Partnership Brings New Level of Healthy Living Services to Walnut Square Affordable Housing Property

Operation Pathways, a subsidiary of The NHP Foundation and provider of a robust range of resident services, announced today at an onsite kick-off event for residents, that it has partnered with Centene Corporation’s Social Health Bridge™ Trust which helps community based organizations and healthcare entities work more effectively to address the social determinants of health – the non-medical barriers to health such as food insecurity, housing instability/homelessness, unemployment, and lack of access to transportation.

The partnership connects high-need residents living in low to middle income housing to social services integrated with their care plan, ensures care gap closure, and reduces reliance on local hospital ERs. New Name Clarifies the Key Role the Hospital Plays in the Mount Sinai Health System

Mount Sinai St. Luke’s hospital will now operate under the name Mount Sinai Morningside, reinforcing the hospital’s integral role as part of the world-renowned Mount Sinai Health System and the primary provider of health care to the 148,000 residents of West Harlem, including Morningside Heights.

The new name was approved by Mount Sinai Health System’s Boards of Trustees, the New York State Department of Health, and the New York Secretary of State. The change becomes effective immediately.

Blessing marketing and public relations leader name “Top Women in PR”

Chaka Jordan, MHA, vice president, Marketing & Planning, Blessing Health System, has been named one of the “Top Women in PR” by PRNEWS. She received the award at a luncheon in New York City on January 28, 2020.

Winners were judged based on their campaign objectives, successes and return on investment over the last year, as well as their overall influence within their organization and public relations profession at large.

PRNEWS honored Jordan for her leadership in the success of Blessing Health System’s “Know Where to Go” campaign. The campaign educates consumers about what conditions that require emergency care and where to go for treatment of non-life threating conditions, which has decreased non-emergent care in Blessing’s emergency department and increased use of Blessing’s retail clinics.

Jordan holds a Master of Health Administration degree from the University of North Carolina at Charlotte, and a Bachelor of Science in Public Health degree from the University of North Carolina at Chapel Hill. She is Green Belt certified in Six Sigma.

She came to Blessing from UNC Healthcare, Chapel Hill, NC. She previously held leadership positions at Health Management Associates, Mercy Health System, Wake Forest Baptist Health as well as Premier, Inc. She currently serves on both the United Way of Adams County and Chaddock board of directors.

First patients undergo new heart procedure at Blessing Health System

“The advantage of TAVR is that it is minimally invasive compared to a surgical aortic valve replacement. For patients who may not be a good candidate for surgery, TAVR can be an option for them to improve their health and quality of life,” said Amy Bates, APRN-AGACNP, Valve Clinic Coordinator.

The first patients in the region with a common, yet serious heart valve condition known as aortic stenosis, have been treated recently using a new procedure at Blessing Health System called Transcatheter Aortic Valve Replacement or TAVR. Blessing is the only healthcare provider in a 75-mile radius performing the TAVR procedure.

The Condition

Aortic stenosis is a narrowing of the aortic valve opening. Stenosis occurs when a valve stiffens due to damage, scarring or deposits, and subsequently has trouble opening. Because the valve is unable to open completely, the heart muscle must work harder to push blood through the valve.

Although some people have aortic stenosis as a result of a congenital heart defect called a bicuspid aortic valve, this condition more commonly develops during aging as calcium or scarring damages the valve and restricts the amount of blood flowing through the valve.

Symptoms of aortic stenosis include a heart murmur, breathlessness, chest pain, pressure or tightness, fainting, or a noticeable decline in activity level.

Jeffery Cook, MD, interventional cardiologist, Blessing Physician Services, says the condition can be missed because its symptoms are also associated with other conditions.

“It’s common to hear patients say. ‘I’ve had a murmur for a long time but no one has thought anything of it.’ By the time we see them, they are very debilitated, very short of breath, passing out, that sort of thing. And, lo and behold, they have severe aortic stenosis.”

Aortic stenosis mainly affects older people – beginning after age 60, but often does not show symptoms until ages 70 or 80.

The New Treatment Option

Treating aortic stenosis requires replacing the malfunctioning heart valves. For many years that had to be done during surgery. Some people are not candidates for surgery due to other health conditions. TAVR is a minimally invasive procedure, during which a new heart valve is placed via a catheter using X-ray guidance.

“The valve device deploys without having to stop the heart,” said John Arnold, MD, cardiothoracic surgeon, Blessing Hospital.

“The advantage of TAVR is that it is minimally invasive compared to a surgical aortic valve replacement. For patients who may not be a good candidate for surgery, TAVR can be an option for them to improve their health and quality of life,” said Amy Bates, APRN-AGACNP, Valve Clinic Coordinator, Blessing Health System.

Patients who undergo TAVR can be released from the hospital in two days, compared to 4-5 days for those who undergo surgical valve replacement.

The TAVR procedure is performed by Drs. Cook and Arnold. Valve Clinic Coordinator Bates assists the patient with arranging for the testing needed before TAVR and with any needs the patient may have after the procedure.

The wound care market is a battle between the drive for innovation and the desire to cut costs, as healthcare market researcher Kalorama Information notes in a new report. While wound care products themselves benefit from the cost cutting trend, as they reduce hospital stays, new reimbursement schemes can impact the $22.3 billion market. Kalorama, part of Science and Medicine Group, made its prediction in its Worldwide Wound Care Market Summary.

A lack of effective treatment options, adding to the increased costs for wound care has contributed to the growing frustration in the industry. It is the goal of new and existing product manufacturers to capitalize on these concerns by offering products designed to improve healing rates and prevent wound formation. Industry participants have attempted to diversify their offerings by acquiring smaller companies with new and innovative technology. This move has created more competition among large wound care companies, driving prices down.

The Quality Payment Incentive that is part of the Merit-based Incentive Payment System (MIPS) and Advanced Alternative Payment Models was implemented by the U.S. Centers for Medicare and Medicaid Services (CMS). Clinicians are included in MIPS if they are an eligible clinician type and meet the low-volume threshold. Beginning in payment year 2019, the low-volume threshold encompassed allowed charges, the number of beneficiaries who receive services, and the number of services provided. Clinicians must participate in MIPS (unless otherwise exempt) if, in the determination period, they bill more than $90,000 for Part B-covered professional services, see more than 200 Part B patients, and provide 200 or more covered professional services to Part B patients.

Changes in 2019 included an expanded list of clinicians who must report, including physical therapists, audiologists, registered dietitians, occupational therapists, clinical psychologists, speech pathologists, and nutrition specialists.

Other changes include expanded codes for deep-tissue pressure injury, effective October 2019.

For home care, starting January 1, 2020, the Centers for Medicare & Medicaid Services (CMS) will offer top reimbursement for the care provided to patients in the Patient-Driven Groupings Model (PDGM), including the assessment, treatment, and evaluation of surgical wounds, nonsurgical wounds, ulcers, burns, and other lesions. Per CMS data, the wound clinical grouping is one of the clinical groupings with the highest reimbursement potential.

Wound care patients have never been easy to manage, and now the industry must work to achieve success with this difficult population within the limitations of the PDGM payment cycle. Not only are wounds painful and stressful for the patient, but time and money also can be wasted when the wrong treatment is provided or even if the same treatment is carried out for too long without success.

Due to delay in the next round of competitive bidding, the program has temporarily ended last year – The gap in competitive bidding contracts is expected to last throughout 2020. What this means for participants is that any Medicare-enrolled durable medical equipment, prosthetics, orthotics, and suppliers (DMEPOS) supplier may furnish DMEPOS items and services to people with Medicare in the former competitive bidding areas (CBAs). Medicare patients can switch their DMEPOS supplier if they wish.
