Clinician Choices Often Differ From Those of Experts When Switching HIV Treatment in Patients With Virologic Suppression

Healthcare providers show marked differences in regimen choice compared with experts when selecting a switch regimen for patients requiring a change in ART for reasons other than virologic failure. The finding was derived from an analysis of data captured between March 2018 and April 2019 from an online treatment decision support tool developed by Clinical Care Options (CCO). The tool allows users to define a specific patient case scenario based on variables such as current regimen, treatment history, and patient characteristics and comorbidities and then to instantly see which switch regimens are recommended for that exact scenario by a panel of 5 leading HIV experts.

In nearly two thirds of the 1098 cases entered into the online tool, healthcare providers were planning to switch to regimens that differed from those selected by experts based on cardiovascular disease, the presence of mutations associated with HIV drug resistance, or the patient’s HLA-B*5701 status.

“Our data show not only that there is a major discrepancy between switch decisions taken by community clinicians compared with experts, but also that an online decision support tool can help guide clinicians to better choices. In many cases where clinicians initially selected an option that differed from the experts, they indicated that they would change their treatment plan after using the tool,” said Edward King, MA, Executive Vice President of CCO.
