Oak Street Health, a network of primary care centers that delivers value-based care to adults on Medicare, today announced further plans for nationwide expansion in 2020 with the company’s first centers in New York and Mississippi anticipated to open later this year. The company plans to open two locations in Brooklyn, N.Y., and a new center in Jackson, Miss.
“The planned opening of our first centers in New York and Mississippi represents a continued commitment to providing access to innovative, high-quality, value-based primary care to older adults where it is most needed across the country,” said Tamara Jurgenson, Chief Growth Officer of Oak Street Health. “We are excited to introduce our model of care to seniors in these new communities.”
Oak Street Health is continuing to move forward with growth plans while also focusing efforts on caring for the company’s more than 85,000 patients across more than 50 centers in eight states during the COVID-19 pandemic. Prior to the pandemic, Oak Street Health opened its first center in Tennessee and also announced plans to enter the state of Texas with centers anticipated to open in the greater Dallas-Fort Worth area by early fall.
“We have been able to meet the unique challenges presented by the pandemic, and our ability to adapt and provide care for our patients at this critical time is a testament to Oak Street Health’s innovative value-based healthcare model,” said Mike Pykosz, CEO of Oak Street Health. “Our patients always come first and with our upcoming entry into New York and Mississippi, we are furthering our company’s mission of rebuilding healthcare as it should be.”
Oak Street Health patients can expect an unmatched healthcare experience that includes quality time with their provider in-person or via telehealth visit; a 24/7 patient support line; individualized, preventive care plans; and access to transportation to and from the center for eligible patients. Additional services, such as behavioral health care and social health support, and Medicare education classes, are also offered to help older adults meet their unique needs in one convenient location.
Since its founding in 2012, Oak Street Health has seen a 51 percent reduction in patient hospital admissions compared to Medicare benchmarks. Oak Street Health will accept multiple health plans at all locations, including traditional Medicare.
