A Nuvision Credit Union Program to Serve Our Frontline COVID-19 Heroes

In the midst of the Coronavirus crisis, Nuvision Credit Union has never lost sight of their core mission or values. For over 85 years Nuvision has been serving hard-working people who are the backbone of our country; now, they are serving those who are serving us all during our time of need.

Nuvision Credit Union is launching its NuvisionCares initiative, a program that will provide meals from local restaurants to our heroes on the frontlines of the COVID-19 battle. The NuvisionCares initiative is designed to help provide a little bit of normalcy and peace of mind for our frontline medical heroes during one of the most challenging times they have ever faced.

“These people are heroes, who even during normal times work incredibly long stressful hours in jobs where they are rarely recognized for their hard work,” said Roger Ballard, CEO of Nuvision Credit Union. “The NuvisionCares initiative gives our vital caregivers one less thing to think about when they come home to their families.”

Starting April 13th, Nuvision will kick off the program at CHOC Children’s Hospital in Orange County. Doctors, nurses, and other frontline medical professionals from CHOC will be given free meals up to $25 that they can order at five participating restaurants near the hospital. On top of giving our frontline caregivers a little needed relief, the program will provide much-needed support and income to employees at these five local small businesses who will benefit as a result of the NuvisionCares initiative.

The five restaurants that will participate in the NuvisionCares program are Rib Trader, Tutto Fresco, Don Jefe Jalapenos, Cortina’s Italian Eatery, and Amarith Café.

“Nuvision Credit Union really helped us to be able to take care of our employees during their time of need. Because of the NuvisionCares program, we were able to bring our crew back in and give them the ability to earn some vital extra income,” said Mehdi Karr owner of Rib Trader in Orange, California.

This is just one of several community initiatives that Nuvision Credit Union has launched to help the communities they serve through the COVID-19 Pandemic.

Thinking about Breast Cancer and Breast Reconstruction During the Coronavirus Pandemic

Breast cancer patients should take this time to pause, and realize that they do not need to accomplish all of their long-term goals immediately. Specifically, breast reconstruction can take a back burner to oncologic treatment without any lasting compromise to the ultimate aesthetic outcome.

A diagnosis of breast cancer is devastating at any time, but it can seem impossible to sort out your best options in the middle of a global pandemic. When healthy people everywhere are already overwhelmed with survival, what are the options for a woman who has just been presented with a cancer diagnosis?

Fortunately, as much as life has changed during this time of social distancing, the basic issues of cancer treatment remain the same. Patients need to evaluate their disease with the help of their oncologists to decide upon the optimal course of therapy. Women need to strengthen their immune systems via diet, exercise, and mental health. They may want to explore groundbreaking metabolic approaches to cancer treatment. And social distancing may even help protect patients who are trying to heal and recover from the stresses of cancer treatment. Finally, breast cancer patients should take this time to pause, and realize that they do not need to accomplish all of their long-term goals immediately. Specifically, breast reconstruction can take a back burner to oncologic treatment without any lasting compromise to the ultimate aesthetic outcome.

The first step is to realize that even in the time of coronavirus, the ultimate goal of breast cancer treatment should remain to prolong and maintain quality of life. While the treatment pathway may be very different depending on the aggressiveness of your tumor, some basic facts remain the same. Oncologic treatment – surgery, chemotherapy, radiation therapy – take precedence over reconstructive treatment. Furthermore, with careful planning, this does not mean that any later breast reconstruction needs to be inferior due to delays. Oncologic treatments can proceed without compromising the subsequent aesthetic outcome as long as there is cooperation between the oncologic surgeon and the reconstructive surgeon.

What types of outcomes are possible depend on the oncologic treatment that is needed, which again, should always take precedence over the reconstructive treatment. For example, if a patient has a less aggressive small tumor and she is being offered lumpectomy and radiation or mastectomy, she can move forward with a lumpectomy. If her margins are clear, she can elect to have a mastectomy at a later date when she would have otherwise had radiation, or she can proceed with radiation therapy. Radiation therapy does have the disadvantage of changing the skin quality of the breast, so that it becomes more stiff and fibrotic than the healthy breast tissue, and radiation can thus permanently and negatively impact breast reconstruction at a later date.

For patients who decide to undergo mastectomy, breast reconstruction during the time of coronavirus is ill-advised. Delaying breast reconstruction during the time of coronavirus may save your life, and it will not ultimately affect your aesthetic outcome. For some patients, there may not be a choice presented anyway since all elective or non-emergent surgeries, including breast reconstruction, are currently prohibited in some states. Regardless, a life-saving mastectomy that removes your tumor takes precedence over prolonging your surgery and anesthesia time with breast reconstruction, so even hospitals that have canceled and prohibited all elective surgeries should allow an essential mastectomy alone to proceed without breast reconstruction.

The advice to delay breast reconstruction during the time of coronavirus holds true for all types of breast reconstruction. Overall, there are two basic types of breast reconstruction: implant-based breast reconstruction and natural tissue breast reconstruction. Implant-based breast reconstruction involves placing a tissue expander or implant either under or over the pectoralis muscle to recreate a breast mound. The tissue expander or implant itself challenges the immune system, however, as it is a foreign body. Even a direct-to-implant over-the-muscle implant reconstruction has a higher risk of wound healing problems than mastectomy alone, and in the time of coronavirus should be avoided. Placing a tissue expander is arguably less stressful to the body than an implant – especially if it is placed over the muscle – but it still requires frequent visits to the plastic surgeons office to inflate the expander. Natural tissue breast reconstruction is ultimately the least stressful to the body from an immune standpoint, but it is the most stressful in the short term since it requires wound healing from both the donor site (abdomen or legs) as well as the chest area. During the time of coronavirus, patients should be focused on their oncologic treatment, and if that includes mastectomy then they need to recover from their oncologic surgery as quickly as possible.

For patients who want the ultimate aesthetic result after surgery, and who want their reconstructed breasts to look and feel as close as possible to their natural breasts, the gold standard in breast reconstruction is a nipple-sparing mastectomy and natural tissue breast reconstruction. With planning and patience this can be achieved with a delayed breast reconstruction. A woman does not need to compromise on her ultimate breast reconstruction if she and her plastic surgeon can convince the oncologic breast surgeon to perform a nipple-sparing mastectomy no matter how large her breasts. Many breast surgeons may want to place a tissue expander as a placeholder until natural tissue breast reconstruction, and this is certainly a possibility. But for women who do not want a foreign body in place at all, and are understandably concerned that even a temporary tissue expander can affect their immune system, then they can leave their chest flat with floppy excess skin including the nipple-areola complex. It is unattractive during the interim and until the ultimate breast reconstruction, because the breast will look shriveled up and empty (similar to a breast with an unexpanded tissue expander), but once the final reconstruction is performed then the skin envelope will be refilled as if the breast reconstruction was performed at the time of mastectomy.

The key to achieving the ultimate breast reconstruction is the nipple-sparing mastectomy. The breast surgeon needs to make sure that she does not remove any skin during the nipple-sparing mastectomy and that she leaves all the skin intact as if there is going to be an immediate breast reconstruction. If she cannot salvage the nipple for oncologic reasons, she can resect the nipple-areola complex alone and close it with a pursestring closure but otherwise preserve the entire skin envelope. This may be difficult for many breast surgeons to accept, because many breast surgeons will naturally focus on the postoperative appearance of a flat chest with empty excess skin, and may be concerned that the breast skin will look deformed until the delayed breast reconstruction. It is crucial, however, to accept this extremely wrinkled appearance of the breasts in order to have enough skin during the ultimate delayed breast reconstruction. Even in large-breasted women, the breast skin will retract during the intervening time (between mastectomy and reconstruction) and it is always possible to resect excess skin later if necessary. Remember, skin that has been injudiciously resected at the time of mastectomy can never be replaced.

When it comes to how long to delay the ultimate breast reconstruction, there is really no time limit. It is preferable, however, to heal completely from the initial mastectomy, because tissues can be friable and delicate in the immediate postoperative period. To allow the body to fully recover from the first oncologic surgery, a minimum of three months after the initial mastectomy is best. During the time of coronavirus, however, longer periods are perfectly acceptable too. A patient may want to make sure that her margins are clear, and that she does not need radiation therapy. If she does need radiation therapy, then she should delay her breast reconstruction until at least 6 months after radiation is completed. Some patients may want to make sure that they do not have any recurrent disease. Risk of recurrence of the primary tumor is highest in the first 18 months after treatment, so some patients may want to wait until after 1-2 years or even later to go back to the operating room for breast reconstruction. No matter how long the wait, the most important principle is to complete oncologic treatment first and then to allow the body to fully recover from the essential oncologic treatment. This is the best preparation for the ultimate breast reconstruction. While it may be psychologically difficult and even traumatic to look at a flat chest with floppy and empty breast skin during the interim, it does not compromise the ultimate aesthetic outcome of the final breast reconstruction at all.

We are living in a difficult time when some hospitals are overflowing with coronavirus patients, and lack the capacity to take on patients for elective surgeries. Breast reconstruction may be prohibited at this time. Furthermore, even when hospitals begin to open to non-essential surgeries, a woman recovering from breast cancer may not want to be the first person to schedule elective surgery as hospitals slowly open to non-coronavirus patients, especially with potential concerns about later waves of disease. Current studies indicate that 50-70% of people infected by COVID-19 are asymptomatic, so it may not be clear who in the hospital is virus-free. The virus has been found lingering in airborne droplets in the hallways outside hospital patient rooms so masks should be worn continually. Caution is advised during the time of coronavirus. For patients who have been newly diagnosed with breast cancer, priorities should be kept straight. Oncologic treatment is paramount – whether it is oncologic surgery, chemotherapy, and/or radiation therapy. Reconstructive treatment can wait, and it can wait a long time. Planning, patience, and prudence will conserve your body’s precious resources during this time, and it will not compromise your ultimate outcome. If you are willing to accept a temporary and interim period of an unreconstructed body, you can still have a delayed breast reconstruction later in time and achieve optimal success in beauty and health.

Taking a Leadership Role Arcis Golf Is “All In” To Support ClubsHelp

Arcis Golf has pledged the support of its 61 clubs in 13 states, taking a significant leadership role in a new nationwide initiative, ClubsHELP.

Formed less than a week ago during the national health crisis, ClubsHELP connects golf clubs with hospitals in their local areas. Arcis Golf was among the first golf management companies to step up and access its extensive club network and key vendors to provide critically needed support to fight the Covid-19 pandemic.

“First of all, I want to say how grateful I am for the heroic frontline responders who are serving our local communities,” said Blake Walker, Founder and CEO of Arcis Golf, the second largest owner and operator of private, resort and daily fee clubs in the U.S. “Their valuable role during this time of crisis and their attention to human welfare is critical to the wellness of our citizens. We recognize that we are part of a larger family and community. We believe that we can make the greatest positive impact by doing what we do best – serving our communities through our clubs.”

This is how the new initiative works:

-A member or the management of each Arcis club will take on the role of captain.

-The club then ‘adopts’ or is matched with a local hospital.

-Working cooperatively, the most pressing needs at the hospital are identified, then clubs, companies, and individual donors pitch in to gather, collect, and deliver the most necessary items and resources to be used by hospital workers.

At this time, the greatest needs are basic food items that can be consumed quickly, PPE supplies, UV lighting for sterilization of phones and keys, and delivery services. Additionally, utilizing untapped commercial resources, raising money, or mobilizing a local delivery service are examples of ways that clubs are supporting local hospitals.

On an ongoing basis every Arcis property engages with its local community by donating rounds and amenities, holding tournaments and events that raise money for charities.

“During these unprecedented times, and with the health and safety of our members, guests and staff on our minds, we simply felt a calling that we needed to do more. Our club leaders are empowered to work side-by-side with their members and patrons to be part of something really special and to engage within their communities to make a real difference,” said Walker.

In fact, it was based on Arcis Golf’s past working relationship with, and corporate-wide support of, the Els for Autism Foundation that Walker and Chief Marketing Officer Chris Crocker first became aware of the opportunity to utilize their clubs to support this new national program.

“When Ernie Els’ manager called, we said, ‘count us in.’ Every community wants to sustain their local hospital and first responders,” noted Crocker. “We know that strength in numbers—clubs and their members’ networks—will make a meaningful impact to those bravely fighting the virus, with grit and grace, from the frontlines each and every day. Just as Ernie has challenged his fellow golf professionals to step up for this cause, I challenge golf clubs across the nation to embrace and support ClubsHELP.”

Golf’s premier organizations, including the National Club Association, and Golf Writers Association of America, have pledged to get behind the ClubsHELP program and make a significant difference in local communities throughout the United States. Additionally, the American Hospital Association (AHA) and the American College of Emergency Physicians (ACEP) have alerted their membership that golf clubs across the country are activating.

Varicose Veins Range of Severity Highlights Importance of Seeking Professional Advice Regarding Solutions, says West Medical

There’s no reason for individuals struggling with varicose veins to remain in pain and/or embarrassment as there are a variety of treatment options available.

A March 16 article on Allure reports on the different ways to treat varicose veins, which is a condition where veins become enlarged or swollen due to issues with blood circulation. The article discusses common minimally invasive treatments that are available, such as sclerotherapy or laser. West Medical, a healthcare group based in Southern California, says that while varicose veins may only be a cosmetic issue for some individuals – and that’s problem enough – it may also cause physical discomfort for others. With today’s resources and advances in medical technology, West Medical comments that there’s no reason for individuals struggling with varicose veins to remain in pain and/or embarrassment as there are a variety of treatment options available.

West Medical says that the simplest method for varicose veins is through the use of compression stockings, which are intended to improve the circulation of blood flow by adding pressure to the veins. With improved circulation, the sometimes painful condition may improve without any further treatment but the cosmetic benefit may be somewhat variable. The Orange County medical group notes that this home treatment is often useful for individuals with minor cases though, or it can at least serve as a temporary solution that may slow down the progression of the issue before medical treatment is sought. While this is an easy home remedy, the clinic notes that this treatment leaves enlarged veins intact.

West Medical says that a more effective and preferred treatment for varicose vein treatment is by targeting veins via endovenous laser treatment (EVLT). This method involves inserting a small laser fiber into the problem vein with ultrasound and allowing the laser to close the vein through the generated heat. The healthcare group notes that this technique only requires a local anesthetic and can be done in one of its offices. The office notes that the EVLT is very minimally invasive, which means less downtime needed for recovery and quicker healing.

The health group notes that because there are different kinds of varicose veins, each varying in severity, it recommends individuals who are seeking out treatment options consult with a physician for the best solution for their situation. Each individual’s case is unique, and a doctor will be able to determine which varicose vein treatment will be the most effective for each patient’s particular circumstance.

These are unprecedented times for our industry. Operators are facing business challenges beyond their control. Nevertheless, many operators and hotel companies are putting the true meaning of ‘hospitality’ into action. Hundreds of operators with dozens of brands are making their properties and facilities available for use by medical personnel and first responders. Maestro’s team is proud to be associated with many of these hoteliers who are giving back to their communities by opening their doors during this difficult time.

The Silver Hotel Group, a family owned company, is offering a safe and comfortable place to rest for front line medical staff and first responders. “Our properties have specific floors dedicated to front-line workers, with a dedicated elevator where possible, to help mitigate the community spread of Covid-19”, said Shivani Ruparell, VP of Silver Hotel Group.

Accent Inns opened its doors to offer rooms at cost for front-line essential service workers, “We considered closing our doors because our reservations office was only taking cancellations and clearly travel was drying up,” said Trina Notman, VP of Marketing & Communications at Accent Inns. “Then, a nurse who was desperate for a place for her colleagues to self-isolate called us for help because some of her colleagues had taken to sleeping in their cars to avoid risking passing the virus on to their families. We quickly created an at cost room rate for front-line essential service workers, and our PR agency, Beattie Tartan, got the word out that Accent Inns was ready to help.” Accent Inns was surprised when people and organizations in its communities offered to pay for the rooms. Accent Inns then partnered with the United Way Greater Victoria and created the Hotels for Frontline Workers Fund. “A wonderful surprise was that we were flooded with calls from people that actually wanted to pay for a night’s stay for these workers. To date we have sold over 1,000 room nights covered by community donations through this program. It is tremendous to be able to tell these workers that their stay is covered.”

Another example is AHLA’s Hospitality for Hope Initiative that was created to boost collaboration between the hotel industry and local, state and federal governments to help employees, communities, and the industry during this unprecedented health crisis. AHLA recently announced a partnership with its partner state associations to support communities across the country by connecting hotels with the health community to find housing and support for medical staff and first responders. Over 16,000 hotels have signed up for AHLA’s Hospitality for Hope Initiative.

The Center for Health Design has assembled tools and resources to assist healthcare providers with current infection control needs and to inform future project planning. It created a website forum that lists hundreds of properties that are willing to share their resources with hospitals and other care providers. The site lists hotels and others with multi-unit or high-occupancy residential facilities and indicates their capacity and willingness to consider these urgent needs.
