BrightStar Care Demonstrates Impact of Personal Care Services on Health Care Utilization in Latest Analysis by Avalere

BrightStar Care, a nationwide franchise that provides a higher standard of quality in-home care, released key findings today from an analysis conducted by Avalere Health, a leading US-based healthcare analytics firm. The analysis evaluated BrightStar Care’s client outcomes and cost benefits.

Demonstrating superior clinical outcomes has been a long-standing challenge for the home care industry due to fragmented care standards across states, the lack of standardized measures in personal care, and the lack of visibility home health care agencies have on claims data for patients. The results of the analysis suggest that BrightStar Care’s interventions were correlated with lower rates of inpatient admissions, emergency department visits, home health and skilled nursing facility utilization for clients diagnosed with certain chronic conditions.

Avalere Health compared BrightStar Care clients to a matched control group of Medicare Beneficiaries and assessed the relative utilization of Medicare services between the two populations for different health care conditions with the average age of the population being 80. Avalere’s analysis revealed that the change (baseline vs. follow-up) in the total cost of care was up to $29,902 lower for patients who received BrightStar Care services when compared to patients who did not receive services. The impact was concentrated in the subpopulation of patients with one of 30 chronic medical conditions, including multiple sclerosis, traumatic amputations, lung cancer, and seizure disorders, among others.

The analysis provides the latest evidence that personal care services, such as medication management, meal preparation, personal care assistance, and patient education, can have a positive impact on overall health, leading, in turn, to a reduction in healthcare costs and value creation for payers. BrightStar Care’s nurse-led care teams coordinate post-acute transitions, oversight on medication compliance, patient education and ongoing monitoring of patient change of condition. This assistance prevents avoidable hospitalizations and emergency room usage and improves the quality of life for clients.

These findings reinforced recent research from Moving Health Home, a coalition advocating for more care to be provided in the home, that showed home-based care can reduce the likelihood of hospital readmissions and reduce mortality. That analysis found that patients who utilize home-based care services within 14 days of discharge from an acute care facility are about 25% more likely to avoid readmission within 30 days of discharge.

Codoxo Unveils New Study; 15 Percent of Telehealth Claims are Non-Compliant with CMS Guidelines

“Before the pandemic, healthcare payers already were under siege, battling fraudulent and other anomalous claims cases. But the rapid adoption of telehealth has triggered an explosion of new novel schemes that can put millions of additional dollars at risk.”

Codoxo, a trusted provider of healthcare artificial intelligence solutions for healthcare payers and agencies, has released a new study revealing that among the 41% of provider specialties showing statistically significant increases for telehealth adoption, up to 15 percent of telehealth claims are non-compliant with CMS guidelines. The report, “Fraud Schemes in a Telehealth Era: What Healthcare Payers Should Know,” highlights provider specialties where telehealth modifiers were used but lack accompanying eligible CMS billing codes, which may indicate wasteful, abusive or erroneous practices and warrant immediate investigation.

“With telehealth here to stay, our analysis of telehealth data and guidance for cost avoidance should be useful to health plans as they navigate a new age of virtualized healthcare delivery,” said Saurav Subedi, Director of Data Science at Codoxo. “Before the pandemic, healthcare payers already were under siege, battling fraudulent and other anomalous claims cases. But the rapid adoption of telehealth has triggered an explosion of new novel schemes that can put millions of additional dollars at risk.”

Codoxo’s study was conducted over claims submitted across various geographic regions from September 2019 to July 2021. The study spans multiple lines of business, including Medicare, Medicaid, and commercial. The report features the top 10 CPT/HCPCS codes linked with telehealth modifiers even though they aren’t among codes recommended by CMS. These 10 codes represent 58% of all non-compliant codes found in Codoxo’s analysis.

Within the first three weeks of the WHO declaring the COVID-19 pandemic, telehealth visits skyrocketed 154 percent, according to the Centers for Disease Control and Prevention, compared to the same time period just one year prior. After sharply peaking in April 2020, telehealth has since leveled off, finding a new elevated steady state at approximately 20-times higher than pre-COVID rates, according to Codoxo’s anonymized health plan data.

AONN+ Supports Accelerating Kids’ Access to Care Act

The Academy of Oncology Nurse & Patient Navigators (AONN+) joined Leukemia & Lymphoma Society and other cancer patient community organizations in supporting increased access to time-sensitive care for children via the bipartisan Accelerating Kids’ Access to Care Act.

Children who are on Medicaid and have medically complex conditions must often travel to different states to receive highly specialized care that is not available in their home state. Currently, the process to seek out-of-state medical service is difficult and full of regulatory hurdles, delaying children from receiving the care they need, and in some cases not receiving access to care at all.

Introduced in both the House and Senate, the Accelerating Kids’ Access to Care Act would better facilitate access to care across state lines. This bill requires state Medicaid programs to establish a process through which qualifying out-of-state providers may enroll as participating providers without undergoing additional screening requirements.

Among other things, a qualifying provider must (1) serve minors, or adults whose condition began as a minor, and (2) have previously been screened for Medicare participation or for participation in the Medicaid program of the state in which the provider is located.

Even though the majority of AONN+ membership work with the adult population, the AONN+ Policy and Advocacy Committee supports this act as a means for best treatment for children with cancer that requires out-of-state medical care. Delayed access to care without a streamlined screening for out-of-state approval can lead to postponed care and ultimately poorer outcomes.

“Navigation is all about not duplicating work and since no formal process to facilitate a streamlined screening exists among the states, this will save time and money on enrollment requirements,” said Sharon Gentry, AONN+ Program Director.

“It also has a time-sensitive element that navigators face daily as they remove barriers for more timely treatment that does improve outcomes in survival. The letter is a reflection on navigation concepts with the ask to coordinate timely specialized care while maintaining a suitable quality of life for the patient.”

The Accelerating Kids’ Access to Care Act does not address authorization of out-of-state care or any reimbursement issues; the bill only instructs Medicaid to create an opt-in pathway for providers in good standing who are caring for children so they can provide time-sensitive care to kids on Medicaid who need it.

This bipartisan legislation is led by Congressmen Katherine Clark (D-MA) and Jaime Beutler (R-WA), and Senators Michael Bennet (D-CO) and Charles Grassley (R-IA).

The rotating clamp attachment is smaller and narrower than the standard clamp with a new bespoke rotation system for the very small clamp attachment, with locking indents to hold fixed positions at pre-set angles.

The cheese is stored on racks to mature and turned 180 degrees every few days. The clamp attachment has been redesigned with a low profile and very narrow rotation head to enable the clamp to access the cheese on the racks and rotate them by 180 degrees.

The cheese is regularly transported to a washing area to be washed at a 45-degree angle on both sides of the cheese. The lifter transports the cheeses to the wash area and securely holds the cheese at the required angle.

The clamp attachment maintains the required fixed angle of 45 degrees by means of bespoke threaded ball index plungers and locking indents, suitable for the narrow rotation mechanism.

The clamp attachment features electric clamping incorporating a low-capacity torque limiter ensuring the clamp does not overtighten and damage the cheese.

The lifting equipment incorporates powered lifting and clamping, with manual rotation.

This lifter is entirely suitable for the food industry and other hygienic applications and features FDA approved nitrile rubber rollers. The design offers impressive stability, strength, and durability and together these features combine to provide the transportation of the rolls with ease, confidence and compete safety.

The attachment is backed up by a full bespoke design service ensuring the right solution can be made to suit most applications and requirements.

Biography

Rob A. Gentile was born one of four boys outside of Pittsburgh, Pennsylvania to Italian immigrants. His grandfather and father worked at a steel company where Rob also began his career right out of high school.

When the steel company that Rob had been working for went out of business, he found himself pursuing many of his talents, including his passion for photography. Rob drove to Guatemala to learn Spanish and became a freelance stringer covering the Contra war in Nicaragua for the New York Times and the Chicago Tribune in the late 1980’s. Rob spent time in combat situations. Often, young soldiers would dive on top of Rob to protect him while they were under fire. They told him they did that so he could help show the world the truth. Rob says that living through war was like training for the challenges that were looming in his life in the not-too-distant future.

Married to his wife, Melanie, for over 30 years, they have devoted themselves to their special needs daughter, Maria, who suffers from Rett Syndrome. Maria is in her twenties, and Rob refers to her as “pure spirit.” Throughout her childhood, he grappled with tough questions about prayer and why children must suffer.

For the past 25 years, he has worked as a sales engineer living in Texas, California, Tennessee and now resides in Charlotte, North Carolina. Rob has won several awards for his sales efforts including diversity supplier award for the Minority Business Development Council.

At age 56 he had a massive heart attack, then flatlined, and had a near-death experience (NDE). Self-discovery and spiritual awakening continued while waiting to receive a donor heart. His first book, “Quarks of Light” became a best-seller.
