Through the evolved partnership, which began in 2018, Forcura and HCHB will help providers better accept patients into care, maximize efficiencies and strengthen their market positions.

The two companies are collaborating to ensure that Forcura’s newly enhanced referral management solution, available to the general market this July, is fully integrated with the patient chart in HCHB. Most notably, the solution will act as a referral hub, featuring functionality that enables clients to manage all referrals in one place regardless of referral source and will allow key patient referral data to automatically populate in both Forcura and HCHB systems.

This added functionality, powered by artificial intelligence and machine learning, will help to streamline care transitions for patients and provide clients increased visibility, faster processing and smarter insights on their business performance and referral partner value.
