U.S. Employees Report High Levels of Family Caregiving Challenges during Covid-19, Torchlight Client Data Review Finds

“COVID-19 has put new stresses on millions of already-overwhelmed Americans who are struggling to balance their careers and jobs with the day to day demands of care. We’re encouraged to see new data and research on how COVID-19 impacts family caregivers.”

Torchlight, a leading provider of employee-caregiver support solutions, today released its client data review of the most pressing family and caregiving concerns of U.S. employees since the onset of COVID-19. The study analyzed the responses of a sampling of Torchlight’s users from March 15, 2020 through May 31, 2020 and found that American families are reporting a steep increase in depression, eating disorders, and legal, financial and estate planning concerns of their elderly family members. The study also revealed a nearly 100-fold increase in elder care scams, and a sharp uptick in mental health and ADHD diagnoses of their children.

The time period of the data capture coincides with the early stay-at-home orders in March through the re-opening of some businesses that occurred by the end of May. The findings were compared against the same employee data during the prior period from January 1, 2020 to March 14, 2020. Currently, more than 1.8 million employees across all industries in the United States have access to Torchlight through their employer-sponsored benefit program.

Shortly before the pandemic hit, the National Alliance for Caregiving (NAC) and AARP released their Caregiving in the US 2020 report which affirmed that more than 20% of all Americans are family caregivers and 23% of caregivers cited worsening health due to caregiving responsibilities. Prior to the pandemic, a growing number of Americans were already challenged with family caregiving responsibilities and their own deteriorating health.

Key findings from Torchlight’s client data since the onset of COVID-19:

● 13.1% increase in anxiety and depression diagnoses across elder care recipients

● 25.6% increase in eating and nutrition concerns among elder caregivers

● 95.5% increase in elder scam concerns among caregivers

● 33.6% increase in concerns about creating/updating legal documents for elders

● 25.6% increase in financial and estate planning concerns among elder caregivers

● 17.9% increase in child mental health diagnoses

● 12.6% increase in ADHD diagnoses

● 16.1% increase in employee concerns about their children’s screen time (including social media use)

The company has also released its Caregiving in the Next Normal Toolkit which is designed for organizations, businesses, individuals, parents, and caregivers in need of assistance and information during the Coronavirus/COVID-19 pandemic. ATD Recognizes Memorial Health System for Innovation in Talent Development

During a virtual awards recognition celebration, the Association for Talent Development (ATD) recognized organizations and individuals for their excellence and contributions across the talent development field.

This year’s winner of the Innovation in Talent Development Award is Memorial Health System.

Use of Pembrolizumab Provided Long-Term Benefits in Patients With Metastatic Melanoma, 10-Year Look Shows

In a study published in JAMA Oncology, Dr. Igor Puzanov took a 10-year look at pembrolizumab use in patients with advanced melanoma

“The study reinforces the fact that immunotherapy can significantly extend the life of patients with melanoma and even lead to cures,” said Dr. Puzanov.

A new 10-year analysis led by Igor Puzanov, MD, MSci, FACP, Director of Early Phase Clinical Trials and Chief of Melanoma at Roswell Park Comprehensive Cancer Center and published Thursday in the journal JAMA Oncology provides new insights into an important question: whether BRAF V600E/K mutation status or previous BRAF inhibitor (BRAFi) therapy with or without a MEK inhibitor (MEKi) affects response to pembrolizumab (brand name Keytruda) in patients with advanced melanoma.

This retrospective analysis of three randomized clinical trials (KEYNOTE-001, KEYNOTE-002 and KEYNOTE-006) involved 1,558 patients with advanced melanoma and known BRAF tumor status (BRAF wild-type or BRAF V600E/K-mutant melanoma) who had all been treated with pembrolizumab, some of whom had undergone prior treatment with BRAF inhibitors with or without MEK inhibitors.

Patients with BRAF wild-type and BRAF V600E/K-mutant melanoma had objective response rates (ORRs) of 39.8% and 34.3%, respectively, and similar respective rates of both 4-year progression-free survival (PFS; 22.9% and 19.8%) and overall survival (OS; 37.5% and 35.1%).

Patients with BRAF V600E/K-mutant melanoma who received previous BRAFi with or without MEKi had baseline characteristics with worse prognosis — lower ORR (28.4% vs 44.2%), 4-year PFS (15.2% vs 27.8%), and OS (26.9% vs 49.3%) — compared to those who had not received earlier targeted therapy.

The results of this study support the use of pembrolizumab for the treatment of advanced melanoma regardless of BRAF V600E/K mutation status or prior BRAF inhibitor, with or without MEK inhibitor therapy.

“Our findings confirmed the long-term, lasting benefits of pembrolizumab for patients with unresectable advanced melanoma and show that the effect is seen regardless of BRAF mutation status — and regardless of earlier treatment with a BRAF-targeting therapy,” says Dr. Puzanov, who is first author on the new publication. “Coupled with what we know from separate studies about nivolumab, we see a clear picture of the benefits immune checkpoint inhibitors have had for thousands of people with advanced melanoma over the last decade.”

Some 40% of patients with metastatic melanoma have BRAF mutations, meaning a particular gene and protein in their tumor cells is modified. Of that group, 90% have an activating BRAF mutation.

“Our long-term view provides evidence to support giving immunotherapy early in a patient’s treatment, before turning to targeted therapies. This course of treatment is now the standard of care, but this is important affirmation for this approach, which was not standard at the time these patients were treated,” Dr. Puzanov notes.

These study findings may help explain the striking decline in deaths from melanoma reported by the American Cancer Society (ACS) a few months ago. A January 2020 report from the ACS documented a 7%-per-year decline in the overall melanoma death rate between 2013 and 2017 in people ages 20 to 64.

“The study reinforces the fact that immunotherapy can significantly extend the life of patients with melanoma and even lead to cures,” Dr. Puzanov says. “But importantly, we also still see a role for targeted therapies. Having both of these treatment options at our disposal has helped to drive incredible progress against in a cancer type that was almost universally fatal a decade ago.”

The authors note that ongoing randomized controlled trials should support clearer and more specific conclusions about the optimal sequencing of these treatments.

Mount Sinai Researchers Identify Distinct Subtypes of Polycystic Ovary Syndrome With Novel Genetic Associations


Mount Sinai researchers have for the first time identified reproductive and metabolic subtypes of polycystic ovary syndrome (PCOS) that are associated with novel gene regions. The findings, which used clustering of clinical, metabolic, and hormonal data from women with the syndrome, promise to transform understanding of the cause of PCOS and, accordingly, will have far-reaching implications for its diagnosis and treatment. The results were published in PLOS Medicine on Tuesday, June 23.

PCOS is among the most common endocrine conditions in reproductive-age women, affecting up to 15 percent of this population worldwide. It is a leading cause of infertility and type 2 diabetes in young women. The cause of PCOS is unknown, but there is a strong inherited susceptibility to the disorder. It is currently diagnosed using different sets of clinical criteria based on expert opinion. There is considerable controversy about which criteria are the most appropriate.

“We’re starting to make headway on what causes PCOS. It’s very frustrating for patients because it’s poorly understood and patients often see several physicians before PCOS is diagnosed,” said senior author Andrea Dunaif, MD, Chief of the Hilda and J. Lester Gabrilove Division of Endocrinology, Diabetes and Bone Disease at the Mount Sinai Health System. “Through genetics, we’re beginning to understand the condition and may have specific targeted therapies in the not-too-distant future.”

Using clinical, biochemical, and genotype data from their previously published PCOS genome-wide association study (GWAS), Dr. Dunaif and her team identified the subtypes using cluster analysis, a way of aggregating similar data in an unbiased manner. Unsupervised cluster analysis, meaning there are no predetermined models of data, was performed in the GWAS cohort of 893 PCOS cases. The clusters were then replicated in an independent group of 263 PCOS cases.

The clustering revealed two distinct PCOS subtypes: a “reproductive” group characterized by higher luteinizing hormone (LH), a hormone that triggers ovulation and acts on the ovaries, and higher levels of sex hormone binding globulin (SHBG), a protein that regulates the ability of testosterone to enter its target tissues, with relatively low body mass index (BMI) and insulin levels; and a “metabolic” group characterized by higher BMI, glucose, and insulin levels with lower SHBG and LH levels.

This current paper builds upon a study that Dr. Dunaif and her team published in The Journal of Endocrinology and Metabolism in April 2019 where they found, using family-based genetic analysis, that rare genetic variants in a gene involved in male hormone production, DENND1A, play major roles in the development of PCOS. This genetic variation could serve as a marker for early PCOS detection.

In the study published today, the researchers developed a predictive model and classified the reproductive and metabolic subtypes in a family-based cohort of 73 women with PCOS, including their sisters. They found that the subtypes tended to cluster in families. They also found that carriers of the previously reported rare variants in DENND1A were more likely to have the reproductive subtype of PCOS. The results suggest that these subtypes are biologically relevant since they have distinct genetic architectures. The findings will substantially assist in gene discovery by allowing the selection of more homogeneous PCOS subtypes, which could increase the likelihood of identifying disease-associated genes.

“In contrast to classifying disorders based on expert opinion, this is a very powerful objective approach to categorizing syndromes like PCOS into distinct subtypes with different causes, treatment and clinical outcomes,” said Dr. Dunaif.

The study was supported by grants from the National Institutes of Health.

Precision ADM Inc. (Precision ADM or “the Company,”) is pleased to announce the appointment of Heather Smart, MSc., P. Eng. as Director of Engineering effective July 6, 2020.

Ms. Smart is a Professional Engineer with over 10 years of Aerospace engineering experience with expertise in the repair and overhaul of gas turbine engines. She earned a Master’s degree in Mechanical Engineering in 2017, specializing in behaviour of aerospace metal alloys. Her most recent professional role involved facilitating procurement and installation of major capital equipment within the Centre for Aerospace Training and Technology, a collaborative industrial campus between Red River College and StandardAero in Winnipeg, Manitoba. As well, she was instrumental in the successful setup of a new Advanced Composite Manufacturing Centre that is remotely linked to Red River College’s Smart Factory in demonstrating the Internet of Things. Ms. Smart is an active member of Engineers & Geoscientists of Manitoba, and sits on the Advisory Board of Red River College’s Technology Access Centre for Aerospace and Manufacturing.
