This event is free and takes place on Saturday, September 21, at L.A. Mission College, 13356 Eldridge Ave., Sylmar, CA 91342, from noon to 4 p.m. The Foundation for Senior Services Wellness Expo will feature entertainment and educational sessions, including: eye health care, physical fitness, financial wellness, and private consultations with experts in a variety of fields.

The mission of the Foundation for Senior Services is to provide seniors and their families with education, options and resources so they can choose quality care and assistance to preserve independence and options. The Gala Dinner ($75 for non-members, $50 for members and non-profits) to follow is scheduled for 5-8 p.m., and the proceeds will be used to supplement qualifying seniors for services that will improve their quality of life. Many older Californians are in need of support as medical bills and housing costs mount, and the Foundation for Senior Services is committed to connecting them to resources.

AdvantagePlusCaregivers.com® is proud to be the title sponsor to such a worthy cause. Advantage Plus Caregivers is committed to providing compassionate care to its clients and will be showcasing its Transitional Care program. They offer their clients the latest developments in senior care. Transitional Care Services is offered to all clients who need to be admitted to the hospital due to illness or injury. The service is recommended to reduce the chances of hospital readmissions, providing their clients access to support so they can focus on recovery in their own homes.

On-demand Transitional Care Managers support families by managing medications, arranging transportation services and setting up follow-up care. Transitional Care Services connects patients to specialists who can conduct safety inspections and home modifications, or install specialty equipment to ensure a smooth transition home after a hospitalization.
