Concerto HealthAI Research Demonstrates that its Patient Reported Outcomes System Effectively Bridges the Communication Gap between Patients and their Clinical Team

Concerto HealthAI, the leading Real-World Data and AI Enterprise Technology company for precision oncology, announced today that it will present research findings which show how utilizing an electronic Patient Reported Outcomes (ePRO) system can be a highly effective and superior way for oncology practices to identify unmet needs of patients and link them to resources for developing Advance Directives (ADs), which are a guide to patients’ wishes for medical decisions during end-of-life care. The data will be presented during a poster presentation at the American Society of Clinical Oncology (ASCO) 2019 Supportive Care in Oncology Symposium being held October 25-26 in San Francisco, California.

ASCO recommends that healthcare providers encourage early advance care planning with their patients. However, many cancer patients do not have advance directives, such as a living will or a durable power of attorney. A potential reason for low rates is inadequate communication between the provider and patient. The value of having advance directives is to allow patients to make decisions about their health and quality of life known in the event they are unable to express them.

Researchers used the Patient Care Monitor™ (PCM), Concerto HealthAI’s proprietary, web-based ePRO system that links PRO data to electronic medical record data in real-time, to survey patients as part of routine clinical care at an initial visit to a community-based oncology practice.

“Utilizing patient reported outcomes has become a practical tool to assess the impact of treatment on patient quality of life and understand the factors that can improve communication between providers and patients,” said Joanne Buzaglo, PhD and Executive Director of PRO Solutions at Concerto HealthAI. “This study demonstrates that using an ePRO system can be a non-threatening way to identify potential questions patients have and provide them with useful information about developing advance directives.”

Overall, of the 4,573 patients that completed the PCM survey including the advance directives module, one quarter (26%) reported having an advance directive at the time of the initial visit to the oncology clinic, specifically 10% indicated having a Living Will and 6% a durable power of attorney. The remaining three-fourths (75%) indicated either not having an advance directive (64%) or not knowing if they had an advance directive (10%). Patients with advance directives were more likely to be older (M=68 v. 56 years) and White (79% v. 21% Black). Of the patients without advance directives, 11% requested more information on them and were more likely to be older (M-59 v. 55 years), female (66% v. 34%), Black (55% v. 45% White) and non-married/non-partnered (55% v. 45%).

“The majority of patients do not have ADs at the time of an initial visit to a community-based oncology practice,” said Jeff Elton, CEO of Concerto HealthAI. “This systematic and advanced approach to capturing data can address the gap in provider-patient communication in order to create the best possible care and end-of-life plan together.”

Bieber’s Toothy Garter Removal is as Risky as it is Risqué, says A-Z Dental Care

While the intimate moment might have made for a memorable photo — using teeth for any other purpose than eating risks damaging them.

An October 10 article on People reports on the mildly risqué Instagram post by Justin Bieber in which he is seen removing his new bride’s garter with his teeth. The post confirmed the pop star’s recent marriage to Hailey Baldwin, destroying the dreams of millions of ardent “Beliebers” around the world. West Covina based dental office A-Z Dental Care, however, says that – while the intimate moment might have made for a memorable photo — using teeth for any other purpose than eating risks damaging them, even if the situation seems harmless.

The office explains that human teeth are specially designed to break down specific types of food. A-Z Dental Care says that molars, for example, are largely meant for grinding vegetables or roots, not biting into fabrics or other objects. When a person uses their teeth on an object, their risk for chipping or breaking a tooth gets exponentially higher, the office notes. Common habits such as biting one’s nails are a frequent cause of chipped teeth, the office adds. A-Z Dental Care says that while it is unlikely that a single event will actually destroy a person’s teeth, getting out of the habit of using teeth instead of scissors or a knife will limit the risk of chipping or breaking substantially.

Of course, A-Z Dental Care notes that even the most oral-health conscious person can still experience chipping or breaking, and when it happens its time to visit the dentist. The dental office says that chipped teeth have the potential to break further so it’s important to schedule an appointment with your dentist right away. If a chipped tooth ends up exposing the pulp of the tooth in even a minute amount, the patient’s risk for infection and costly restorative procedures skyrockets. Taking care of a chipped tooth sooner rather than later is always in a patient’s best interests, the office notes.

If a patient experiences a broken or missing tooth, however, the matter is much more urgent, says A-Z Dental Care. Broken teeth have a high risk of causing infection as food particles can get stuck inside of hard-to-reach crevasses, allowing infection-causing bacteria to flourish. The office also notes that missing teeth can lead to bone loss if unchecked for long periods of time, as there is nothing to stimulate bone tissue production in the gap. Getting dental implant as soon as possible can reduce the risk of bone loss and infection considerably while also restoring lost confidence in a patient’s smile and full functionality.

Hand and Upper Limb Orthopedic Surgeon Alejandro Badia Offers Tips to Avoid Cubital Tunnel Syndrome

As a nerve-compression disorder of the upper limbs, cubital tunnel syndrome is second only to carpal tunnel syndrome in frequency of occurrence.

Sometimes, the elbow’s “funny bone” is just not that funny, especially when the nerve passing in it – the ulnar nerve — becomes irritated and inflamed due to chronic pressure and impingement – a condition called cubital tunnel syndrome, says noted hand and upper limb orthopedic surgeon Alejandro Badia, MD. He emphasizes that early diagnosis is essential.

In fact, experts contend recovery rates are nearly 90 percent when the condition is treated within a year of onset but drop to under 70 percent if a patient does not get help beyond a year. As a nerve-compression disorder of the upper limbs, cubital tunnel syndrome is second only to carpal tunnel syndrome in frequency of occurrence.

“The ulnar nerve lies directly behind the medial epicondyle – the elbow’s bony protrusion or “funny bone” — and is vulnerable to pressure and constriction. When the pressure on the nerve becomes great enough to disturb the way the nerve works, then numbness, tingling, and pain may develop along the inner portion of the elbow and in the forearm, hand, and fingers, specifically the ‘pinky’ and ring fingers,” explains Dr. Badia. He is founder and chief medical officer of the Florida-based Badia Hand to Shoulder Center and OrthoNOW®.

In severe cases, the condition can lead to weakness in the fingers, decreased hand grip and wasting of hand muscles.

“Unless properly diagnosed and treated, nerve compression disorders worsen over time and can cause nerve damage that becomes permanent or, in the best of circumstances, requires months of healing. When ignored, the hand loses fine dexterity and task such as typing, playing the piano or say, performing surgery, become near impossible,” Dr. Badia says.

The cubital tunnel is anatomically similar although less defined than the carpal tunnel in the wrist. It is a narrow passageway, composed of ligament, muscle and bone, behind the inside edge of the elbow. The ulnar nerve runs through the cubital tunnel on its way down the forearm and into the hand where the nerve controls the thumb and small hand muscles.

Symptoms of nerve compression within the cubital tunnel most frequently occur when the elbow is rested on hard surfaces for extended periods; is exposed to repetitive bending and straightening, particularly during work-related activities like painting; or kept in a bent position, as happens while driving, sleeping or holding a phone.

Other risks for developing the condition are injury to the elbow; presence of joint-inflammatory disorders like arthritis; obesity; and engagement in sports, particularly those involving throwing and overhead movements, says Dr. Badia.

The anatomy of the upper limb also may predispose some people to developing cubital tunnel syndrome. In those cases, the nerve may glide over the elbow’s bony bump as the elbow is bent and straightened. Eventually, the ongoing motion causes the nerve and connective tissue around it to become tender.

Conservative therapy is normally the first-line treatment for the syndrome, including application of cold compresses, over-the-counter anti-inflammatory medications, corticosteroid injections, prescribed exercises, changes in pattern of elbow use by avoiding activities that pressure the joint, wearing of a protective elbow pad — even splinting of the elbow at night to prevent over-bending it during sleep. However, should these approaches fail, surgical intervention may be required, Dr. Badia notes.

Dr. Badia is a recognized expert in endoscopic tunnel release – an innovative surgical procedure. The technique involves insertion of an endoscope through a tiny incision in the elbow, enabling a surgeon to visualize the operating field and relieve tension on the ulnar nerve minimally invasively.

“Endoscopic tunnel release is quicker and less painful than standard surgical approaches and typically requires minimal recovery time, and I am able to get folks back to most jobs within a week. In fact, I have had several throwing athletes return to the mound within 3 weeks due to the minimally invasive endoscopic approach,” Dr. Badia states. “Patients have experienced immediate relief of symptoms following the procedure. I have had several patients that noted relief in the recovery room when local anesthesia wore off!”

But, even newer treatments for cubital tunnel syndrome may be on the horizon. Two case reports published in the May 2019 issue of the journal Medicine indicate that pulsed radio frequency (PRF) stimulation applied to the ulna nerve at the medial epicondyle relieved the neuropathic pain of patients who had failed earlier therapy involving oral medication and steroid injections, however this will never replace the mechanical effect of actual nerve decompression in cases where the nerve has clear signs of compression

The best medicine, of course, is prevention, which is why Dr, Badia offers these tips to lower risk for cubital tunnel syndrome:

  • Avoid resting the elbows hard surfaces for long periods of time.
  • Warm up properly before engaging in strenuous exercise, physical work or sports.
  • Take frequent breaks when performing activities requiring constant arm and elbow motion and avoid prolonged periods where the elbow is fully bent (flexed).
  • Lose weight.
  • Most importantly, if that “funny bone” is no laughing matter, contact an orthopedic specialist as soon as possible, Dr. Badia concludes.

American Association of Gynecologic Laparoscopists (AAGL) announces the 48th Annual Global Congress on Minimally Invasive Gynecologic Surgery (MIGS), November 9-13, 2019 at the Vancouver Convention Centre – West (1055 Canada Pl, Vancouver, BC V6C 0C3, Canada) in stunning Vancouver, B.C., Canada. AAGL’s congress is the premier scientific conference for gynecologic surgeons from around the world, offering the latest education and best practices in MIGS. This year the congress is expected to welcome over 2,000 physicians, both domestic and international, for the collegial 5-day gathering. Additionally, 65 companies will be exhibiting, and 1,000 industry representatives and executives will be in attendance, as the AAGL global congress is the ideal conference for medical device, technology, innovation, and product launches.

Additional non-CME presentations will be offered in 28 Open Communication Sessions, and more than 400 Virtual Poster Sessions.
