The Quinism Foundation and Veterans for Common Sense Call on Congress to Fund Research into Chronic Quinoline Encephalopathy


The Quinism Foundation and Veterans for Common Sense are calling on Congress to fund critical research into the chronic adverse neurological and psychiatric effects of mefloquine and related quinoline antimalarial drugs, following release of a report by an ad hoc committee of the National Academies of Sciences, Engineering, and Medicine (NASEM). The NASEM committee was charged by the U.S. Department of Veterans Affairs with looking at “long-term health effects” of antimalarial medication, with “special attention” “to possible long-term neurologic effects,” and “long-term psychiatric effects.” [1]

On February 25, 2020, the NASEM committee publicly released its final report. It observed a “disconnect between the level of concern raised–millions of people have used the drugs, and there are many known concurrent events and case reports of adverse events–and the systematic research that has been conducted, particularly in areas such as the use of mefloquine and persistent neurological or psychiatric outcomes.” The committee found “there is a sharp contrast between the abundant amount of literature pertaining to concurrent adverse events that are experienced while a drug is being used or shortly following its cessation and the dearth of information, especially high-quality information, pertaining to adverse experiences after the use of that drug has ended.” [1]

“To address the critical research needs identified by NASEM, Congress should establish chronic quinoline encephalopathy (neuropsychiatric quinism) as a perennial research topic under the Peer-Reviewed Medical Research Program (PRMRP) within the Department of Defense (DoD) Congressionally Directed Medical Research Program (CDMRP),” said Remington Nevin, MD, MPH, DrPH, executive director of The Quinism Foundation.

The Quinism Foundation and Veterans for Common Sense identified five focus areas for future research, including conducting high-quality epidemiological studies related to persistent health effects of antimalarial quinolines, particularly mefloquine.

“In a chapter titled ‘Improving the Quality of Research on the Long-Term Health Effects of Antimalarial Drugs,'” Dr. Nevin said, “the committee noted the importance of epidemiological study designs that ‘allow for the discovery of symptoms or diagnoses that covary. For example, if certain symptoms or diagnoses occur together in the same patients, there may be reason to consider a syndrome of “neuropsychiatric” symptoms that co-occur, rather than looking individually at separate neurologic or psychiatric experiences.’ The committee also noted that a ‘challenge when studying adverse events of drugs is that the occurrence of adverse events [e.g. prodromal psychiatric symptoms such as nightmares, insomnia, anxiety, depression, restlessness, and confusion during use of mefloquine (i.e., symptomatic exposure)] may cause an individual to decide to modify the dose, or even stop the drug completely, without consulting a health professional.'”

“This focus area should aim to fund epidemiological studies meeting the NASEM Committee’s criteria relative to persistent health effects of antimalarial quinolines, particularly mefloquine – to include valid assessment of symptomatic quinoline exposure and particularly symptomatic mefloquine exposure, and subsequent covarying symptoms or diagnoses consistent with the described presentation of chronic quinoline encephalopathy,” Dr. Nevin said.

Other focus areas identified include defining persistent central nervous system effects of antimalarial quinoline neurotoxicity, defining adverse neurophysiological effects of antimalarial quinolines, disentangling comorbid neuropsychiatric diagnoses confounded by antimalarial quinoline toxicity, and developing effective treatments.

Dr. Nevin said that, “this latter focus area should aim to develop treatable targets and treatments for patients with putative chronic quinoline encephalopathy – including those diagnosed with PTSD or TBI – to improve their health and lives.” Dr. Nevin said in this respect that, “although the committee wrote in its report that it held open sessions during their study to ‘listen to individual veterans and others, such as spouses and advocates, who are concerned about aspects of health that may be related to use of these antimalarial drugs’, the committee was prohibited from reviewing patient reports.”

“Had the committee been directed or allowed to review the medical records of affected patients, the unmet clinical needs of these individuals would have likely been readily apparent,” said Anthony Hardie, National Chair and Director of Veterans for Common Sense, a national veterans organization.

As the word “alliance” explains in its name, New Alliance Academy works very closely with the students’ families in order to achieve the students’ goals. The Global Recovery Initiatives Foundation (GRI) announced today the addition of four new members to its board of directors: international rock icon Nikki Sixx; entertainment industry legend Allen Kovac; Fors Marsh Group’s (FMG) Director of Special Initiatives Matt Escoubas, and iHeartMedia Vice President Alexandra Cameron.

About the New Board Members:

Nikki Sixx is a founding member and the bassist of the band Mötley Crüe (over 100 million albums sold worldwide) as well as the author of three New York Times bestselling books, including his memoir The Heroin Diaries, which will debut as a musical in 2020. He is 18 years sober and has been a dedicated recovery advocate, philanthropist, and public speaker. He is a frequent media contributor to CNN, Fast Company, MSNBC, The Washington Post, The Guardian, and Rolling Stone. He has also spoken to members of Congress for National Alcohol and Drug Addiction Recovery Month.

Allen Kovac, a 40-year veteran of the entertainment industry, founded Better Noise Music (AKA, Eleven Seven Music Group), Tenth Street Entertainment, and TDA Productions. He has developed and promoted a wide range of artists, including Tom Petty, Talking Heads, Willie Nelson, Waylon Jennings, Duran Duran, The Cranberries, Mötley Crüe, and Meatloaf.

In 2006, Allen founded Better Noise Music to give artists an integrated worldwide marketing platform and direct distribution with iTunes, Amazon, Apple Music, Spotify, YouTube, and Deezer. Better Noise has been named the #1 Active Rock label by Mediabase two years in a row (2018 and 2019).

In 2019, Allen was honored by Sir Richard Branson with the Innovator Award at the Association of Independent Music (AIM) Awards.

Allen has produced a number of programs that focus on SUD. He co-produced the highly successful Netflix biopic, The Dirt, based on the best-selling autobiography of the band Mötley Crüe. He also produced Sno Babies, a gripping story about a 16-year-old heroin-addicted honor student that shows how one can begin recovery; the movie is scheduled for release in 2020. In addition, he is the producer of The Heroin Diaries, a theater show based on the bestselling memoir of the same name by Nikki Sixx, which will run in 20 cities across the United States beginning in fall 2020.

Matt Escoubas is Director of Special Initiatives at Fors Marsh Group (FMG) where he oversees the company’s social purpose division tasked with addressing social challenges through in-kind, employee-led project teams. Throughout Matt’s career he has championed causes that strive to improve lives through evidence-based research. He brings more than 25 years of experience to his current role having led a wide array of public health and social change campaigns on behalf of federal and private sector clients. His work spans a range of topics, including opioid use disorder, addiction recovery, heart disease and stroke prevention, tobacco prevention, and pediatric palliative care. His efforts have received multiple industry awards, including a PR News Platinum Award, Bronze Anvil Award, two National Health Information Awards, and three Thoth Awards.

Alexandra Cameron is the Vice President of Strategic Partnerships & Government Initiatives for iHeart Media. She is also a co-founder of the National Opioid Action Coalition (NOAC), which has produced two events and multiple radio campaigns to break the stigma around SUD and has adopted GRI as its charity partner for SUD issues. Before joining iHeart Media, Alexandra led Keek, Inc., a startup social video platform that was one of the first video platforms for Facebook Messenger. At Keek, she was responsible for all business operations, sales, and marketing, and grew the number of registered users to 75 million.
