Monoclonal Antibodies Market for Cancer Treatment Tops $73 Billion: Kalorama Information

The global market for monoclonal antibodies in cancer treatment topped $73.1 billion in 2022, and is forecast to increase to $94.8 billion in 2027. This is the finding of Kalorama Information’s recent report on Precision Cancer Therapeutics Market, 2022. Kalorama Information is a medical market research firm that has been covering pharma and in vitro diagnostics (IVD) for two decades, and is owned by Science and Medicine Group.

Monoclonal antibodies are immune system proteins that are created in the lab. Monoclonal antibodies are homogeneous populations of identical antibody molecules produced by specialized tissue cell culture lines. The procedure uses cell fusion techniques and standard in vitro tissue culture systems. The two essential biologic components are immunized mice or rats and myeloma tumor cell lines, which are of lymphoid origin.

Monoclonal antibodies are finding wide application in many areas of medicine and biologic science. Thousands of monoclonal antibodies have been made against many different types of antigens. Monoclonal antibodies have begun to replace conventional antibodies in blood banking and are used in the identification of organisms in the bacteriology laboratory. Monoclonal antibodies have also been extensively used in radioimmunoassay to measure serum levels of various substances.

Among monoclonal products are the following:

“Avastin (bevacizumab)”

Avastin is designed to inhibit VEGF, a protein that plays an important role in tumor angiogenesis and maintenance of existing tumor vessels. Avastin contains the active ingredient bevacizumab, which binds VEGF and prevents the interaction of VEGF with receptors on the surface of endothelial cells.

Avastin is most notably approved for the treatment of metastatic colorectal cancer. The FDA approved Avastin in February 2004 for the first line treatment in combination with intravenous 5-Fluorouracil-based chemotherapy treatment. Avastin received approval for the same indication in the European Union in January 2005 after Roche submitted a marketing authorization for the product in December 2003. Avastin was also approved in June 2006 in the United States for second line treatment for colorectal cancer.

Other approvals for Avastin have followed, including approvals for brain, kidney, lung, ovarian and other cancers.

“Blincyto (blinatuomab)”

Blincyto offered by Amgen is a monoclonal antibody that is indicated for the treatment of CD19-positive B-cell precursor acute lymphoblastic leukemia in the first or second complete remission with minimal residual disease. The product was FDA approved in February 2018.

“Darzalex (daratumumab)”

Darzalex injection is a CD38-directed cytolytic antibody. CD38 is a surface protein expressed across multiple myeloma cells and not limited to a specific disease stage. It is indicated for use as a combination therapy for multiple myeloma, and a monotherapy for patients with multiple myeloma who have received at least three prior lines of therapy.

In 2012, Janssen Biotech and Genmab A/S entered an agreement for Janssen to undertake the development, commercialization and manufacturing. Approved by the FDA originally in November 2015, Darzalex has many more indication approvals.

“Erbitux (cetuximab)”

Erbitux is a monoclonal antibody designed to target and block the EGFR, which is expressed on the surface of certain cancer cells. Erbitux binds specifically to epidermal growth factor receptor (EGFR, HER1, c-ErbB-1) on both normal and tumor cells, and competitively inhibits the binding of epidermal growth factor (EGF) and other ligands, such as transforming growth factor-alpha.

Geneticure Presents Data Successfully Using Genetics to Identify Responders to Renal Denervation

Geneticure, a global leader in the genetic variability of response to hypertension treatments, presented data at the 2022 American Heart Association Scientific Sessions meeting from a recent clinical study in which they successfully predicted patients who will respond to renal denervation (RDN). In addition, the Geneticure test for renal denervation also identified patients with an increase in blood pressure, following the procedure.

Geneticure genotyped subjects from the Medtronic OFF-MED renal denervation trial to assess the relationship between the genetic scoring and 24-hour blood pressure changes from baseline to three months. There was a significant relationship between the Geneticure score and response to renal denervation. When divided into a median split of the Geneticure score, those with a high responder score had a ~14mmHg drop in blood pressure and those with a low score had no change in blood pressure, a statistically significant difference (p<0.001). When dividing the patients into tertiles, the highest tertile demonstrated a drop in blood pressure of ~13mmHg, the middle tertile a drop of ~6mmHg, and the low tertile an increase in BP of ~5mmHg.

Geneticure’s cheek-swab test analyzes genes from organ systems known to be important in RDN, which are then entered into a weighted algorithm. Previous studies have demonstrated a range of findings from no difference in RDN compared to a sham procedure, to just under 5mmHg response vs. sham. More importantly, without genetic screening, RDN shows no clinically meaningful drop in blood pressure (widely considered to be 5 to 10mmHg) when compared to sham in ~30% of patients and an increase in blood pressure post-denervation in ~20% of patients, even in trails that have no medication changes during the study.

“These data are critical because all of the recent RDN trials have failed, or shown a relatively low <5mmHg, drop in blood pressure between RDN and sham”, said Dr. Eric Snyder, Geneticure co-founder, and COO. “Further, more recent long-term follow-up trials from the ‘real life’ database of patients with radiofrequency ablation have relied heavily on data carried forward from 6, 12, or 24 months to the end of the trial in the sham group, thus leading to an artificial difference in the two groups. This artificial increase in blood pressure in the sham group in these trials is abundantly clear because each trial demonstrates no significant difference in most variables in non-imputed data within the appendix”.

The number and variety of protein therapeutics have increased dramatically in recent years. To satisfy therapeutic demand, these molecules must be produced at large scale in mammalian or bacterial cell hosts; but with the greater variety in protein therapeutic molecules, comes greater challenges in purifying complicated molecules to a level of quality that is required, with a process that is scalable. In this webinar, the featured speaker presents process development approaches used to overcome challenges in the recovery of a bioactive multi-unit fusion protein.

Overexpression of proteins in a bacterial host often leads to the formation of inclusion bodies; the process of purifying proteins from inclusion bodies can be laborious and the yields are often low. A greater challenge is the need to recover bioactive proteins with a high content of cysteines.

The bioactive form of the protein of interest is a monomer, but because of several cysteines in the sequence, this fusion protein can form monomers in several conformations, as well as dimers, and higher order complexes during refolding stage, with some of the monomeric conformers having higher activity than others. The speaker concentrated on the optimization of a purification process — which would produce an increased yield of the most active monomeric conformer while minimizing the presence of other protein forms — more specifically, the solubilization process from inclusion bodies, protein refolding and further purification of the bioactive monomer using column chromatography.

The speaker will present data from small-scale solubilization and refold studies, which were employed to discover optimized conditions for parameters such as buffer formulation as well as incubation time and temperature. The presented data will also help determine operating parameters for purification by column chromatography, which were found to be critical for minimizing formation of higher order complexes during processing.

In an effort to further define the correlation between climate change and radon gas, CR3 launched a publication dedicated to making it clear that radon gas is affected by climate change.

The 2022 November issue of CR3 News Magazine offers a strong look at some of the climate change variations and how they potentially increase radon levels.
