This innovative program was designed to minimize exposure in light of the new risks posed by COVID-19, though the benefits of the program have lasting applications for all home-based services. Constellation continues to provide access to skilled professionals including nurses, therapists, social workers, and home health aides. By integrating technology and support services into its care delivery process, the company is now able to better connect customers to care and the outside world.
Through “virtual visits,” Constellation’s in-home teams are able to supplement visits, access advanced care via video conferencing with nurse practitioners, or connect individuals with family members and loved ones. They also have the ability to integrate wireless technology to monitor vital signs and alert providers to changes in health conditions. The program also coordinates diagnostic and delivery services including pharmacy, medical supplies, and groceries.
Many elderly recipients of home care are already unable to travel to appointments or arrange care. Combined with the increased risks associated with exposure to COVID-19 and corresponding recommendations to stay home, this program offers an improved service offering for home health care and hospice services. “These past couple of months have been a whirlwind of change for everyone.
