The Benefits of a Fully Digital Approach to Oral Surgery


In this retrospective triple case report, published in the Journal of Oral Implantology, researchers aimed to present and discuss the technical considerations related to using a new maxillary sinus surgical guide to improve surgical outcomes.

LAWRENCE, KS, Sept. 21, 2023 /PRNewswire-PRWeb/ — Journal of Oral Implantology – Dental implant procedures can be challenging no matter the placement location in the mouth; however, implants in the back of the mouth—premolar and molars—can be especially difficult. The complication occurs when the original tooth is extracted, which can expand or pneumatize the sinus cavity. Dental Implant surgeons frequently use a sinus floor elevation via lateral window (LSFE) procedure to help counteract this effect. While the LSFE is a predictable, commonly used technique, it can lead to adverse effects, including sinus membrane perforation and significant bleeding. Safer molar extraction and implantation techniques are necessary to decrease these adverse effects. The Journal of Oral Implantology recently published a retrospective triple case report by collaborating researchers from the Pontifical University Catholic of Rio Grande do Sul, Porto Alegre, Brazil; the University of Michigan School of Dentistry, Ann Arbor, MI; the Shahid. Beheshti University of Medical Sciences, Tehran, Iran; and the Virginia Commonwealth University, Richmond, VA. Lead author Priscila C. Meneghetti, DDS, MS, and colleagues state, “To promote accurate surgical procedures and improve the surgical workflow with simultaneous implant placement, a 3D printed surgical guide was designed to incorporate the lower contour of the maxillary sinus access window and the ideal 3D position of the implant. This article aims to present and discuss technical considerations related to using this new maxillary sinus surgical guide to improve surgical outcomes.”

Meneghetti et al. conducted a retrospective three-case report on patients qualified for the LSFE technique with a simultaneous implantation procedure using the “all-in-one LSFE and implant placement surgical guide” designed by an experienced digital dental clinical. All patients underwent dental implant surgery to replace the first molar in conjunction with LSFE done under intravenous sedation. The implant achieved primary stability in all cases, and the procedures were done without complication.

Postoperative follow-up occurred at two, four, eight weeks, and five months. No severe adverse effects, including unmanageable pain, swelling, nausea, or nosebleeds, were reported in any of the three patients. All implants were confirmed to be successfully placed and healing as expected. This advanced digital dentistry technique also successfully minimized sinus membrane perforation and excessive bleeding that can occur during the LSFE surgery.

This retrospective triple case report showed that innovative digital dentistry workflow could significantly limit complications stemming from LSFE surgery with subsequent molar implantation. Meneghetti and colleagues state, “The relevant digital workflow was described and supported by documentation of three clinical cases with patients experiencing successful grafting and implant placement procedures. However, further studies must be performed to determine if this technique would be generalizable, as only one operator performed all procedures. Additionally, more investigation could clarify if this method would translate to multiple implant placements and sites lacking posterior teeth.”

SCD is a group of inherited disorders marked by defects in hemoglobin, the protein that carries oxygen throughout the body. Abnormal hemoglobin causes oval-shaped red blood cells to take on a sickle-like shape. These misshapen red blood cells die much quicker than healthy cells and can block blood flow to different parts of the body, leading to complications such as severe pain, swelling in hands or feet, delayed growth and vaso-occlusive crisis (VOC), resulting in severe pain and sometimes death.

Altemia is Lobe’s proprietary formulation of emulsified docosahexaenoic acid ethyl ester (DHA) concentration designed for patients with SCD. It is a medical food for the replacement of DHA, the first in the U.S. designed specifically to support patients with SCD. Previous studies have demonstrated beneficial outcomes for patients while taking the primary component of Altemia under physician supervision, including a reduction in VOC and hospital visits and improvement in critical biomarkers of inflammation.1,2

Of the approximately 100,000 people in the U.S. with SCD, almost all (94%) are Black. SCD occurs in one out of every 365 Black births. The life expectancy for people with the most severe form of SCD is 30 years less than those without the disease. Introducing a novel dietary management approach to SCD like Altemia provides new options for vulnerable patient populations.

That scoping choice was motivated by a thesis that protein engineering, a high-growth branch of biotechnology (e.g., AlphaFold), has an outsize potential for climate impact but is underfunded. While protein engineering has the potential for impact in sustainability applications ranging from biomanufacturing to agriculture to mining, it falls into a climate tech funding gap: protein engineering efforts for sustainability are too application-focused to be funded as fundamental research, yet too risky to be funded as applied research or backed by private capital. Homeworld’s analysis of 177,962 National Science Foundation (NSF) grants and 188,487 Department of Energy (DoE)-funded publications showed that only 0.1% of NSF funding since 2009 and 1% of DoE-backed publications were for protein engineering in the context of climate. Beyond fast funding, Homeworld will provide a cohort and mentorship program for the applicants it funds and will educate the broader community by publicly sharing high-quality problem statements derived from the projects it funds. Through combined funding, community building, and education, Homeworld’s goal is for the Garden Grants program to direct the rapid growth of protein engineering technology toward sustainability.

“At this point, we’ve met with over 500 practitioners across the climate biotech ecosystem,” said Daniel Goodwin, Founding Co-Director of Homeworld Collective. “Creating funding resources — like our Garden Grants program — is critical, but not sufficient to support a thriving community in climate biotech. We need to go beyond financial resources and think about what makes a community productive, and make tools that improve the productivity of an entire field by supporting the needs of practitioners as fully as possible.”

One such tool that Homeworld Collective is focusing on is technology roadmap development and the work that it facilitates — the roadmaps identify the most promising and impactful research pathways in emerging areas of climate biotech. Homeworld Collective then connects biotechnologists with engineers in disparate disciplines who together have the requisite expertise to address the novel research challenges identified by the roadmaps. Once these problem-specific sub-communities are formed, Homeworld advocates for funding, whether to third parties or by creating new types of grants in the Garden Grants program. The result is a virtuous cycle where knowledge creation, community building, and funding come together for maximum impact. Homeworld Collective recently released Part 1 of its three-part Roadmap for Biotechnology in Carbon Dioxide Removal (CDR), an emerging interdisciplinary technology space that is anticipated to become one of the world’s largest industries by midcentury. As part of the roadmap, the organization identified a set of 13 actionable problems researchers can begin working on today in the field that the IPCC says will be a critical component of the transition to a decarbonized society.

The climate technology market is expected to grow at a CAGR of 24% through 2033 to a total market size of $183 billion. Climate biotechnology is a rapidly developing sub-field that leverages the power of cutting-edge biotechnology to develop a broad set of solutions to climate change, yet it is a field that is underdefined and under-resourced. In establishing Homeworld Collective, climate solutions — including sustainable manufacturing, efficient mining of critical minerals for the energy transition, the improvement of crop productivity and climate resistance, and the conservation and restoration of ecosystems — will be prioritized, funded, developed, and deployed at an accelerated pace, simultaneously elevating the field of climate biotechnology and the individuals innovating within it simultaneously.
