist Vytalize Health with optimizing their acute, post-acute, and ambulatory networks across eight states.
