NTOCC is excited about aligning our educational resources with ABQAURP in support of improving transitions and care coordination.

Throughout a long-standing relationship, The National Transitions of Care Coalition (NTOCC) and the American Board of Quality Assurance and Utilization Review Physicians(ABQAURP) have worked together to bring insight to successful care transitions for the health care team. The newest initiative is an educational endeavor by NTOCC that will be accredited by ABQAURP for CME/CE and available for health care professionals from both NTOCC and ABQAURP. These live and recorded webinars are a part of a lunch and learn series advancing the mission to improve care transitions.

Transitions of care, whether between physicians, within hospital departments, or when patients are discharged, are a major challenge to health care delivery. Reducing unnecessary, wasteful, duplicative, and potentially harmful services is essential to providing quality, accountable, and affordable health care. NTOCC’s Lunch & Learn Series brings various perspectives from multiple health care stakeholders on transitions of care interventions, services, resources, and public policy issues.

Dr. James Lett, NTOCC President, shared, “NTOCC is excited about aligning our educational resources with the American Board of Quality Assurance & Utilization Review Physicians (ABQAURP) in support of improving transitions and care coordination for clinicians, patients, and family caregivers. Our combined efforts address the various levels of care and interventions that are available to clinicians and help to focus the need for improved patient-centered care”.

As an ACCME-Accredited with Commendation provider, ABQAURP is excited to jointly provide these courses to the health care community.

Furthermore, ABQAURP provides certification to enhance the professional’s credentials by achieving Certification in Health Care Quality and Management with additional Sub-Specialty certification available in Transitions of Care, Case Management, Physician Advisor, Managed Care, Patient Safety/Risk Management, and Workers’ Compensation. ABQAURP has worked collaboratively with the American College of Physician Advisors, CMSA, and NTOCC to facilitate increased qualifications, competence, and authority within the field of Health Care Quality Management, essential to ensuring value-driven, safe, and equitable health care for patients. Certified professionals have demonstrated their competence, skills, and commitment to breaking down the barriers between different health care settings and working collaboratively for the good of the patient.

About NTOCC

The National Transitions of Care Coalition is a 501(c)(4) organization dedicated to addressing a serious U.S. health care issue: filling the gaps that occur when patients leave one care setting and move to another care setting. These transitions include patients moving from primary care to specialty physicians; moving or transferring patients from the emergency department to intensive care or surgery; and when patients are discharged from the hospital to home, assisted living arrangements, or skilled nursing facilities.

Since 2006, NTOCC’s Advisors Council of over 30 organizations has shared a common goal of improving the quality of Transitions of Care. Working in conjunction with 450 Associate Member organizations and over 3,000 individual professional subscribers, NTOCC has developed tools and resources made available to everyone in the health care industry including providers, payers, patients and consumers.
