ISPE Unveils First of Its Kind Guidance Document and Launches New Community of Practice on Pharmaceutical Compounding


ISPE Guide: 503B Compounding – Regulatory Basis and Industry Good Practices for Outsourcing Facilities

In response to industry demands and regulatory complexities, ISPE developed this comprehensive guide to serve as an invaluable resource for 503B outsourcing facilities, aiding them in understanding the regulatory requirements outlined in the FD&C Act Section 503B. The Guide combines FDA regulations and recommendations with pharmaceutical industry standards, providing a go-to document for 503B facilities of all sizes. Various aspects of the compounding process are covered, from the importance of CGMPs to establishing a quality system (including qualifying suppliers and vendors), receipt of raw materials/active ingredients, and shipping of finished drug products. The Guide also provides recommendations for facility and equipment design, drawing from aseptic manufacturing practices and scaled to meet the needs of 503B facilities, and presents industry best practices for aseptic manufacturing, emphasizing personnel training and qualification.

The Guide addresses microbiological and analytical testing, including verifying the suitability of compendial methods and validating non-compendial methods. It covers beyond-use dating, offering essential insights into limited stability testing along with stability best practices. A dedicated chapter on preparing for regulatory inspections provides facilities with a valuable resource for this key aspect in ensuring compliance.

ISPE Community of Practice on Pharmaceutical Compounding

Recognizing the pressing need for best practice sharing, training, and guidance for creative problem solving in compounding pharmacies, ISPE launched a new ISPE Community of Practice (CoP) on Pharmaceutical Compounding. This CoP is designed to serve as a dynamic and collaborative community for ISPE members to exchange insights, share best practices, and engage in discussions related to pharmaceutical compounding through the ISPE Engage online platform.

The CoP seeks to foster innovation to improve the practice of pharmaceutical compounding, and to disseminate ideas, knowledge, and best practices through generation of ISPE content, including guidance documents, Pharmaceutical Engineering® Magazine articles, webinars, blog posts, conference presentations, and training materials. The new CoP will provide a venue for industry and regulatory informal interactions to drive practical and effective design and operational practices, while addressing regulatory expectations and providing solid scientific justification for practices accepted by industry and regulators alike.

Martin Searle Solicitors run their annual October 2023 campaign “Disability Matters” to stamp out Disability Discrimination in the Workplace


A 2020 study carried out by Citizens Advice found that disabled people were twice as likely to face redundancy at work. The Office of National Statistics (ONS) also found that disabled people earn 13.8% less on average than non-disabled people – almost 2 less per hour. The disability employment gap, which shows the difference between the employment rates of disabled and non-disabled people, begun widening during the Coronavirus pandemic, and is how at 29.8% – the widest point since 2018. Although the measures introduced by the government at the height of the pandemic have now been dropped, in January 2023 the ONS estimated that two million people (3.1% of the population) were experiencing long COVID symptoms, with 19% of these reporting that their ability to undertake day-to-day activities had been “limited a lot” since the onset of symptoms.

As with Chronic Fatigue Syndrome, employers are often unaware of their responsibilities to employees who are experiencing long COVID, and may not realise that their employees are disabled under the Equality Act, and that they could be inadvertently discriminating against them at work.

New ‘Plutus Health Revenue Cycle Management Challenges Index, 2023’ offers insights into healthcare revenue management


“Our new ‘Plutus Health Revenue Cycle Management Challenges Index, 2023‘ shows how big of a burden insurance denials are for healthcare providers. With 40% of survey respondents reporting that they are losing more than half a million dollars in annual revenue every year simply because of insurance denials, the scale of the problem is immense,” said Thomas John, Founder and CEO of Plutus Health, Inc.

“Despite AI [artificial intelligence] and RPA [robotic process automation] offering proven solutions, providers still seem hesitant to implement these new technologies into their RCM process. We hope this survey shows just how urgent it is for the healthcare industry as a whole to embrace these new technologies, which have the potential to drastically increase revenue for healthcare providers across the country,” he said.

Claim denials stood out as the top challenge for providers — and it’s costing them a lot of money. More than 40% of the healthcare providers we surveyed said they lose more than half a million U.S. dollars in annual revenue each year due to denied insurance claims. And 18% reported losing more than a million U.S. dollars annually.

Nearly 60% of respondents said that dealing with insurance denials was one of their most significant RCM challenges, and 42% said denial management was one of the stages in their RCM process that is regularly due to staff shortages.

So, it’s no surprise that 43% of respondents said denial management was among their greatest priorities in 2023. And 20% of respondents noted that achieving clean claim rates and lower denial rates were also a top key performance indicator (KPI) that the RCM teams focused on in 2023.

Managing staffing needs also causes stress for healthcare providers, with about 40% of respondents reporting it as one of their most significant challenges. Nearly half of the respondents also revealed that they rely on external support for RCM, either using a combination of internal and external teams or outsourcing to one or more vendors.

Healthcare providers who’ve tried new technologies — such as RPA and AI — have seen improvements in their overall RCM process.

Nearly 20% of the respondents who use AI and RPA said the greatest improvement since implementing the technology was improved efficiency in filing claims, and 18% reported reduced data-entry errors.

Nearly 30% of respondents who used AI and RPA said it resulted in faster cash flow and collections, while about 20% said it resulted in more insightful and readily accessible business analytics.
