Delaware Valley Accountable Care Wins 2019 Doyle Award for Innovative Post-Acute Care Strategy

MCG Health presents Delaware Valley ACO with the 2019 Richard L. Doyle Award

Having MCG’s care guidelines as evidence-based tools to help guide our care coordination efforts with our post-acute care partners are some of the important components that help drive our success.

MCG Health, part of the Hearst Health network and a leading provider of informed care strategies, has named Delaware Valley Accountable Care Organization (DVACO) a recipient of the 2019 Richard L. Doyle Award for Innovation and Leadership in Healthcare.

The Philadelphia-area ACO was recognized for implementing a unique post-acute strategy which significantly reduced patient length of stay in skilled nursing facilities (SNFs) and improved care coordination across the post-acute continuum of care. By leveraging MCG’s Chronic Care and Recovery Facility Care guidelines and a web-based tool to track their patients’ stays in post-acute care, Delaware Valley ACO reduced the average length of stay (LOS) by one full day and achieved a $1.3 million savings in Medicare expenditures. The ACO also decreased home health spend by 8% over the same timeframe. Since the ACO’s inception in 2014, they have been able to reduce overall SNF spend in their network by an impressive 30%.

Delaware Valley ACO’s strategic initiative used claims data to define the top 20 diagnoses for patients utilizing a SNF. The ACO then partnered with MCG to publish length of stay guidelines for these diagnoses and share them across their SNF network. This critical step encouraged proactive multi-disciplinary care planning at the time of admission. In turn, this helped to identify barriers to discharge earlier in the patient’s stay and initiate the proper actions to resolve them. Delaware Valley ACO’s SNF partners now proactively communicate with a patient’s primary care physician (and/or ambulatory care coordinator) to discuss clinical, social, medication, or palliative care goals.

“We learned pretty quickly that establishing preferred post-acute care networks was the easy part of our strategy; leveraging them to improve the care of our patients and improve value was more challenging,” said Beth Souder, PT, MSPT, the Director of Post-Acute Networks at Delaware Valley ACO. She added, “Having MCG’s care guidelines as evidence-based tools to help guide our care coordination efforts with our post-acute care partners are some of the important components that help drive our success.”

About the Richard L. Doyle Award for Innovation and Leadership in Healthcare

The Doyle Award was developed to recognize organizations that make innovative use of the care guidelines to help deliver effective healthcare. Dr. Doyle, the care guidelines founding editor, was a hospital chief-of-staff when he began creating clinical guidelines in the 1980s to help improve healthcare efficiency and quality at Mercy Hospital in San Diego. He later joined Milliman & Robertson and in 1988 published the first set of what was to become the care guidelines.

Judges for this award are independent healthcare quality experts, not currently associated with MCG. Applicants were judged on how well their projects supported the MCG mission to help drive effective care, and the names and locations of the nominees were redacted prior to the judges’ review. Judges looked for evidence of improvements in healthcare quality and patient safety; patient/member satisfaction; staff efficiency, productivity and satisfaction; internal/external communication; and effective use of resources.

Cosmetic plastic surgery experts will discuss their particular specialties so that viewers can develop a better understanding of this industry.

Emmy winning and Academy Award nominated actor, Lawrence Fishburne, will host a “Behind the Scenes” episode on cosmetic plastic surgery. The educational program presents information on a variety of topics that impact people around the work. It uses a documentary style format to engage viewers on important issues and events of interest.

Cosmetic plastic surgery experts will discuss their particular specialties so that viewers can develop a better understanding of this industry. Cosmetic surgery is defined as any voluntary program that is performed for the sole purpose of improving someone’s appearance and reducing the appearance of aging. The American Society of Plastic Surgeons reported that in 2018 there were 1.8 million cosmetic surgeries performed.

An additional 15.9 million noninvasive cosmetic procedures were also performed. These include injectables, such as Botox and Dysport, which have grown in popularity increasing 21.5% since 2014. While women still remain the primary gender requesting cosmetic procedures, there is a growing trend of men having cosmetic work done.

The most prevalent procedures are implants and injectables such as Botox and facelifts. Other procedures including liposuction, nose surgery, and eyelid surgery are also popular. Experts will discuss these procedures and the implications of preserving youthfulness and enhancing beauty. Surgical risks and potential complications will also be covered, and how expertise is required in the field to avoid such issues.

As a quality educational series, every “Behind the Scenes” episode strives to educate on a wide variety of topics and issues. To engage viewers, programs share information through a series of inspiring stories for an in-depth look at a topic from experts.

Highmark’s Outcomes-Based Contract for SYMBICORT® Improves Patient Outcomes for Asthma, COPD

Highmark today announced that its outcomes-based contract with AstraZeneca for the respiratory medicine SYMBICORT® demonstrated positive results in improving patient outcomes for the treatment of asthma and chronic obstructive pulmonary disease (COPD).

Under outcomes-based contracts, drug companies are accountable for the clinical effectiveness of medications and the per-member, per-month medical and pharmacy costs for members who use the drug compared to the costs for members who use other drugs to treat their health condition. The goal is to ensure that Highmark members have access to the most clinically effective, cost-efficient medications possible.

Highmark initially announced its outcomes-based contract with AstraZeneca for SYMBICORT in April of 2018. SYMBICORT, approved for the treatment of asthma and COPD, helps decrease inflammation and open up the airways in patients’ lungs. As part of the contract, Highmark reviewed specific pharmacy and medical criteria to determine if asthma or COPD-related symptoms for Highmark patients on SYMBICORT were in-line with the clinical trial results provided by AstraZeneca.

Highmark found that the number of patients taking SYMBICORT who saw a worsening of symptoms requiring them to visit a health care provider or hospital were substantially lower than expected. Among Highmark members on SYMBICORT with a primary diagnosis of asthma or COPD, more than half experienced a stabilization or improvement of their symptoms.

Highmark and AstraZeneca’s contract for SYMBICORT was one of the first outcomes-based agreements involving asthma and COPD. The contract applied to Highmark’s commercial members in its national health insurance markets and its core health insurance markets in Pennsylvania, West Virginia and Delaware. Slightly more than 30,000 SYMBICORT prescriptions were filled for Highmark commercial members in 2018.

Highmark has also forged outcomes-based contracts for the treatment of several other high-cost, chronic health conditions, including diabetes and inflammatory bowel disease.

“Outcomes-based agreements like this show the value of medicines like SYMBICORT to both patients and the health system in reducing exacerbations, hospitalizations and inpatient visits,” says Sarah Marchè, senior VP of pharmacy services for Highmark. “As Highmark furthers its leadership in value-based strategies, results like these are proof that a shared commitment to identifying the right medicine and approach in a priority therapy area can improve patient outcomes.”

State of Washington Wins NASCIO Award for Track-Kit Sexual Assault Kit Tracking System

The State of Washington received a NASCIO award for implementing STACS DNA’s Track-Kit to track the location and status of every sexual assault kit across the state.

For its implementation of the Track-Kit Sexual Assault Kit System from STACS DNA, the State of Washington received a 2019 State IT Recognition Award from the National Association of State Chief Information Officers (NASCIO). Washington State Patrol (WSP), STACS DNA’s client, won the State CIO Office Special Recognition Award from over 130 submissions.

As described in WSP’s submission, “Prior to this legislation, survivors had no way of knowing the location or status of their kit. The Tracking System provides start-to-finish SAK inventory tracking to deliver accountability, transparency and information sharing among all stakeholders, and most importantly provide the survivor with peace of mind. Through the extended use of this system, it will create more efficiency and accountability in SAK evidence workflows, thereby preventing the accumulation of un-submitted kits.”

STACS DNA played a significant role in developing and delivering statewide training, as described in the proposal. Live web-based training proves cost-effective, timely, and reduces the number of trainers needed. The company also provides online training resources, assistance and curriculum to end users, which are continuously updated using lessons learned during deployment. FAQs, self-paced videos and full user documentation are accessible within Track-Kit. The proposal also outlines the legislation, stakeholders, implementation process and impact of the system.

WSP has become a national pioneer in regards to its SAK Tracking System Program as many states have used Washington’s model for their own SAK tracking programs. Globally, the program has received worldwide recognition following presentations at several international meetings.
