Sentara Home Care Employs Telehealth to Reduce Hospital Readmissions

We believe in the ability of technology to bolster patient care and highly value patient engagement to promote patient independence.

Sentara Home Care Services has achieved a 30-day all-cause readmission rate of 9.09% for their telehealth patients over seven months; 39% lower than the average hospital readmission rate among Medicare patients in Virginia. In December 2018, Sentara Home Care partnered with Health Recovery Solutions, bringing advanced telehealth and remote monitoring to their high-acuity patients.

Patients enrolled in the telehealth program primarily suffer from CHF, COPD, and other chronic conditions. Sentara telehealth patients show an average acuity of 3.62 according to SHP, placing their risk for readmission 25% higher than Sentara’s non-telehealth patients. Sentara’s telehealth program has maintained an average patient satisfaction score of over 90% from January through June of this year.

Sentara attributes the telehealth program’s success to the care continuum set up by the clinical team. The telehealth clinical team leverages the use of a centralized care model and careful clinical documentation to ensure communication across providers and quality care for all patients.

Patients are enrolled in Sentara’s telehealth program following a direct physician referral or home health nurse assessment. Once enrolled, patients receive a 4G tablet, pre-loaded with HRS’ software, and a suite of Bluetooth devices to record their vital signs, including weight, blood pressure, and heart rate. Via the tablet, patients also have access to educational videos and quizzes, daily symptom surveys, and medication reminders. Patient data, from biometric readings to survey and quiz answers, are forwarded in real-time from the patient’s tablet to the clinicians’ dashboard at Sentara’s central office.

Sentara’s centralized care model places a dedicated team of nurses at the core of the care team. The primary responsibility of the nursing team is to monitor the vital signs and health status of patients on the telehealth program. The telehealth nurses collaborate directly with nurses and physicians when an intervention is required to mitigate a patient’s symptoms or prevent an ED visit or hospitalization.

When a patient’s vitals fall out of normal range, the central telehealth team contacts the patient immediately to retest the patient’s vital signs and document any additional symptoms. An intervention is deemed necessary if the patient’s vital sign abnormalities persist, prompting the central telehealth team to contact the patient’s nurse, as well as send a trending report to the patient’s physician.

The tablet and telehealth software provided by HRS enable continuous communication between patient, clinician, and caregiver through voice calling, three-way video conferencing, and text messaging platforms. In recent months, Sentara’s telehealth team has significantly increased the use of text messaging to improve patient engagement, resulting in monthly patient satisfaction scores over 90%.

“We have experienced an increase in patient engagement when patients utilize all features of the system, i.e. those patients reviewing their disease-specific educational sessions tend to be more engaged in their care, said Kim Bradley, Nurse Executive and Director of Clinical Services at Sentara Home Care. “We believe in the ability of technology to bolster patient care and highly value patient engagement to promote patient independence.”

“Sentara’s Telehealth Team is capitalizing on the full capabilities of a telehealth and remote monitoring platform through their detailed clinical documentation, use of text messaging, and implementation of a centralized telehealth model, resulting in fewer hospital readmissions and healthier patients,” added Nolan Santosa, Director of Client Success at HRS.

The Charles E. Holman Morgellons Disease Foundation Announces The Release of Skin Deep: The Battle Over Morgellons

News Image

The documentary Skin Deep: The Battle Over Morgellons has teamed with world-renowned educational distributor Video Project to release the 90 minute film for educational and community screenings. Skin Deep is an investigation by Emmy-winning filmmaker Pi Ware into the fierce controversy surrounding the illness known as Morgellons disease. The documentary is now available for purchase to universities, libraries, medical schools and community groups and comes with a free 20 page study guide.

If you began producing tiny fibers from your body, you would go seek treatment from a doctor, wouldn’t you? Well countless have already experienced this phenomenon, and have found their pleas to physicians fall on deaf ears. Skin Deep is a comprehensive look at Morgellons disease, most well known for afflicting Joni Mitchell. Long thought to be a delusion, new studies and recent discoveries are giving the disease a scientific legitimacy and offering hope to those afflicted. Skin Deep speaks with advocates, skeptics, and sufferers alike to explore this troubling phenomenon, and shines a light on how the medical community reacts to emerging diseases.

Says director Pi Ware, “With many new diseases, there is a period of time before the science of the disease is widely accepted, a time when doctors say, ‘What you’re suggesting goes against what I learned in medical school,’ a time when patients are vulnerable to the prejudice and arrogance of otherwise well-meaning physicians.” Medical history is filled with similar examples: from ulcers (once believed to be caused by stress, now proven to be caused by bacteria) to Multiple Sclerosis (called “hysterical paralysis” until CAT scans and MRIs were invented) to lupus (over 50% of all lupus patients have been told their symptoms are “all in their head”). Ware continues, “There exists a period of time when the victims of a disease are “blamed” for their disease, and the resulting shame, confusion and anger doubles their suffering. Morgellons patients are living in that time right now.”

Skin Deep began filming in the spring of 2015. The film follows several Morgellons patients and physicians who treat them—using antibiotics or anti-psychotics depending on the doctor’s belief system. The film focuses on the battle between a nurse (Cindy Casey-Holman) who is convinced she has a biologically-based disease and a skeptical dermatologist (Dr. Steven Feldman) who favors psychiatric diagnoses. The film climaxes in a showdown at the Morgellons Conference in Austin, Texas, where bridges between doctors and patients will either be built… or burned.

Educational copies of Skin Deep: The Battle Over Morgellons are available at videoproject.com – https://www.videoproject.com/Skin-Deep.html Each purchase includes a free 20 page study guide.

The documentary Skin Deep: The Battle Over Morgellons will also celebrate it’s World Premiere this Saturday, October 5th at 2pm in downtown Los Angeles. Tickets are available at awarenessfestival.org

Clinician Choices Often Differ From Those of Experts When Switching HIV Treatment in Patients With Virologic Suppression

“In many cases where clinicians initially selected an option that differed from the experts, they indicated that they would change their treatment plan after using the tool.” – Edward King, MA, Executive Vice President of CCO.

Healthcare providers show marked differences in regimen choice compared with experts when selecting a switch regimen for patients requiring a change in ART for reasons other than virologic failure. The finding was derived from an analysis of data captured between March 2018 and April 2019 from an online treatment decision support tool developed by Clinical Care Options (CCO). The tool allows users to define a specific patient case scenario based on variables such as current regimen, treatment history, and patient characteristics and comorbidities and then to instantly see which switch regimens are recommended for that exact scenario by a panel of 5 leading HIV experts.

In nearly two thirds of the 1098 cases entered into the online tool, healthcare providers were planning to switch to regimens that differed from those selected by experts based on cardiovascular disease, the presence of mutations associated with HIV drug resistance, or the patient’s HLA-B*5701 status.

“Our data show not only that there is a major discrepancy between switch decisions taken by community clinicians compared with experts, but also that an online decision support tool can help guide clinicians to better choices. In many cases where clinicians initially selected an option that differed from the experts, they indicated that they would change their treatment plan after using the tool,” said Edward King, MA, Executive Vice President of CCO.

The results were presented October 5 at IDWeek 2019 in Washington, DC, and can be reviewed at https://www.clinicaloptions.com/publications/2019/10_2019_hivswitch.

Visikol Expands Image Analysis as a Service Capabilities

News Image

At Visikol we have assisted many pharma and biotech companies with making sense of large multi-dimensional data sets and extracting key quantitative metrics

Imaging data sets are generated through drug discovery research projects using a wide-range of imaging modalities such as optical projection tomography, optical coherence tomography, echocardiogram, light sheet microscopy, MRI and X-ray CT. While many research groups have the capability to generate these data sets inhouse, a lot of research groups struggle with transforming these large and sometimes noisy data sets into actionable insights that allow them to make more informed decisions during the drug discovery process.

“At Visikol, we have assisted many pharma and biotech companies with making sense of large multi-dimensional data sets and extracting key quantitative metrics. We find that most off-the-shelf image analysis platforms are limited in their flexibility and a lot of research groups reach out to us when they need help with building customized image analysis solutions,” described Visikol Chief Science Officer Dr. Tom Villani.

To start an image analysis project with Visikol, Clients discuss with the Visikol project team the types of end-points they are interested in and the details of their specific research question. Clients will then send along a hard drive of data or upload files to Visikol’s secure cloud-based file sharing platform. From this data, Visikol can apply its 3Screen™ image analysis tools which are a suite of digital pathology algorithms designed for use with both two-dimensional and three-dimensional image data sets. These tools are ultimately used to transform the data sets provided by Clients into actionable insights that allow Clients to make more informed decisions during the drug discovery processes.

“Over the last few years, we have worked with Clients on projects ranging from analyzing 40-plex multiplex slide data sets and processing thousands of in vivo optical coherence retina imaging data sets to identifying novel image-based biomarkers such as in our recent work with Enzyvant Therapeutics,” described Visikol Director of Imaging Services Dr. Graeme Gardner. While there are many digital pathology companies in the marketplace today offering services as well as off-the-shelf digital pathology solutions, Visikol has become a leader in this space through its expertise with multi-dimensional datasets, on-demand custom software development and image analysis software validation and clinical deployment.

About Visikol

Visikol is a CRO focused on advanced drug discovery that is leading the fields of bio-imaging, bioinformatics and image analysis. We conduct end-to-end drug discovery services that include both 2D and 3D in vitro models and assays, 3D whole mount tissue imaging, digital pathology and custom drug discovery projects. Visikol offers a portfolio of drug discovery services ranging from 2D and 3D cell culture model and assay development to in vitro screening, animal tissue histology and automated image processing. The focus of these services is to transform tissues into images and ultimately into quantitative data sets that can be mined for actionable insights that help our Clients make more informed decisions during the drug discovery process. Additionally, Visikol manufactures and sells a suite of tissue clearing reagents and 3D immuno-labeling kits. These products allow researchers to easily and rapidly image whole tissues and 3D cell culture models in 3D instead of traditional 2D sectioning.
