WEDI Shares Results of Prior Authorization Survey in Testimony to NCVHS

WEDI, the nation’s leading nonprofit authority on the use of health IT to create efficiencies in health care information exchange and a trusted advisor to the U.S. Department of Health and Human Services (HHS), presented prior authorization (PA) survey results during testimony late last year before the HHS National Committee on Vital and Health Statistics (NCVHS). In the meeting, Jay Eisenstock, Chair of the WEDI Board of Directors, shared how the results highlighted technology limitations, increased PA requirements and the desire for mandated standards.

The survey, conducted by the WEDI Prior Authorization Subworkgroup in August 2019, analyzed the current medical services’ prior authorization process and will be used to develop guiding principles to support increased automation and standardization. The survey was distributed by WEDI to member and non-member organizations and received 127 total responses including 65 from providers, 31 from payers and 31 from vendors. In fact, our colleagues at ONC continue to drive this conversation with the topic being a key point of discussion during last week’s Health Information Technology Advisory Committee (HITAC) meeting.”

Among key findings of the survey:

Trends in PA volume

  • 84% of providers reported the number of medical services that require PA has increased
  • 55% of vendors reported the number of medical services that require PA has increased
  • 42% of payers reported the number of medical services that require PA has increased

Determining whether a PA is required without initiating a PA request is difficult

  • 62% of providers reported they DO NOT have a technology to evaluate whether a PA is required without initiating a PA request
  • Only 45% of vendors reported having technology to evaluate whether PA is required without initiating a PA request

Results of the survey were also presented during a December briefing on Capitol Hill, where a panel of WEDI experts addressed policymakers and federal agencies to announce emerging automation trends that can reduce PA challenges, lower costs and improve patient care. Panel participants included Jay Eisenstock; Heather McComas, Director, Administrative Simplification Initiatives, American Medical Association; Sagran Moodley, Senior Vice President, UnitedHealthcare, Chair, DaVinci Project, Chair DRLS/CMS; and Jon Zimmerman, CEO, Holon Solutions.

In order for an assisted living community to achieve a deficiency-free survey, all services provided at the community must meet or surpass all state and federal guidelines. A deficiency-free survey in California’s thorough inspection is rare and a key indicator of excellence for an assisted living community. According to CDPH’s 2016-17 data report (most recent available) over 17,000 deficiencies were recorded among senior living communities.

Assisted living communities are inspected periodically to assess services and systems in place and oversee fulfilment with federal and state laws and regulations. In a multi-day process, surveyors arrive unannounced to conduct interviews, inspect the physical building, observe the operations, and check records. Inspectors determine compliance with regulations governing areas including quality of care, quality of service, safety, residents’ rights, and confidentiality. The assisted living community is awarded a “deficiency-free” survey when all of these regulations are met.

Starting Hearts Launches #SaveMoreLives Challenge and Partners with Presidential Daughter, Susan Ford Bales, to Help Save 100,000 Lives Per Year by 2040

Starting Hearts, a nonprofit committed to increasing awareness of Sudden Cardiac Arrest (SCA), announced it will kick off its first-ever Save More Lives Challenge on Feb. 1, 2020, to align with American Heart Month. This educational campaign is aimed at engaging the public in lifesaving techniques that can decrease the number one cause of death in the U.S., Sudden Cardiac Arrest.

“I founded Starting Hearts in 2010 after suffering a Sudden Cardiac Arrest at the young age of 27. Since then, our mission to combat the rise of SCA fatalities has accomplished remarkable success as a result of placing lifesaving defibrillators throughout Colorado’s Eagle County. Three New Organizations Commit to Improve Diagnostic Quality and Safety


“We need the active commitment of patients, clinicians, leaders in hospitals and health systems to come together to say reducing patient harm from diagnostic error is a priority,” said Paul L. Epner, Chief Executive Officer and co-founder of the Society to Improve Diagnosis in Medicine (SIDM).

Three leading healthcare organizations, Advocate Aurora Health, COLA Inc, and University Hospitals have pledged to take action to improve diagnostic quality and safety by joining the Coalition to Improve Diagnosis, which already includes nearly 60 of the most prominent organizations in healthcare and patient advocacy.

“Diagnostic error is one of the most important safety problems in health care today and causes the most patient harm,” said Paul L. Epner, Chief Executive Officer and co-founder of the Society to Improve Diagnosis in Medicine (SIDM). “Diagnosis is a complex process that involves the entire health care system working together effectively. That is why we need the active commitment of patients, clinicians, leaders in hospitals and health systems to come together to say reducing patient harm from diagnostic error is a priority. We need to work together to identify and implement strategies that improve diagnostic quality and safety.”

Convened and led by SIDM, the Coalition to Improve Diagnosis is the only major collaboration of diverse healthcare organizations singularly focused on improving the diagnostic process. The organizations behind the effort represent clinicians, patients, health systems, researchers and testing professionals—all acknowledging that improvement will require sustained work over several years with all stakeholders involved. Together, they work to find solutions that enhance diagnostic quality and safety, reduce harm and ultimately, ensure better health outcomes for patients.

The new coalition members represent large hospitals and health systems, as well as a laboratory accreditation organization. These three organizations have joined the nearly 60 other organizations taking action to improve diagnostic quality and safety.

Advocate Aurora Health is one of the ten largest not-for-profit, integrated health systems in the United States. Caring for nearly 3 million patients and offering more sites of care than any other health system in the Midwest. Diagnostic accuracy as a key aspect of their Patient Safety Strategic Plan. Through using tools such as the SPADE analysis and Safer Dx they aim to understand the improvement opportunities specific to the diagnostic process and develop strategies that help prevent resultant patient safety incidents.

“We are proud to join the Coalition to Improve Diagnosis as one of many ways we’re advancing our patient safety journey to achieve our goal of zero preventable patient harm.,” said Carrie E. Nelson, MD, MS, FAAFP, System Vice President Population Health & Health Outcomes at Advocate Aurora Health. “The literature is clear that diagnostic quality is at the root of some of the most serious patient safety events. Through our involvement with the Coalition, we aim to learn from and contribute to the community and knowledge base brought together under SIDM’s leadership to deliver consistent excellence in diagnostic quality.”

COLA, Inc accredits nearly 7,000 medical laboratories and provides the clinical laboratory with a program of education, consultation, and accreditation. As a national, physician-directed organization, they accredit laboratories of all types and sizes to strengthen the safety and accuracy of laboratory testing, which impacts patient experience, patient care, care management, and the interactions of the care team. As part of their efforts to improve diagnostic quality and safety COLA has developed an educational forum called Lab Testing Matters, in which a multi-disciplinary community of experts share their knowledge and best practices, and a near patient testing research initiative that highlights the value of real-time laboratory information to accurate, timely diagnosis and treatment.

“Reducing diagnostic error involves a multitude of disciplines along with patients and caregivers to form a strong, cohesive team working in concert to provide the best possible outcomes,” said Nancy Stratton, CEO at COLA. “As a physician-directed organization and by sharing our expertise and experience in laboratory accreditation, we are eager to participate with coalition members to find meaningful and practical ways to improve safety and quality in patient care,” she added.

University Hospitals located in Cleveland, OH, offers the region’s largest network of primary care physicians, outpatient centers and hospitals. They are in the process of developing a Center for Excellence in Diagnosis with a focus upon the rational use of diagnostic resources. The Center will focus upon optimizing health care provider diagnostic practices, engaging patients in the diagnostic process, developing guidelines for appropriate use of new and emerging diagnostic tests, and education to improve clinical diagnosis and reasoning.

“Improving quality and safety in general is a top priority at University Hospitals. We are especially focused upon improving diagnostic processes and reducing diagnostic error. Our commitment to the Coalition is a natural extension of the involvement of key system leaders in improving diagnosis,” said Goutham Rao, MD, FAHA, Chief Clinician Experience Officer at University Hospitals. “We are invested in developing a center of diagnostic excellence with an emphasis upon identifying, implementing, and evaluating best diagnostic processes across a variety of settings and disciplines. The Coalition will allow us to share and learn from other members as we move forward in this new decade.”

Mount Sinai Named Among Nation’s Top Health Care Employers for Diversity by Forbes

Statista surveyed more than 60,000 employees working for businesses with at least 1,000 employees apiece in the United States. Participants rated their organization on topics including age, gender, ethnicity, and disability. Underrepresented groups, including women and ethnic minorities, also provided their views on other employers within their industry. Diversity across leadership including top executives and boards, and access to public information, such as a designated chief diversity and inclusion officer, contributed to a company’s overall score.

The Fenway Institute Trump Year 3 Report Documents Expanded Harm to LGBTQIA+ Community

During Donald Trump’s third year as president, we moved from worrying that anti-LGBT policies would harm people to seeing the damage of these policies in action.

Today, The Fenway Institute of Fenway Health released a policy brief demonstrating that in 2019, the Trump Administration dramatically expanded upon discriminatory, anti-LGBT policies implemented in 2017 and 2018 that are harming the health and well-being of LGBTQIA+ people in America and around the world.

“During Donald Trump’s third year as president, we moved from worrying that anti-LGBT policies would harm people to seeing the damage of these policies in action,” said Sean Cahill, PhD, Director of Health Policy Research at The Fenway Institute. “The most dramatic examples have been the deaths of two transgender women seeking asylum in the United States who were detained in facilities currently being sued for their alleged abuse of LGBT migrants. Other moves have made LGBT people and people living with HIV much more vulnerable to discrimination in health care, social services, employment, education, and access to basic government services.”

Much of the harm LGBT people are now experiencing is the result of discriminatory actions that target them, such as rolling back sexual orientation and gender identity nondiscrimination provisions in health care, employment, and housing while expanding discriminatory religious refusal policies. But other policies that are not specifically aimed at LGBT people, such as restricting asylum hearings in the United States only to people who have been denied asylum in another country, and the maltreatment of people in border detention facilities, have disproportionately affected LGBT people from Central America seeking asylum to escape anti-LGBT violence.

In June, a transgender woman from El Salvador, who spent six weeks in a detention center in New Mexico that has been sued by the ACLU for creating “unconscionable conditions” for LGBT immigrants, died after being hospitalized for chest pain. The woman, a nurse, had repeatedly asked for an IV solution or, barring that, water, salt, and sugar so she could make and administer her own solution, but was denied medical care.

Two initiatives by the Trump Administration aimed at improving the conditions of LGBT people around the world and ending the transmission of HIV have been undermined by the administration’s own anti-LGBT policies.

During his 2019 State of the Union address, President Trump announced the Ending the HIV Epidemic Initiative, a new plan to end the HIV epidemic by 2030 with increased investments in outreach and prevention to communities hit hardest by HIV. But the campaign does little to address anti-LGBT discrimination and stigma, which are the underlying drivers behind the HIV epidemic among the most disproportionately affected populations in this country. Meanwhile, the Trump Administration’s work to uphold religious refusal protections, repeal nondiscrimination protections for LGBT Americans, and other such actions may actually increase discrimination against LGBT people, creating additional barriers to success for the HIV initiative.

In February 2019, the Trump Administration announced that it would launch a global campaign to end the criminalization of homosexuality as it is still a crime in 70 countries around the world, with 13 of them allowing for the death penalty. Since the announcement, the Trump Administration has been inconsistent in implementing its policy. While the US State Department publicly condemned anti-LGBT laws and actions in Chechnya and Brunei, it has been silent on similar anti-LGBT actions taken this year by Saudi Arabia, Qatar, and Kenya. The U.S. Ambassador to Zambia, who criticized the imprisonment of a gay couple there for 15 years, was told to leave the country by Zambia’s president, and was recalled by the U.S. State Department last month.

Other examples of actions taken by the Trump Administration in 2019 that undermine the health and well-being of LGBT people and people living with HIV include:


  • Proposing a new rule to reverse the Obama-era final rule implementing Section 1557 of the Affordable Care Act (ACA), which prohibits discrimination in the provision of health care. The 2016 Section 1557 rule was implemented to address anti-LGBT discrimination in health care, which can range from being verbally or physically harassed to being denied treatment altogether.
  • Proposing a new rule to remove explicit sexual orientation and gender identity nondiscrimination provisions from important health care regulations governing health insurance exchanges, Medicaid regulations, and the Program of All-Inclusive Care for the Elderly.
  • Proposing a new rule to remove regulatory provisions that explicitly prohibit organizations that receive HHS grant funding from discriminating on the basis of sexual orientation, gender identity, sex, and religion. Such a rule would affect organizations that provide a wide array of health and social services, including health care at federally funded community health centers, HIV and STI testing and prevention, refugee resettlement, elder care programs, childcare and after-school programs, community meal programs, and adoption and foster care services.
  • Nominating judges for lifetime federal appointments who have expressed anti-LGBT sentiments and/or have histories of ruling against the interests of LGBT Americans.
  • Arguing before the U.S. Supreme Court in favor of limiting existing nondiscrimination provisions under Title VII that have been used to protect LGBT workers.
  • Proposing an amendment to the Equal Access Rule, which ensures that homeless shelters do not discriminate based on sexual orientation or gender identity.
  • Amending the 2019 Notice of Funding Availability Act, administered by the Department of Housing and Urban Development, to remove a crucial incentive that encouraged housing providers to support LGBT individuals. LGBTQ youth and transgender people, especially people of color, experience disproportionately high rates of homelessness.
  • Eliminating disparate impact civil rights regulations which treat policies that are neutral on paper but have an unequal impact in practice as forms of discrimination, even if there was no discriminatory intent.
  • Finalizing a rule that allows healthcare providers to refuse to participate in medical procedures, such as infertility treatment for same-sex couples, abortion, and HIV treatment for religious reasons.
  • Proposing a new rule that would remove sexual orientation data collection for foster youth and foster and adoptive parents in the Adoption and Foster Care Analysis and Reporting System.
  • Implementing a ban on transgender troops that was first proposed in 2017 and which puts an estimated 13,600 transgender individuals at risk of being discharged.
