Schedule an Appointment with an Ohio Medical Marijuana Doctor

Once you’ve completed our eligibility survey, the next step is to schedule an appointment with one of our Ohio medical marijuana doctors. Due to the COVID-19 pandemic, the State Medical Board of Ohio is allowing medical cannabis physicians to utilize Telemedicine for both initial recommendations and renewals. This means you may see your DocMJ physician from the comfort of your own home via Telemedicine using a computer, tablet, or smartphone. After your appointment is scheduled our office will send you an email containing a set of electronic intake forms. These forms must be completed at least 24 hours prior to your in-person or Telemedicine appointment. If you experience any issues while completing your forms, please contact our office directly at (877) 899.3626 for assistance.

You will be required to submit a copy of your most recent medical records and your Ohio drivers license or state ID card. You may upload your medical records to your intake forms or fax them to our office. If you do not have your records on hand and are unable to obtain them, please let our office know and we will be happy to request them for you.

Get Approved & Visit a Dispensary

You will find out during your appointment if you qualify for a recommendation. If you are approved your physician will enter your information into the OHMM Registry. Once entered into the Registry you will need to complete your application and pay the $50 state-required fee for your Ohio medical marijuana card. This fee is separate from your exam fee. Immediately after making your payment, you will receive an email containing a digital copy of your Ohio Medical Marijuana Registry Card. This digital card can be printed out or saved to your phone and can be used to purchase medical marijuana products at any dispensary in Ohio. Please note, you are required to be seen by your medical marijuana doctor once every 12 months.
