Breast Reconstruction Specialist Surgeon Dr. Constance M Chen with Tips on Why Feeling is Lost After Mastectomy and How Sensation Can Be Restored

While pleased with the appearance of her new breasts, some women are disappointed to realize that to her reconstructed breast is numb to touch and sexual arousal.

Autologous reconstruction – using a woman’s own tissue to create a new breast – has become the “gold standard” for breast reconstruction, either immediately following mastectomy or later. “Nipple-sparing mastectomy with autologous tissue breast reconstruction delivers a soft, warm, natural breast that is often barely distinguishable from a woman’s original breast,” says plastic surgeon and breast specialist Dr. Constance M. Chen. “Sometimes, however, the natural look and feel of the reconstructed breast may be more apparent to others than it is to the woman herself.” While pleased with the appearance of her new breasts, some women are disappointed to realize that to her reconstructed breast is numb to touch and sexual arousal.

“As breast reconstruction techniques have been developed, the emphasis until now has been on how the woman would look,” says Dr. Chen. “The next frontier in reconstructive surgery is to reconnect nerves and restore sensation to the breast. New techniques show great promise and many women are now experiencing varying degrees of restored feeling.”

Dr. Chen answers questions about loss and restoration of feeling in reconstructed breasts.

“Why is feeling lost after mastectomy? Can it return?”

During a mastectomy, the nerves that provide feeling to the breast and nipple are severed, causing loss of sensation in the remaining skin. Some women experience a limited return of sensation over time, dependent in part on the type of reconstruction they have. Implants form a physical barrier that prevents nerves from growing through to the skin. Autologous reconstruction, which transfers blood vessels and nerves along with fat and skin, offers the best opportunity for nerve regeneration since nerves are more likely to grow in natural tissue.

Of the four different types of nerve sensation – deep pressure, light touch, pain, and temperature – deep pressure sensation returns most quickly and temperature most slowly. But nerves grow very slowly and even when the regenerated nerves reach the skin after months or years, the quality of sensation is variable.

“How can sensation be restored?”

Because nerve regeneration is slow and results are unpredictable, reconnecting nerves has not always been a surgical priority. But new, meticulous microsurgical techniques that use neural tubes and nerve grafts have enabled surgeons to offer each woman the best possible chance for restoration of feeling.

Two specific methods of autologous reconstruction offer the best opportunity for successfully reconnecting nerves via a procedure called neurrropahy, a microsurgical technique that attaches two nerves. In perforator flap breast reconstruction, the skin, fat, and blood vessels (the “flap”) are transferred from a donor site, often the abdomen or upper inner thighs, to create the new breast. During the flap surgery, a sensory nerve that provides feeling to the lower abdominal skin can be included and transferred with the flap to the chest. Surgeons specially trained in this technique can then connect this nerve to nerves that were severed during the mastectomy. This additional surgical step has shown great promise in restoring feeling better and faster than waiting and hoping for nerves to regenerate.

“How can I ensure that I have the best possible chance to have feeling restored?”

One of the most distressing aspects of loss of feeling is that many women were assured that their reconstructed breast would look and feel natural but weren’t told that they would lose most or all of the feeling in the breast.

The importance of the return of sensation is different for every woman and should be discussed in the initial surgical consultation so she knows what to expect and has the opportunity to ensure that her surgeon is trained in the techniques that will provide the best outcome.

“What constitutes feeling ‘herself’ again after breast cancer is an individual matter that has both physical and emotional aspects,” says Dr. Chen. “Our responsibility to each patient is to ensure that her expectations are realistic and to offer her choices that let her decide what is the best way for her to regain her confidence and preserve her sense of self.”

Profits Run President, Bill Poulos, Reviews Coronavirus Panic Selling Affects on the Stock Market


The market will eventually find a bottom and at that point there will be buying opportunities abound.

Bill Poulos, Founder and President of Profits Run, recently shared his thoughts on the current state of the stock market. Last week was reportedly the worst week for the Dow and S&P 500 since the 2008 financial crisis. Monday, the Dow rallied 5% with the largest percentage gain since 2009. Thursday, all major indexes dipped amid new COVID-19 death reports in the United States. In his statement, Poulos explained that the market is observing panic selling based on coronavirus fears however, the market was due for a correction and will eventually return to normal.

Poulos stated, “What we witnessed last week in stock markets around the world is panic selling. Triggered of course by the coronavirus uncertainty. During periods of panic selling all rationale is overcome by emotion as well as headline driven computer algorithms. The market was already well overdue for a correction and was waiting for the right headline to trigger that correction and right on cue we have the coronavirus scare. Now corrections do not usually occur so abruptly in a matter of five days but this one did and it may not be over. But the market will eventually find a bottom and at that point there will be buying opportunities abound.”

USA Today noted that the fallout over the virus has dropped more than 3,500 points from the Dow Jones Industrial Average in one week alone, its largest decline ever. On Thursday, February 27th, the Dow plummeted nearly 1,200 points, its biggest one-day point drop ever.

Coronavirus is the name of a family of diseases. The current iteration, COVID-19, has not been experienced before and continues to spread around the world. As of today, there are at least 100 confirmed cases in the USA.

Poulos concluded, “The last panic selling of note occurred in December 2018 when the Fed was viewed as raising interest rates too rapidly. By late December there were few buyers to be found. And that marked the bottom of that particular panic selloff… While we don’t know for sure when the current selloff will dissipate when it does there will be opportunities abound once again.”

More Than 1.7 Million Women In Texas Live In Contraceptive Deserts


“It is unacceptable that more than 1.7 million women in Texas face tremendous barriers to accessing contraception they need and deserve in order to decide if, when and under what circumstances to get pregnant and have a child,” said Ginny Ehrlich, CEO, Power to Decide.

Recent data from Power to Decide, show that 1,747,510 women living at or below 250% of the poverty level in Texas live in contraceptive deserts, counties in which there is not reasonable access to a health center offering the full range of contraceptive methods. Currently, across the country more than 19 million U.S. women of low income live in contraceptive deserts.

“It is unacceptable that more than 1.7 million women in Texas face tremendous barriers to accessing contraception they need and deserve in order to decide if, when and under what circumstances to get pregnant and have a child,” said Ginny Ehrlich, CEO, Power to Decide. “Because access to contraception is limited in many parts of Texas, women who are already struggling to make ends meet are now burdened with additional costs to access basic health care. Many of these women are being faced with the untenable decision of deciding whether to use their limited transportation and child-care dollars to go to work or get to a clinic. This is compounded by the fact that women who are faced with long distance travel may also need to take unpaid time off from work just to access basic health care.”

Women in Texas are also far less likely to have health coverage than women in other parts of the country, which limits their ability to get the birth control method that is right for them. In Texas, one quarter (24.3%) of women of reproductive age (13-44) are uninsured, double the national average of 11.7%.

“Women in Texas face a challenging contraceptive access landscape, which was made even worse by the recent federal approval of a waiver severely limiting the providers who can provide family planning services with Medicaid funding. Texas could help rather than hinder women in need through policies that protect and expand access to contraception,” said Ehrlich.

Expanding Medicaid to childless adults would help decrease the percentage of uninsured women, and by extension, give them contraceptive coverage. In addition, allowing pharmacists to prescribe contraception, and requiring insurance to cover an extended supply of prescription contraceptives can make it easier to access some contraceptive methods. Texas can also guard against additional barriers to access by enacting policies that protect insurance coverage of the full range of contraceptive methods. Columbus Pediatric Dental Office to Host Fourth Biannual Community Day


Hines Little Smiles will provide no-charge dental services for local kids on March 7th.

This Saturday, March 7, 2020, Hines Little Smiles will again step up to help ensure the oral health of uninsured or underinsured children from the Columbus, Ohio, area. The pediatric dental practice’s fourth Biannual Community Day is expected to open at 9:00 AM and extend until 3:00 PM. Patients will be accepted in order of arrival, must be accompanied by a parent or legal guardian, and will receive basic dental services, including cleanings, fillings and extractions, free of charge.

Hines Little Smiles hosted their first Biannual Community Day in 2018, in response to rising dental care costs and an insurance system that was unable to provide support and care for children from low-income families. The event was met by enormous response from parents of children in the local community, and has since become a twice-yearly affair.

“Ohio has worked hard to make sure pediatric dental care is a priority,” says Dr. Mitzi Hines, who opened the pediatric dental practice in 2009. “Statewide, we’re meeting and beating our annual targets for reduced dental decay in children and increased access to dental care. However, at a recent board meeting for the KidSMILES Pediatric Dental Clinic I learned that children who need dental care, but cannot afford it, would fill up the Horseshoe (Ohio Stadium) 5 times over. That’s over half a million children in Ohio alone who need care and aren’t receiving it regularly.”

Medicaid does cover some dental care for children, but this care is often insufficient, and reimbursed at levels far below the actual cost of care. “In 2018, we had to make a decision as a practice,” says Dr. Hines. “We could keep trying to fight with Medicaid about coverage for biannual maintenance cleanings and other preventive children’s dentistry, or put that time, energy, and budget into providing a higher level of care that is still completely free for underinsured children in our community.”

Dr. Hines says the turnout at each free, walk-in community event, held twice a year, has been outstanding. “We see a lot of new kids every year, which just hammers home the knowledge that we made the right choice for our community.”

The free dental services provided during the event include standard cleanings, fillings, extractions, and more. (Treatments requiring general anesthesia cannot be provided.) Children will be seen on a first come, first serve basis, and all patients up to age 18 will be accepted regardless of their ability to pay.

“At Hines Little Smiles, our focus is on the children of our community,” says Dr. Hines. “During our last event, we had 38 volunteers on site for 6 hours. In those 228 total volunteer hours, we were able to serve the needs of 47 children and dispensed nearly $25,000 worth of quality dental services. We can’t wait for our fourth no-charge day, and plan to continue this biannual event. It’s a solution that is working for our community’s kids.”
