Demand for vasectomy services has increased significantly after the overturning of Roe V Wade. Men seeking vasectomy are encountering long wait times and expensive procedures. Vasectomy training in the United States is not adequate and creates unnecessary barriers for patients seeking permanent male contraception. A Raleigh Ob/Gyn has created a vasectomy training program to address the deficiencies in vasectomy provision.

Vasectomy providers have seen a surge in requests for vasectomy appointments and some men are having to wait up to six months to have a procedure. This surge in demand is primarily composed of men without children and couples who fear limited options should unplanned pregnancy occur. Vasectomy training in the US is inadequate to meet the increased demand. Men are having to wait up to six months to have a vasectomy procedure. As a result of the failure of the Affordable Care Act to mandate vasectomy coverage, patients are frequently having to pay for the entire procedure. The cost for vasectomy can range from $600 to $5,000. Provision of vasectomy is monopolized by urology specialists and this creates unnecessary barriers for patients seeking vasectomy.
Dr. Charles Monteith is an Obstetrician Gynecologist in Raleigh, North Carolina who is a ‘non-traditional’ provider of vasectomy. As a result of the overturning of Roe v. Vasectomy Training In The United States Is Inadequate
In most of the developed world, family physicians perform the majority of vasectomy procedures. In the United States, the majority of vasectomy procedures are performed by urologic specialist. Most vasectomy training occurs during residency. Urology programs provide most of the training and there are no formal vasectomy training programs for vasectomy after residency.
There are three major problems with vasectomy training in the US:

Vasectomy provision is monopolized by urologic specialist. A 2002 survey of vasectomy providers in the US revealed 79% of vasectomy procedures are performed by urologists and only 13% of vasectomy procedures are performed by family physicians (1). A 2019 survey revealed most urologist were unwilling to train non-urologist to perform vasectomy (2). - Newly trained urologic specialist are not comfortable performing office based vasectomy. A 2021 survey of urology training physicians revealed they were significantly less comfortable performing vasectomy in the office setting versus in the operating room. They blamed low numbers of vasectomy training cases and a lack of autonomy as barriers to their vasectomy training (3).
- Most primary care physicians are not provided vasectomy training during residency. A 2021 survey of US Primary Care Residency programs revealed fewer than 10% of Family Medicine residency programs offer vasectomy procedural training (4)
Limiting vasectomy training to urologic specialist decreases patient access to vasectomy and increases the cost of vasectomy. Not only are urologist unwilling to train non-urologist but newly trained urologist are not comfortable offering office based vasectomy. Urologist who are uncomfortable providing office based vasectomy either will not perform the procedure, will perform limited numbers of procedures, or will only perform procedures in an operating room setting. This greatly increases the cost of vasectomy.
A lack of vasectomy training for primary care physicians and the poor vasectomy training of urology residents limits access to vasectomy. This creates a bottle neck shortage for what is ultimately a simple preventative health care procedure that is highly effective at preventing unplanned pregnancy.

Four Week Vasectomy Training Program With Dr. Monteith
To increase the number of skilled vasectomy providers, Dr. Monteith has created a minimally invasive vasectomy training mentorship for primary care physicians.
