Caravan Health to Share Projected Results for Nation’s Largest-Ever ACO

Today, Caravan Health announced that it will host a live webinar on Wednesday, March 4 to provide a progress report on the first year of the nation’s largest-ever accountable care organization (ACO). During this event, Caravan Health and its customers will share their unique approach to mitigating typical variances in ACO savings and losses.

Many Medicare Shared Savings Program (MSSP) participants see unreliable financial performance despite having the right processes in place. For most providers, the missing piece is scale. In 2019, Caravan Health pioneered the collaborative ACO model to help providers achieve consistent and predictable results. Today, the Caravan collaborative ACO supports approximately 235,000 attributed Medicare lives, allowing participating providers to achieve scale without losing autonomy.

Yakima Valley Farm Workers Clinic has seen significant positive results since joining the Caravan-supported ACO. The Clinic went from a zero percent annual wellness visit rate to more than 40 percent within one year of joining the collaborative. This is just one of many such examples.
