HealthTeamWorks launches Primary Care First Support Program

HealthTeamWorks, the Colorado-based, non-profit national leader in medical practice transformation, announced formation of its Primary Care First (PCF) Practice Support Program, a low-cost service suite designed to improve practice capabilities and boost performance in the new Primary Care First models announced in April by the Centers for Medicare & Medicaid Services. The practice support services include online coaching consultation, peer learning forums, and in-person collaborative learning to shared best practices that help practices optimize total Medicare payment under the PCF models.

HealthTeamWorks is also offering a free PCF Revenue Calculator Tool designed to help primary care practices better understand, prepare for and ultimately succeed in the PCF payment model. The tool uses Medicare Fee for Service revenue modeling as a comparator to further inform practices’ PCF business decision-making and planning. Free webinars demonstrating how to use the tool are scheduled for November 26 and December 11.

According to CMS, Primary Care First rewards practices for meeting quality and cost-of-care outcomes targets. PCF payment model options provide opportunities for advanced primary care practices to increase total Medicare payment and promise simpler reporting burdens.

CMS will provide data and resources to guide the operational and care delivery aims that practices need to achieve to thrive in Primary Care First. HealthTeamWorks’ new PCF support services complement and supplement CMS’ resources and include a practice readiness assessment, coaching, and technical support that will not be provided by CMS.

HealthTeamWorks PCF Support services are delivered virtually, so they’re both convenient and affordable. Practices collaborate during small-group coaching sessions and monthly topic-specific learning meetings. Flexible pricing includes a shared-risk option with a reduced cost to enroll.
