SITC Releases Executive Summary from the Crisis in Clinical Research Virtual Summit

On Aug. 17, 2022, the Society for Immunotherapy of Cancer (SITC) convened key oncology experts from academia, industry and government, including the FDA and NCI, to lead a national conversation to address a crisis in clinical research. Limited patient access to clinical trials and slowed drug development due to staffing shortages, administrative burden, and current business models have led to drastically reduced capacity to conduct potentially lifesaving oncology clinical trials.

The virtual summit convened over 600 stakeholders and 30 panelists to discuss the multi-faceted problems in oncology clinical research today. From this summit, SITC has created an Executive Summary document that outlines the existing problem, the summit goals and proposed solutions that could expand access to clinical trials for patients and increase capacity at sites conducting the trials.

“The overall goal of this summit was to better define the issues that exist and discuss possible solutions to this critical situation,” said SITC President Leisha Emens, MD, PhD. “This incredible event crystalized the core issues through robust discussions with relevant stakeholders that ultimately brought the crisis into the national spotlight.”

SITC’s proposed solutions for addressing the crisis facing clinical research include:

  • Standardization and Centralization of Clinical Research Resources
  • Cross-institutional Data Availability
  • Maximizing Efficiency Across Clinical Research Operations
  • Diversification of Clinical Research Sites and Populations

This executive summary, created as a tangible deliverable from the summit, also includes the next steps that SITC is working on with relevant stakeholders in an effort to address the four key takeaways.

“The breakthrough developments of the CAT-SDoH and SVM open the door to a new approach to the measurement of SDoH,” said Robert Gibbons, Ph.D., Blum-Riese Professor, University of Chicago. “If we know a person’s zip code, the SVM items for that person’s environment can be immediately determined.”

A new research study published in Health Services Research presents the development and validation of a new metric for social vulnerability. The breakthrough development of the social vulnerability metric (SVM) opens the door to a new approach to the estimation of social vulnerability from the ecological perspective. The SVM can serve as a tool for public health policy and practice, significantly impacting the understanding of an individual’s risk based solely on their zip code.

According to the US Department of Health and Human Services, social vulnerability refers to the potential negative effects on communities caused by external stresses on human health1. Social Determinants of Health (SDoH) are environmental conditions such as economic stability, access to healthcare, or place of residence, that impact an individual’s health and well-being, often contributing to health disparities and inequities.2,3 When SDoH is accurately measured, it can help public health departments and policymakers appropriately target interventions for communities with the greatest needs.

The SVM was developed using 2018 data obtained from the Federal and publicly available Agency for Healthcare Research and Quality (AHRQ) Social Determinants of Health (SDoH) Database, a dataset of SDoH variables from data sources spanning 2009 to 2018, measured at the Zip Code Tabulation Area (ZCTA).4 ZCTAs differ from zip codes because they only include populated areas; for example zip codes that only include PO boxes are not included in ZCTAs. From these data, researchers fit a bifactor MIRT model to the reviewed set of 94 different SDoH variables to provide a single-value SVM score.5 The bifactor model was applied to data representative of 33,120 zip codes without missing data, which includes 99% of all U.S. zip codes.

The SVM was validated against several health indicators at the national, state and city level. First, the SVM was validated against nationwide age-adjusted all-cause mortality rates provided by the CDC at the county level. The SVM accounted for 46% of the nationwide variability in mortality rates (r=0.68), whereas CDC’s measure, the Social Vulnerability Index (SVI) only accounted for 12% of the variability in mortality rates (r=0.34). Second, it was validated against State of California zip-code level COVID-19 vaccination rates (r=-0.68) and emergency department visits for asthma (r=0.62 for youth and r=0.60 for adults). The negative correlations with COVID-19 vaccination rates indicate that more socially vulnerable populations had less access to COVID-19 vaccines early in the pandemic. Third, it was validated using the Chicago Department of Public Health COVID-19 mortality and vaccination rates. The associations between the SVM scores and COVID mortality and vaccination rates. The correlations across the 58 zip codes were r=0.71 for COVID mortality and r=-0.85 for first vaccination rates. Socially vulnerable communities experienced six times the rate of COVID-19 mortality compared to less socially vulnerable communities (300/100,000 versus 50/100,000 respectively) and a much lower first vaccination rate (32% versus 79% respectively).

Gibbons, PhD, Department of Health Policy and Management, Johns Hopkins Bloomberg School of Public Health; Cody Brannan, MS, University of Chicago; and Diane S. Lauderdale, PhD, University of Chicago, also developed an interactive web application.

  • View the relationship between county and zip-code SVM scores and 24 health indicators (such as asthma, diabetes, and stroke) nationally or by state
  • View or download the county or zip-code level SVM data

“Social determinants of health not only impact people’s health and wellbeing, but it can also contribute to health disparities and inequities,” said Lauren Saulsberry, PhD. “With this study, the SVM was derived from various SDoH variables from multiple nationally representative public databases using multidimensional item response theory (MIRT), ultimately providing a higher level of precision than any existing SDoH metric. The SVM offers a measurement tool improving upon the performance of existing SDoH composite measures and has broad applicability to public health that may help in directing future policies and interventions.”

Researchers also have an exciting addition to this already forward-thinking work. Adaptive Testing Technologies, developer of the CAT-MH® assessment tools, has been collecting data using the CAT-SDoH, a measure of an individual’s personal perspective of their life satisfaction. Together with the SVM, the ability to measure SDoH accurately and precisely in a population has never been greater. When combined, the SVM and CAT-SDoH further improves the adaptive assessment of an individual’s true social vulnerability, reflected as a synthesis of both their ecological and personal perspective.

About the Study

The objective of this study was to derive and validate a new ecological measure of the social determinants of health (SDoH), calculable at the zip code or county level. The SVM was constructed from U.S. zip-code tabulation area level measures from survey data using multidimensional Item Response Theory and validated using outcomes including all-cause mortality, COVID-19 vaccination and mortality rates, and emergency department visits for asthma. The SVM was also compared with the CDC/ATSDR Social Vulnerability Index (SVI) to determine convergent validity and differential predictive accuracy.

Catalent and Ethicann Sign Development and License Agreement for New Fast-Dissolve Cannabinoid-Based Treatments

Catalent, the global leader in enabling the development and supply of better treatments across multiple modalities, and Ethicann Pharmaceuticals Inc., a Canadian/U.S. specialty pharmaceutical company focused on the development of high-value cannabinoid drug therapies, today announced that the companies had executed a development and license agreement to develop Ethicann’s clinical drug pipeline using Catalent’s proprietary Zydis® orally disintegrating tablet (ODT) technology.

Under the terms of the license, financial details of which have not been disclosed, Catalent will look to develop Ethicann’s cannabidiol (CBD) and tetrahydrocannabinol (THC) pharmaceutical products using its Zydis technology in clinical trials across a range of indications including multiple sclerosis (MS) spasticity, chemotherapy-induced nausea and vomiting, chronic pain for cancer, and epilepsy. This agreement follows a feasibility study that showed the Ethicann EPI-002 candidate to treat MS spasticity formulated with Zydis ODT dissolves in approximately three seconds without the need for water and is rapidly absorbed through the lingual and sublingual mucosa into the blood, providing therapeutic advantage by bypassing the liver, which typically removes 30-45% of cannabinoids absorbed through the stomach. Ethicann anticipates performing a human bioequivalence study of EPI-002 in late 2023-early 2024, which is required for new drug approvals in Canada.

Zydis is a unique, freeze-dried, oral solid dosage form that disperses almost instantly in the mouth, without the need for water and has a dispersion speed of as little as three seconds. Zydis is the world’s fastest and best-in-class orally disintegrating tablet and is used in over 35 drugs approved in 60 countries.

Hospital M&A Activity Slowed in Q4:2022, According to Acquisition Data from LevinPro HC

Headwinds and market challenges took a toll on Hospital M&A activity in the fourth quarter of 2022. According to new acquisition data captured in the LevinPro HC platform, there were 19 transactions announced in the Hospital sector in Q4:22, a 10% decline from activity in Q3:22 when 21 deals were announced. Fourth quarter activity was virtually level with Q4:21 transaction volume when 18 deals were announced.

Announced spending reached $821 million in Q4:22, a decline of 89% compared with the third quarter of 2022. In Q3, Mediclinic International Ltd., a diversified international private healthcare services group founded in South Africa, was purchased for $7.42 billion by Remgro Limited and MSC Mediterranean Shipping Company, one of the largest hospital deals on record.

Although deal volume was slow, there were still several notable deal announcements, especially from large health systems. In the fourth quarter, Yale New Haven Health System, the most extensive healthcare system in Connecticut, purchased the operations and outpatient clinic services of three hospitals from Prospect Medical Holdings to expand its network in the state. In a separate deal, Prospect Medical Holdings purchased the real estate of the three hospitals from Medical Properties Trust, Inc. for $457 million, the largest deal of the fourth quarter based on disclosed price.

In December, OhioHealth closed negotiations with both Guernsey Health Systems and Van Wert Health, adding two hospitals, more than 100 beds and a collection of physician groups, outpatient clinics and home health operations to its network.

Two major health systems in Michigan, University of Michigan Health System and Sparrow Health System, merged in early December, creating a $7 billion organization and one of the largest healthcare providers in the state.

Most fourth-quarter transactions targeted systems and organizations with more than 100 beds and several facilities. Very few deals focused on critical access hospitals or community hospitals. In the 19 deals announced in Q4:22, more than 5,000 beds and 45 hospitals were acquired or involved in a merger.
