PurposeCare Acquires Michigan-based Home Sweet Home


PurposeCare, a leading provider of coordinated home care and home health services in the Midwest, announced today the acquisition of Home Sweet Home In-Home Care (“Home Sweet Home”), a St. Joseph, Michigan-based independent home health care agency that has provided high-quality services to southwest Michigan residents since its inception in 2011. Home Sweet Home has enjoyed a sterling reputation within the community for delivering outstanding care and is well respected by all stakeholders in the company including clients, referral partners and payors.

The acquisition will provide additional depth to PurposeCare’s suite of home health care services. PurposeCare, a portfolio company of Lorient Capital, provides home based care services to more than 3,000 clients a week. The company’s primary audience are individuals who are dually eligible for Medicare and Medicaid. The PurposeCare platform is focused on leveraging the caregiver’s presence in the home to identify changes in its client’s condition, enabling a timely clinical intervention from the nurses and therapists on the PurposeCare team. With the acquisition of Home Sweet Home, PurposeCare will expand their services to an additional 400 patients and more than 300 caregivers.
