SAC Health announced the award of a $2.139 million grant from the California Department of Health Care Services that will help increase the healthcare management of those who are in most need throughout the region. Through the following programs: ​California Advancing and​Innovating Medi-Cal (CalAIM) Initiative; Providing Access and Transforming Health (PATH); Capacity and Infrastructure Transition, Expansion and Development (CITED) Initiative, SAC Health will increase the number of Enhanced Care Management teams by four, resulting in 10 teams, serving over 2,000 patients at any given time.

Enhanced Care Management is a whole-person, interdisciplinary approach to care that addresses the clinical and non-clinical needs of eligible Medi-Cal Members who have the most complex medical and social needs. It allows for a systematic coordination of services and comprehensive care management that is personal, community-based, and multi-disciplinary. SAC Health implemented Enhanced Care Management in (given year) through funding from Inland Empire Health Plan (IEHP).
